Anatomical variations of left hepatic vein and outflow reconstruction techniques in pediatric living donor liver transplantation

Summary: Anatomical variations in the left hepatic vein are common in left lateral segment (LLS) donors, but there is a lack of studies and algorithms for customized outflow reconstruction in LLS grafts with variant anatomy. A database analysis of 296 pediatric LLS living donor liver transplantation cases identified three types of left hepatic vein anatomy. Postoperative outcomes between LLS grafts with single and reconstructed multiple outflows showed no significant differences in hepatic vein thrombosis/stenosis, major morbidity, and 5-year survival. This classification and proposed schema provide a simple and effective tool for preoperative donor assessment and yield consistently reproducible outcomes.

Two patients experienced anastomotic restenosis, while one patient had an anastomotic perforation due to balloon dilatation. The mean follow-up period was 34 months, suggesting that MCA without general anesthesia is a safe and feasible alternative to surgery in gastrointestinal obstruction cases.

The study concluded that LMH is safe and feasible in elderly patients with HCC.

Summary: Research shows that metastatectomy can significantly improve survival in pancreatic ductal adenocarcinoma (PDAC) patients with isolated lung metastasis compared to those receiving only chemotherapy or supportive treatment. This finding is consistent with external validation. It is recommended to consider multimodal therapy with chemotherapy and surgical treatment for PDAC patients with isolated lung metastasis.
