Summary: Organ transplantation is an effective treatment for end-stage organ failure, but post transplantation diabetes mellitus (PTDM) is a severe complication that affects the survival of recipients. However, PTDM is often overlooked or poorly managed in its early stage. This article provides an overview of the incidence, pathogenesis, and risk factors for PTDM, aiming to improve the quality of life of recipients.

Summary: The risk factors for late-onset Pneumocystis jirovecii pneumonia (PCP) after liver transplantation (LT) have been analyzed in this study. Age >= 65 years at LT, cytomegalovirus infection, steroid pulse therapy, hepatocellular carcinoma recurrence, and lymphocytopenia were identified as independent risk factors for PCP. Targeted prophylaxis considering these risk factors could improve the prevention of this potentially lethal complication.

Summary: The value of follow-up blood culture (FUBC) in the management of Gram-negative bacteremia (GNB) is controversial. This study evaluated bedside risk predictors and found that hemodialysis, fever on the day of FUBCs, intravascular device, no use of in vitro active antibiotic within 24 h, non-fermenting bacteria, and multidrug resistance were independent risk factors for positive FUBCs in GNB. The yield of FUBCs increased as the number of bedside risk predictors increased, and positive FUBCs were significantly associated with 30-day mortality.
