Absolute risk and predictors of the growth of acute spontaneous intracerebral haemorrhage: a systematic review and meta-analysis of individual patient data

Background Intracerebral haemorrhage growth is associated with poor clinical outcome and is a therapeutic target for improving outcome. We aimed to determine the absolute risk and predictors of intracerebral haemorrhage growth, develop and validate prediction models, and evaluate the added value of CT angiography. Methods In a systematic review of OVID MEDLINE-with additional hand-searching of relevant studies' bibliographies-from Jan 1, 1970, to Dec 31, 2015, we identified observational cohorts and randomised trials with repeat scanning protocols that included at least ten patients with acute intracerebral haemorrhage. We sought individual patient-level data from corresponding authors for patients aged 18 years or older with data available from brain imaging initially done 0.5-24 h and repeated fewer than 6 days after symptom onset, who had baseline intracerebral haemorrhage volume of less than 150 mL, and did not undergo acute treatment that might reduce intracerebral haemorrhage volume. We estimated the absolute risk and predictors of the primary outcome of intracerebral haemorrhage growth (defined as >6 mL increase in intracerebral haemorrhage volume on repeat imaging) using multivariable logistic regression models in development and validation cohorts in four subgroups of patients, using a hierarchical approach: patients not taking anticoagulant therapy at intracerebral haemorrhage onset (who constituted the largest subgroup), patients taking anticoagulant therapy at intracerebral haemorrhage onset, patients from cohorts that included at least some patients taking anticoagulant therapy at intracerebral haemorrhage onset, and patients for whom both information about anticoagulant therapy at intracerebral haemorrhage onset and spot sign on acute CT angiography were known. Findings Of 4191 studies identified, 77 were eligible for inclusion. Overall, 36 (47%) cohorts provided data on 5435 eligible patients. 5076 of these patients were not taking anticoagulant therapy at symptom onset (median age 67 years, IQR 56-76), of whom 1009 (20%) had intracerebral haemorrhage growth. Multivariable models of patients with data on antiplatelet therapy use, data on anticoagulant therapy use, and assessment of CT angiography spot sign at symptom onset showed that time from symptom onset to baseline imaging (odds ratio 0.50, 95% CI 0.36-0.70; p<0.0001), intracerebral haemorrhage volume on baseline imaging (7.18, 4.46-11.60; p<0.0001), antiplatelet use (1.68, 1.06-2.66; p=0.026), and anticoagulant use (3.48, 1.96-6.16; p<0.0001) were independent predictors of intracerebral haemorrhage growth (C-index 0.78, 95% CI 0.75-0.82). Addition of CT angiography spot sign (odds ratio 4.46, 95% CI 2.95-6.75; p<0.0001) to the model increased the C-index by 0.05 (95% CI 0.03-0.07). Interpretation In this large patient-level meta-analysis, models using four or five predictors had acceptable to good discrimination. These models could inform the location and frequency of observations on patients in clinical practice, explain treatment effects in prior randomised trials, and guide the design of future trials. Sustainable ligand-modified based composite material for the selective and effective cadmium(II) capturing from wastewater

Summary: The chemical ligand BSBAE was embedded indirectly on mesoporous silica to fabricate optical composite materials (OCM) for detecting and removing toxic cadmium (Cd(II)) ions from wastewater solutions. The OCM demonstrated high sensitivity and selectivity towards Cd(II) ions in acidic pH conditions (pH 3.50) and showed distinguishable color changes upon the addition of trace levels of Cd(II) ions. Other metal ions did not significantly affect the performance of the OCM, which had a detection limit of 0.32 μg/L and a maximum adsorption capacity of 179.65 mg/g for Cd(II) ions. Cd(II) ions could be successfully desorbed from the saturated OCM using 0.25 M HCl, and the regenerated OCM maintained its selectivity and adsorption capacity. However, the adsorption efficiency slightly decreased after multiple cycles. Therefore, the OCM offers a cost-effective and viable alternative for detecting and removing toxic Cd(II) ions from wastewater.

