The glomerular endothelium emerges as a key player in diabetic nephropathy

The effects of diabetes on glomerular structure and particularly that of the glomerular capillary wall have been extensively documented. By correlation with clinical measurements, Weil and colleagues provide important insights into the functional significance of glomerular structural changes in type 2 diabetes. Podocyte detachment correlates with albumin-to-creatinine ratio, but less strongly than does loss of endothelial fenestrations, which also correlates with reduced glomerular filtration rate. The role of the glomerular endothelium in diabetic nephropathy demands further scrutiny. Kidney International (2012) 82, 949-951. doi:10.1038/ki.2012.258

Summary: The study demonstrates that using human glomerular endothelial cells with all HLA antigens deleted as targets and recapitulating a large array of potential non-HLA antibodies in a single test can confirm the detrimental impact of non-HLA antibodies on microvascular inflammation and graft outcome after kidney transplantation, independent of HLA antibodies.

Summary: The impairment of the alternative complement pathway contributes to rare kidney diseases such as aHUS and C3G. Mutations in the classical complement pathway component C2, specifically the R249C and S250C variants, were found to stabilize classical C3 convertases and increase complement-dependent cytotoxicity. The study suggests CRP as a potential trigger for excessive complement activation in carriers of GoF mutations in complement C2.

GlomSpheres as a 3D co-culture spheroid model of the kidney glomerulus for rapid drug-screening

Summary: Tuffin et al. demonstrate a method for culturing conditionally immortalized human podocytes and GEnCs in 3D by forming spheroid cultures termed GlomSpheres, which are found to segregate in an organized manner. These are shown to establish a GBM-like extracellular matrix with increased cell differentiation and maturity when compared to 2D cultures and their use demonstrated in pharmaceutical screening using rapid imaging approaches.

Cerebral microvascular endothelial glycocalyx damage, its implications on the blood-brain barrier and a possible contributor to cognitive impairment

Summary: The socio-economic impact of diseases associated with cognitive impairment is increasing. Research into the cerebral endothelial glycocalyx (CeGC) could offer effective treatments. The CeGC plays a vital role in protecting parenchymal tissue and maintaining the blood-brain barrier (BBB), and its degradation is linked to several cognitive impairment conditions. Further research is needed to fully understand the diagnostic and therapeutic potential of CeGC.

Summary: Diabetic cardiomyopathy is a serious complication of diabetes that is characterized by diastolic dysfunction and can progress to heart failure. This study suggests that damage to the endothelial glycocalyx in the coronary microcirculation may contribute to increased microvascular permeability and impaired cardiac function in diabetes.

Summary: This study comprehensively characterized the fenestrations of glomerular endothelial cells (GEnC) and investigated their changes in diabetes and their impact on renal filtration function. The study identified EHD3 as a potential regulator of fenestration formation and suggested that its loss may contribute to decreased fenestration density and declining glomerular filtration function. Therefore, EHD3 may be a novel therapeutic target for restoring filtration function in disease.

Studying eGlx, especially its direct visualization in clinical samples, is technically challenging. Further research is needed to develop more clinically applicable techniques for assessing eGlx health.

Summary: This study investigated the damage of endothelial glycocalyx (eGlx) in dogs with myxomatous mitral valve disease (MMVD) and in dogs with a hypercoagulable state through measuring serum hyaluronan concentrations. The results showed that serum hyaluronan concentrations could serve as a clinically applicable marker of eGlx health and indicate the presence of eGlx damage in dogs with MMVD and dogs in a hypercoagulable state.

Summary: The fenestrations of glomerular endothelial cells (GEnCs) are crucial for the function of the glomerular filtration barrier. This review provides an update on the current understanding of the regulation and biogenesis of fenestrations and discusses the diseases in which loss of GEnC fenestrations may contribute to loss of glomerular filtration function. The literature reviewed includes studies on GEnCs as well as other cell types with similar fenestration structures, such as liver sinusoidal endothelial cells.
