Serum miR-142-3p is associated with early relapse in operable lung adenocarcinoma patients

The outcome of resectable non-small cell lung cancer (NSCLC) is critically determined by metastatic spread: About 30-50% of early-stage NSCLC patients encounter tumour recurrence within 5 years after surgery. A biomarker-driven stratification of early-stage lung cancer with a high risk of recurrence may improve therapy management and patient care. The aim of this study was to identify microRNAs (miRNAs) in serum of patients associated with early relapse in pulmonary adenocarcinoma. Serum samples were collected from 204 patients before surgery. miRNA screening was done using qRT-PCR based low-density arrays (664 miRNAs) comparing adenocarcinoma patients (n = 40) with and without recurrence 24 months after surgery. Selected miRNAs associated with disease recurrence were Validated in an independent patient cohort (n = 114). miRNAs were also measured in advanced adenocarcinoma patients (n = 29), and individuals with benign pulmonary diagnosis (n = 21). Circulating miR-142-3p (p = 0.005) and miR-29b (p = 0.01) were identified in the screening and confirmed in the validation cohort to be increased in sera of early-stage adenocarcinoma patients suffering from recurrence within 24 months. Elevated miRNA levels were exclusively observed in the group of high-risk patient diagnosed for operable adenocarcinoma compared with benign diagnosis or advanced tumour disease. The differentiation between pulmonary adenocarcinoma patients with low and high risk for recurrence was improved by accounting for both miR-142-3p levels and tumour stage (p = 0.007; AUC = 0.78). In conclusion, circulating miR-142-3p was found to be associated with a high risk of recurrence in early-stage lung adenocarcinoma patients, and a putative serum marker for risk assessment. Computed tomographic airway morphology after targeted lung denervation treatment in COPD

Summary: This post-hoc analysis investigated the changes in airway CT-parameters after targeted lung denervation (TLD) and their association with treatment response. Improvement in air trapping was significantly associated with an improvement in residual volume (RV) in the treatment group. Furthermore, improvements in Pi10 and airway lumen were significantly associated with improvements in both RV and FEV1. Findings suggest that improving airway characteristics leads to reduced air trapping and improved clinical outcomes.

Summary: This study examined the relationships between smoking habit, clinical stage, and sex with age at diagnosis of lung adenocarcinoma. The results showed that smoking, advanced stage, and female sex were associated with younger age at diagnosis. This study provides new insights into the clinical variables influencing age at diagnosis and opens up possibilities for further research on the genetic and molecular mechanisms responsible for these associations.

Summary: Hypoxia-inducible factor 1 alpha (HIF-1a) is a transcription factor that regulates the cellular response to hypoxia and is upregulated in all types of solid tumor. Hepatocellular carcinoma (HCC) is a highly vascular and hypoxic tumor, and treatments like TACE and TAE can exacerbate hypoxia in HCC. Hypoxia-activated prodrugs (HAPs) have potential as a targeted treatment for hypoxic HCC, but more research is needed to understand their effects in combination with embolization.

Summary: Diagnostic testing is crucial in medicine, but there is significant variation in the methodology, definitions, and reporting of results in studies of diagnostic testing in respiratory medicine, leading to conflicting or ambiguous results. To address this issue, a group of 20 respiratory journal editors developed reporting standards for such studies, providing a rigorous methodology for authors, peer reviewers, and researchers. The standards cover four key areas, including defining the reference standard of truth, measures of test performance for dichotomous outcomes, measures of test performance for multichotomous outcomes, and a useful definition of diagnostic yield. The importance of using contingency tables for reporting results is emphasized, along with a practical checklist for reporting diagnostic testing studies.
