Early and late postoperative changes in the quality of life after pancreatic surgery

To compare health-related quality of life (QoL) before and after surgery for pancreatic disease. A retrospective analysis of prospectively gathered data is presented. A total of 174 patients of 230 planned for pancreatic surgery between March and December 2009 at a German high-volume center completed the Short Form-36 (SF-36) Health Survey preoperatively, 133 of them at 3 months and 83 at 24 months after surgery. Data was analysed according to diagnosis and procedure, and compared to German population norms. QoL in the study group was worse than that of age-matched healthy population at all time points. It decreased continuously in the cancer group, decreased early and showed a trend toward recovery late in patients with benign tumors and chronic pancreatitis. Distal pancreatectomy was the best tolerated and total pancreatectomy the worst tolerated procedure. Older age and development of pancreatic insufficiency affected negatively QoL. In patients with pancreatic disease, diagnosis determined QoL preoperatively and late after surgery, while in the early postoperative period, type and extent of surgery was the leading factor. Total pancreatectomy had a profound negative effect on QoL and should be reserved for carefully selected patients only.

Preoperative evaluation of CT angiograms, awareness of any anomalous arterial anatomy of the upper abdomen, and meticulous surgical technique are crucial for achieving oncologically radical surgery without compromising the arterial liver supply.

Long-term survival after pancreaticoduodenectomy in patients with ductal adenocarcinoma of the pancreatic head

Summary: In this study, the overall rate of long-term survival (LTS) and its contributing factors after pancreatic ductal adenocarcinoma (PDAC) surgery were investigated. Clinical and laboratory parameters such as carbohydrate antigen 19-9 levels, tumor grade, and lymph vessel invasion were found to be significantly associated with LTS. The study concluded that cancer related characteristics play a role in predicting LTS in multimodally treated patients after curative PDAC surgery.

Risk factors for perforated marginal ulcers following pancreaticoduodenectomy and prospective analysis of marginal ulcer development

Summary: Surgery for symptomatic patients with serous cystic neoplasms (SCN) of the pancreas is considered reasonable and safe. The main reason for futile resections of benign asymptomatic SCN is preoperative diagnostic uncertainty about suspected malignancy, therefore surgery for supposed SCN without symptoms is justified only in carefully selected patients with suspected malignancy.

The number needed to vaccinate (NNV) to prevent COVID-19-related deaths is more favorable in surgical patients compared to the general population. Prioritizing preoperative vaccination of patients needing elective surgery globally could prevent additional COVID-19-related deaths in 1 year.
