Mechanisms in the pathogenesis of malignant tumours in neurofibromatosis type 1

Neurofibromatosis type 1(NF1) is a familial tumour syndrome. Malignant tumours can arise in the nervous and non-nervous system in either childhood or adulthood, with malignant peripheral nerve sheath tumours being most common. Rhabdomyosarcoma and neuroblastoma are paediatric neoplasms that are more common in children with NF1 than in those without the syndrome. Gastrointestinal stromal tumours, somatostatinomas, breast cancer, and phaeochromocytomas are seen in adults with NF1. Several pathways are thought to be involved in the development of tumours associated with NF1: rat sarcoma viral oncogene homologue (RAS)-mitogen activated protein kinase (MAPK), mammalian target of rapamycin (mTOR), and P21 protein (Cdc42/Rac)-activated kinase 1 (PAK1). New insights into the pathogenesis of these tumours will lead to a better understanding of tumour origin and development and will hopefully allow the discovery of new and specific treatments.

Summary: This study provides a comprehensive overview of the CNGA3 variant spectrum in 1060 genetically confirmed ACHM patients, with 385 of them carrying likely disease-causing variants in CNGA3. By combining their own genetic data with literature and public database records, the researchers extended the CNGA3 variant spectrum to a total of 316 variants, with 244 of them interpreted as likely disease-causing. They also reported 48 novel likely disease-causing variants, with 24 of them being missense substitutions. Extensive in silico analyses and functional data were provided to advance the pathogenicity assessment of the identified variants.

Case report: Coexistence of myotonia congenita and Brugada syndrome in one family

Summary: Myotonia congenita is a rare neuromuscular disorder caused by CLCN1 mutations. Although extramuscular manifestations were not previously believed to occur in chloride skeletal channelopathies, recent cases have described cardiac manifestations. This report discusses a family with autosomal dominant myotonia congenita and Brugada syndrome, and explores the possible involvement of CLCN1 gene mutations in primary cardiac arrhythmia.

Summary: This study evaluated changes in electrocardiographic parameters during ajmaline testing for the diagnosis of Brugada syndrome (BrS). The results showed that important heart rate variability changes were induced by ajmaline in BrS patients, and increased microvolt T wave alternans (mTWA) was observed only in BrS patients. Patients with BrS who experienced ventricular arrhythmias during follow-up showed worse changes during ajmaline testing, including lower low frequency power and higher maximum mTWA.

Summary: This study assessed underlying atrial cardiomyopathy in Brugada syndrome (BrS) and the effect of ajmaline test on the atrium of BrS patients using electrocardiogram imaging (ECGI). The study found that total atrial conduction time (TACT) and local atrial conduction time (LACT) were significantly prolonged in BrS patients, particularly after ajmaline administration and in patients with pathogenic SCN5A variants. TACT was identified as an independent predictor of atrial tachyarrhythmias (ATas) in BrS patients.
