Immunologic Biomarkers for Clinical and Therapeutic Management of Psoriasis

Background. The therapeutic management of psoriasis includes conventional treatments as well as the new generation of highly effective TNF-alpha inhibitors. However, psoriasis has proven to be a complex therapeutic challenge and treatment failures are not uncommon. Thus, laboratory biomarkers of disease progression/therapeutic efficacy may greatly help in the clinical management of psoriasis. Aims. To identify laboratory biomarkers for clinical management and therapeutic monitoring of psoriasis. Methods. An observational study performed on 59 patients, presenting moderate to severe psoriasis, undergoing treatment with anti-TNF-alpha agents (etanercept, adalimumab, and infliximab). Soluble and cellular immune/inflammatory parameters were assessed at baseline and after 12 and 24 weeks of treatment. Results. Clinical efficacy was achieved in 88% of the subjects at 12 weeks, reaching 90% after 24 weeks. IL-6 and IL-22, which were elevated at baseline, were significantly reduced, in association with a significant decrease of CLA+ T cells and an increase of Treg lymphocytes. T, B, and NK cell subsets and T cell response to recall antigens did not show any evidence of immune suppression. Conclusions. Immune/inflammatory parameters including IL-6 and IL-22, CLA+ T cells, and Treg lymphocytes may prove to be valuable laboratory tools for the clinical and therapeutic monitoring of psoriasis.

Psoriasis in difficult to treat areas: treatment role in improving health-related quality of life and perception of the disease stigma

Summary: This study evaluated the impact of psoriasis on different body sites on the quality of life of patients and observed changes in quality of life during treatment. The results showed that itching acts as a mediator between the extension of psoriasis and its impact on quality of life, and nail psoriasis plays a predominant role in affecting the quality of life of patients.

Coesisting inflammatory skin diseases: Tildrakizumab to control psoriasis and omalizumab for urticaria

Summary: The number of individuals suffering from autoimmune conditions is increasing in Western countries, and they are often prone to developing a second autoimmune disorder. This case report demonstrates the successful treatment of a patient with psoriasis vulgaris and chronic spontaneous urticaria using a combination of two different antibodies. The study suggests that the two therapies may act synergistically to achieve a favorable outcome.

This case report highlights the importance of considering lifestyle and cutaneous background before starting immunotherapy for cancer treatment, with a focus on pharmacogenomics and the possibility of modified skin microbiota predisposing to cutaneous infections in patients treated with PD-1 inhibitors.

Role of Neutrophil-to-Lymphocyte Ratio (NLR) in Patients with Mycosis Fungoides

Summary: The neutrophil/lymphocyte ratio (NLR) is found to be associated with the severity and prognosis of various cancers, but its role in mycosis fungoides (MF) is unclear. This study aimed to assess the association of NLR with different stages of MF and determine if higher values are related to a more aggressive MF. The retrospective analysis of 302 MF patients found that higher NLR values were significantly associated with advanced stages of MF, suggesting that NLR can serve as a marker for advanced MF, guiding physicians in patient management.

A Real-Life Study on the Use of Tildrakizumab in Psoriatic Patients

Summary: Tildrakizumab is an approved treatment for moderate-to-severe plaque-type psoriasis, targeting the p19 subunit of interleukin IL-23. A study was conducted on 53 psoriatic patients who received Tildrakizumab every 12 weeks for 52 weeks, showing significant improvement in Psoriasis Area and Severity Index (PASI), Dermatology Life Quality Index (DLQI), Nail Psoriasis Severity Index (NAPSI), and Palmoplantar Psoriasis Physician Global Assessment (PPPGA). Disease remission was achieved from week 16 and maintained until week 52.

The Diagnosis and Management of Cutaneous Metastases from Melanoma

Summary: Melanoma, the deadliest form of skin cancer, accounts for almost 90% of skin cancer mortality. Despite advances in immune and targeted therapies, many patients still experience disease progression. Skin metastases, a common occurrence in melanoma, can be treated through various methods including surgery, injections, and systemic therapies. Further research is needed to understand the underlying mechanisms and differences in behavior and prognosis among melanoma patients with skin metastases.

Epithelial-to-Mesenchymal Transition Gene Signature in Circulating Melanoma Cells: Biological and Clinical Relevance

Summary: The most promising method for monitoring patients with minimal morbidity is the detection of circulating melanoma cells (CMCs). We have shown that CD45(-)CD146(+)ABCB5(+) CMCs are a rare primitive stem/mesenchymal CMCs population associated with disease progression. Investigating the gene expression of isolated CMCs' epithelial-to-mesenchymal transition (EMT) gene signature can provide potential clinical value.

Tissue factor as a potential coagulative/vascular marker in relapsing-remitting multiple sclerosis
