Summary: This study investigated changes in sub-regional thalamic functional connectivity after liver transplantation in cirrhotic patients with and without a history of overt hepatic encephalopathy (OHE). The results showed that the renormalization of FC in the cortico-thalamic loop might be a neuro-substrate for the recovery of cognitive function after LT in cirrhotic patients, and hyperconnectivity between thalamic subregions and the inferior parietal lobe might be an important feature of OHE. Changes in FC in the thalamus could potentially serve as biomarkers for the recovery of cognitive function in cirrhotic patients after LT.

Summary: This study found that selenium deficiency is highly prevalent in patients with chronic liver disease (CLD) and is associated with an increased risk of overt hepatic encephalopathy (OHE) development. The serum selenium levels were lower in patients with cirrhosis compared to those with chronic hepatitis, and they were negatively correlated with several disease severity indicators. The ALBI score remained significantly associated with selenium deficiency, and selenium deficiency was associated with the occurrence of OHE during the follow-up period.

Summary: This study evaluated the role of presepsin in the early diagnosis of infections in patients with liver cirrhosis (LC) and hepatic encephalopathy (HE). The results showed that presepsin levels >980 pg/mL could enhance the suspicion of bacterial infections, and it may be an adequate non-invasive tool for the early diagnosis of infections in these patients.

Summary: This study explored the relationship between preoperative serum IL-6 levels and the risk of overt HE (OHE) after TIPS. The results showed that preoperative IL-6 was positively associated with a higher risk of OHE after TIPS, with higher predictive power compared to other indexes. Age and IL-6 were identified as independent risk factors for OHE after TIPS, and IL-6 was also a risk factor for coma in OHE patients.

Summary: Alcoholic cirrhosis and notable hydrothorax are independent risk factors for post-TIPS OHE in liver cirrhosis, together with the stent diameter of 10 mm, PPG decrement >= 60%, MELD score >= 10 and blood ammonia. The established risk score is reliable to identify high-risk individuals of developing post-TIPS OHE.

Summary: The study investigated the impact of body mass index (BMI) on the occurrence of overt hepatic encephalopathy (OHE) after the transjugular intrahepatic portosystemic shunt (TIPS) procedure in decompensated cirrhotic patients. A retrospective observational cohort study of 145 cirrhotic patients receiving TIPS was conducted, and the relationship between BMI and clinical outcomes, as well as risk factors for post-TIPS OHE, was analyzed. The results showed that overweight/obese patients had a higher frequency of OHE compared to normal weight patients, and overweight/obesity and older age were identified as independent risk factors for post-TIPS OHE according to the logistic regression analysis. The study concludes that overweight/obesity and older age may increase the risk of post-TIPS OHE in cirrhotic patients.

Summary: This study aimed to evaluate the association between large paraumbilical vein (L-PUV) and the occurrence of overt hepatic encephalopathy (OHE) after transjugular intrahepatic portosystemic shunt (TIPS). The results showed that patients with L-PUV had a significantly higher 2-year OHE rate, and L-PUV was the only independent risk factor for the incidence of 2-year OHE.
