Summary: This study examined the effects of nutritional therapy on patients with liver cirrhosis and associated minimal hepatic encephalopathy (MHE). The results showed that nutritional therapy effectively improved cognitive functions, health-related quality of life, anthropometry, endotoxins, and inflammatory markers in these patients.

Summary: This study investigated changes in sub-regional thalamic functional connectivity after liver transplantation in cirrhotic patients with and without a history of overt hepatic encephalopathy (OHE). The results showed that the renormalization of FC in the cortico-thalamic loop might be a neuro-substrate for the recovery of cognitive function after LT in cirrhotic patients, and hyperconnectivity between thalamic subregions and the inferior parietal lobe might be an important feature of OHE. Changes in FC in the thalamus could potentially serve as biomarkers for the recovery of cognitive function in cirrhotic patients after LT.

Summary: Hepatic encephalopathy is a severe metabolic syndrome associated with liver disorders, with limited treatment options available. The disease is a neuropsychiatric complication linked with cognitive decline, coma, and death, and its pathogenesis remains unclear. Melatonin, as the sole endogenous hormone related to HE, has neuroprotective effects by crossing the blood-brain barrier.

Summary: This study evaluated the role of presepsin in the early diagnosis of infections in patients with liver cirrhosis (LC) and hepatic encephalopathy (HE). The results showed that presepsin levels >980 pg/mL could enhance the suspicion of bacterial infections, and it may be an adequate non-invasive tool for the early diagnosis of infections in these patients.

Summary: This study found that selenium deficiency is highly prevalent in patients with chronic liver disease (CLD) and is associated with an increased risk of overt hepatic encephalopathy (OHE) development. The serum selenium levels were lower in patients with cirrhosis compared to those with chronic hepatitis, and they were negatively correlated with several disease severity indicators. The ALBI score remained significantly associated with selenium deficiency, and selenium deficiency was associated with the occurrence of OHE during the follow-up period.

Summary: Neurocognitive impairment is present in cirrhosis, but liver transplantation can partially reverse this damage. This study found significant disrupted connectivity in cirrhotic patients, which improved after transplantation, showing potentially different recovery trajectories for patients with and without overt hepatic encephalopathy (OHE).

Summary: A dynamic nomogram has been developed to predict the risk of 30-day transplant-free mortality in patients with OHE and HBC. The MELD score and NLR are independently associated with the 30-day mortality. The nomogram shows good predictive accuracy and can guide clinical decision-making.

Summary: The systematic review and meta-analysis showed that albumin infusion is beneficial in the treatment and prevention of hepatic encephalopathy. Albumin infusion was associated with reduced risk of OHE and inpatient mortality. Further well-powered randomized trials are needed to confirm these findings.

Summary: The study demonstrated that rifaximin significantly reduces the incidence of overt HE after TIPS, with good tolerability. However, the results primarily apply to patients with alcoholic cirrhosis, and further research is needed to evaluate the effects on other types of cirrhosis.

Summary: Hepatic encephalopathy (HE) is a frequent and severe complication of liver disease that often goes undiagnosed due to a lack of understanding and consensus on diagnosis. Non-specific symptoms and comorbidities in cirrhotic patients make differential diagnosis challenging. Guidelines are provided to assist clinicians in diagnosing and treating HE. Liver transplantation may be indicated for HE, but its reversibility post-transplantation and the consequences in patients with other neurological disorders remain controversial.

Summary: The study investigated the impact of body mass index (BMI) on the occurrence of overt hepatic encephalopathy (OHE) after the transjugular intrahepatic portosystemic shunt (TIPS) procedure in decompensated cirrhotic patients. A retrospective observational cohort study of 145 cirrhotic patients receiving TIPS was conducted, and the relationship between BMI and clinical outcomes, as well as risk factors for post-TIPS OHE, was analyzed. The results showed that overweight/obese patients had a higher frequency of OHE compared to normal weight patients, and overweight/obesity and older age were identified as independent risk factors for post-TIPS OHE according to the logistic regression analysis. The study concludes that overweight/obesity and older age may increase the risk of post-TIPS OHE in cirrhotic patients.
