Hip replacement: Landmark surgery in modern medical history

Total hip replacement (THR) is most often performed to treat end-stage symptomatic osteoarthritis. Patients typically present with increasing pain, restricted mobility and stiffness. In this procedure, the femoral head and part of the femoral neck are excised. The acetabulum is enlarged and an acetabular cup is inserted. The femoral head is replaced by a femoral component, the stem of which is inserted into the medullary canal of the femur. The components can be either cemented in place or press-fit (cementless). The THR concept was popularised by Sir John Charnley in the 1960s and although, over half a century of development has resulted in incremental improvements, the procedure is not dramatically different from the one he described. However, over the last two decades there have been significant changes in the types of bearing surfaces used. Metal on polyethylene continues to be the workhorse for the majority of cases. In the young and active, bearing surfaces with low wear rate are increasingly used. Since the early 1960s, THR has played an important role in alleviating pain and restoring mobility to millions of people. The cost-effectiveness of THR in treating advanced osteoarthritis makes it one of the most successful of all surgical interventions. (C) 2013 Elsevier Ireland Ltd. All rights reserved.

The effect of age on the outcomes of cementless mobile bearing unicompartmental knee replacements

Summary: The study demonstrates that cementless mobile bearing UKR has low reoperation and revision rates in all age groups, with similar functional outcomes. Cementless UKR should be considered for patients of all ages without age being a contraindication.

The effect of body mass index on the outcomes of cementless medial mobile-bearing unicompartmental knee replacements

Summary: The study investigated the effect of body mass index (BMI) on the outcomes of cementless unicompartmental knee replacements (UKRs). The results showed that the 10-year reoperation and revision rates of cementless UKRs were low in all BMI groups, with no significant differences between the groups. Although the higher BMI groups had slightly worse functional outcomes, the improvement in function tended to be better compared to preoperative state.

Comparison of five-year clinical outcomes of 524 cemented and cementless medial unicompartmental knee replacements

Summary: This study compared the outcomes of cemented and cementless Unicompartmental Knee Replacements (UKR) at 5 years post-surgery. The results showed that cementless UKR had better clinical outcomes, particularly in terms of pain relief, compared to cemented UKR. Both types of replacements were safe with low reoperation and complication rates, and a 5-year survival rate of 99%.

The Oxford Domed Lateral Unicompartmental Knee Replacement implant: Increasing wall height reduces the risk of bearing dislocation

Summary: The study aimed to reduce the risk of dislocation in Oxford Domed Lateral replacements by using a robotic path-planning algorithm to determine the Vertical Distraction needed for Dislocation. Results showed that increasing bearing thickness can increase the risk of medial dislocation, while increasing wall height and keeping the bearing closer to the tibial wall can decrease the risk of dislocation in other directions.

Application of a robotics path planning algorithm to assess the risk of mobile bearing dislocation in lateral unicompartmental knee replacement

Summary: This study aimed to develop a reliable and efficient tool for assessing bearing dislocation risk. A novel robotics dislocation analysis tool was developed by modifying a robotics software package and using a robotics path planning algorithm. The results of this tool showed excellent agreement with the mechanical rig, indicating its reliability and validity.

Total and partial knee arthroplasty implants that maintain native load transfer in the tibia

High rate of radiolucent lines following the cemented original design of the ATTUNE total knee arthroplasty

Summary: The aim of this study was to assess the rate of radiolucent lines (RLLs) formation in the cemented original design of the ATTUNE total knee arthroplasty (TKA) and their relationship to loosening. A systematic search was conducted and 12 studies were included in the meta-analysis. The results showed high rates of RLLs in the ATTUNE TKA, but low rates of loosening and revision. Close surveillance of patients who have undergone TKA with the original ATTUNE system is recommended.

Osteochondral Allografts for Large Osteochondral Lesions of the Knee Joint: Indications, Surgical Techniques and Results

Summary: The main indications for osteochondral allograft transplantation of the knee are full-thickness cartilage lesions, deep lesions with subchondral damage, and failed previous surgical procedures. The success of the transplantation depends on factors such as the type of lesion and age of the patient. OCA transplantation has shown a 75% success rate at 12 years follow-up.