A meta-analysis investigating the relationship between inflammation in autoimmune disease, elevated CRP, and the risk of dementia

Summary: Alzheimer's disease is a common type of dementia and poses a growing challenge for global health systems. Inflammation has emerged as a key focus in Alzheimer's disease research, and it has been found to drive the associated pathologies. This study investigates whether chronic inflammation caused by diseases like inflammatory bowel disease and rheumatoid arthritis could increase the risk of developing dementia. The results show a higher risk of dementia in patients with chronic inflammation and an independent association with high levels of CRP. This meta-analysis provides evidence that chronic elevation of CRP in autoimmune diseases is directly linked to an increased risk of Alzheimer's disease.

Summary: This study retrospectively analyzed the experience of using a mobile stroke unit (MSU) for direct to angiosuite (DTAS) transfer in patients with acute ischemic stroke (AIS). The results showed that the DTAS strategy can reduce the workflow time for endovascular thrombectomy (EVT).

Ayurvedic Treatment in the Rehabilitation of Ischemic Stroke Patients in India: A Randomized Controlled Trial (RESTORE) study protocol

Summary: This study aims to compare the effects of traditional Ayurvedic rehabilitative treatment and conventional physiotherapy on the recovery of sensory and motor impairments in patients with ischemic stroke. The study includes stable adult patients with their first acute ischemic stroke within 1 to 3 months, who will be randomly assigned to receive either Ayurvedic treatment or conventional physiotherapy for 1 month. The primary outcome measure is the Fugl Meyer Assessment-Upper extremity for physical performance at 90 days, and the secondary outcomes include the modified Rankin Scale, Barthel Index, Berg Balance, and SF-36 at 90 days. Safety outcomes include a composite of irreversible morbidity and mortality rates.

Clinical, Imaging Characteristics and Outcome of Intracerebral Hemorrhage Caused by Structural Vascular Lesions

Summary: This study investigated the clinical, imaging, and outcome characteristics of intracerebral hemorrhage (ICH) caused by structural vascular lesions. Compared to primary ICH, ICH caused by structural vascular lesions was found to occur in younger patients with a lower incidence of hypertension and diabetes. These patients predominantly had lobar hemorrhages and smaller baseline hematoma volumes. Additionally, they had lower mortality rates at 30 days and 3 months and better functional outcomes at 3 months.

Blood-Based Biomarkers in Intracerebral Hemorrhage

Summary: Intracerebral hemorrhage is a dangerous subtype of stroke with high rates of morbidity and mortality. Targeting prevention of hematoma growth and perihematomal edema expansion shows promise as a therapeutic approach. Blood-based biomarkers are being studied as potential diagnostic, predictive, and prognostic markers due to their accessibility and acceptance in clinical practice. Validation studies are needed to determine the usefulness of these biomarkers in the future.

The Treatment of Severe Periodontitis Using a Local Antiseptic Desiccant and Subgingival Mechanical Instrumentation: A Pilot Study

Summary: This clinical study compared the additional clinical benefits of combining a local desiccant antimicrobial agent (HY) with subgingival mechanical instrumentation (SRP) to SRP alone in treating severe periodontitis. Full-mouth periodontal examinations were performed at baseline and after a three-month follow-up for patients with stages III and IV periodontitis. The results showed no significant differences in the periodontal parameters between the test group (SRP plus HY) and the control group (SRP alone) after the treatment.

Summary: New technologies and disruptions caused by COVID-19 have led to the expansion of decentralized clinical trials, which hold promise for increasing efficiency and reducing barriers. The Trial Innovation Network has provided recommendations for decentralized approaches and has consulted on numerous research proposals. While some elements of decentralized trials have worked well, others require further refinement and assessment. Efforts are needed to build trust and ensure access to remote technology for inclusive participation in decentralized clinical trials.

Benefits in quality of life following an obstructive sleep apnea screening and treatment program in patients with acute ischemic stroke

Summary: This study aimed to determine the effect of obstructive sleep apnea (OSA) intervention on the quality of life and modified Rankin score of stroke patients. The results showed that a significant improvement in quality of life was observed in the intervention group, particularly among patients with good adherence to continuous positive airway pressure (CPAP) treatment. However, no significant improvement was observed in stroke patients. Therefore, OSA intervention has a significant impact on improving the quality of life in patients.

Summary: This study aimed to investigate the safety and efficacy of different doses of intra-arterial bone marrow mononuclear cells (BMMNCs) transplantation in patients with acute stroke. The results showed no significant improvement in the primary efficacy outcome at 180 days compared to the control group. Further clinical trials are needed to explore potential improvements at different timepoints.
