Summary: This study evaluated the knee loading and contact location of a control group, a pre-surgery group, and the same group 12 months post-surgery using a musculoskeletal modeling approach. The results showed that surgery changed the contact force in the medial compartment during gait, with a reduction in contact force post-surgery despite increased walking speed. This study provides insight into the effects of high tibial osteotomy on knee loading and supports the aim of surgery to unload the medial knee compartment.

Summary: Varus malalignment combined with an increased posterior tibial slope in the ACL deficient knee is a common pathology with challenging treatment. The presented biplanar osteotomy technique addresses both components of malalignment in a single step, with precise preoperative planning and intraoperative imaging verification. Resection of a bony wedge and bone allograft filling are performed according to the preoperative planning, and fixation is achieved with lag screws and plate fixation.

Summary: This study compared the outcomes of patients who underwent total knee arthroplasty (TKA) following unilateral proximal tibial osteotomy (PTO). The results showed that PTO did not significantly affect the subsequent TKA. Patients who had undergone PTO-TKA had comparable clinical function, subjective outcomes, and knee preference to those who had undergone TKA-only.

Summary: This randomized controlled trial assessed the effects of intra-articular injection of adipose-derived mesenchymal stem cells (ADMSCs) after medial open-wedge high tibial osteotomy (MOWHTO) on cartilage defect regeneration, functional improvement, and safety in osteoarthritic knee with varus malalignment. The results showed that the ADMSC group had significantly better cartilage regeneration, arthroscopic assessment, and functional improvement compared to the control group, without any treatment-related adverse events.

Summary: This study evaluated the influence of patellofemoral cartilage defects on the postoperative outcome of high tibial osteotomy (HTO) in patients with varus medial compartment osteoarthritis. The results showed that the long-term outcomes of HTO remained good to excellent even in the presence of patellofemoral defects. Overcorrection should be avoided, and distal biplanar HTO should be considered for patients with trochlear or kissing lesions of the patellofemoral joint.

The results showed a low rate of intraoperative complications and a relatively high rate of early postoperative complications. Surgeons should consider factors such as smoking, concomitant chondroplasty, and concomitant ligament reconstruction when counseling patients.
