Summary: This study compared short-term and long-term outcomes between laparoscopic major hepatectomy (LMH) and open major hepatectomy (OMH) in elderly patients with HCC, showing that LMH had lower postoperative complications and shorter hospital stay compared to OMH, with comparable mortality rates at 90 days and overall survival rates at 1, 3, and 5 years. The study concluded that LMH is safe and feasible in elderly patients with HCC.

Summary: The European deprivation index (EDI) does not impact overall survival after liver transplantation (LT) for hepatocellular carcinoma (HCC). However, the number of HCC nodules and time spent on the waiting list are independent prognostic factors for survival after LT for HCC.

French survey on a cohort of emergency general surgery modifications induced by lockdown of the SARS-CoV-2 pandemic

Clement Julien, Pauline Duconseil, Diane Mege, Damien Massalou, Todor Ivanov, Raffaele Brustia, Ghislain Pauleau, Stephane Berdah, Thierry Bege

Summary: The study found that many hospitals in France experienced a significant reduction in surgical activities during the COVID-19 pandemic, and deviations in management were observed among emergency general surgery patients. Additionally, medical delays and a shift from surgical to medical treatment were associated with a loss of chance for patients.

Summary: In elderly patients (> 70 years) with HCC in Milan criteria, surgical resection provided better long-term oncological outcomes compared to radiofrequency ablation, despite a longer length of stay and a higher rate of severe postoperative complications.

Summary: This study compared the effectiveness of LLR and RFA in elderly patients with single HCC smaller than 3 cm. The results showed that despite the longer length of hospital stay and operative time, LLR had a comparable postoperative course and better overall and disease-free survival in patients with HCC located in the anterolateral segments.

Can Surgical Resection of Metastatic Lesions Be Beneficial to Pancreatic Ductal Adenocarcinoma Patients with Isolated Lung Metastasis?

Won-Gun Yun, Wooil Kwon, Youngmin Han, Hee Ju Sohn, Hyeong Seok Kim, Mirang Lee, Hongbeom Kim, Alexander S. Thomas, Michael D. Kluger, Jin-Young Jang

Summary: Research shows that metastatectomy can significantly improve survival in pancreatic ductal adenocarcinoma (PDAC) patients with isolated lung metastasis compared to those receiving only chemotherapy or supportive treatment. This finding is consistent with external validation. It is recommended to consider multimodal therapy with chemotherapy and surgical treatment for PDAC patients with isolated lung metastasis.

Paradigm shift: should preoperative 3D reconstruction models become mandatory before hepatectomy for hepatocellular carcinoma (HCC)? Results of a multicenter prospective trial

Emanuele Felli, Emmanuel Boleslawski, Daniele Sommacale, Olivier Scatton, Raffaele Brustia, Lilian Schwarz, Daniel Cherqui, Thomas Zacharias, Alexis Laurent, Jean-Yves Mabrut, Catherine Schuster, Benoit Gallix, Patrick Pessaux

Summary: This study analyzed the role of preoperative 3D reconstruction models in selecting the surgical approach and modifying the established plan for hepatocellular carcinoma patients. The results showed that 3D imaging had a better prediction of the actual surgical procedure compared to conventional 2D imaging.
