Unilateral anomalies of kidney development: why is left not right?

Abnormal renal development results in congenital anomalies of the kidney and urinary tract. As many studies suggest that renal malformations are more often found on the left side, a meta-analysis was performed on the distribution of five different unilateral anomalies: multicystic dysplastic kidney, renal agenesis/ aplasia, renal ectopia, pelviureteral junction obstruction, and non-obstructive non-refluxing megaureter. Of these anomalies, the left side was affected in 53%, 57%, 56.9%, 63.2%, and 62.5% of patients, respectively, significantly different when compared with an anticipated 50% of left-sided anomalies. An exception to this left-side predominance was found in females with combined genital anomalies and unilateral renal agenesis that commonly present on the right side. The exact mechanisms leading to these lateralizations remain to be determined but may involve vascular development, differential gene expression, or susceptibility to environmental factors such as hypoxia. This remains largely speculative, however, illustrating our limited knowledge of embryogenesis in general and nephrogenesis in particular.

The podocyte as a direct target of glucocorticoids in nephrotic syndrome

Summary: Glucocorticoids have been found to directly affect podocytes, promoting their recovery and limiting inflammatory responses, potentially having a positive impact on the treatment of nephrotic syndrome. Current research is primarily conducted using animal and cell models, but there are still limitations in fully replicating the human condition. New and promising models such as glomerulus-on-a-chip and kidney organoids have the potential to be developed into functional models for nephrotic syndrome in the future.

Long-term stoma-related reinterventions after anterior resection for rectal cancer with or without anastomosis: population data from the Dutch snapshot study

Higher cefotaxime doses were found to be necessary for adequate exposure in most pathogens, with different dosing strategies recommended based on the pathogen's minimum inhibitory concentration (MIC). Increasing dose frequency or using continuous infusion may improve target attainment for intermediately susceptible pathogens.

Maturation of GFR in Term-Born Neonates: An Individual Participant Data Meta-Analysis

The results showed that AKI prevalence was higher and ARC prevalence was lower when using mGFR, and age- and sex-dependent equations had the highest accuracy for diagnosing AKI and ARC.

Summary: Scattered tubular cells (STCs) in the proximal tubule increase in number after acute kidney injury. STCs are preferentially located within the inner bends of the tubule and their number increases with age. STCs represent a transient state of dedifferentiated proximal tubule epithelial cells (PTECs) showing metabolic shift towards glycolysis, facilitating cellular survival after kidney injury.

Environmental and parental risk factors for congenital solitary functioning kidney - a case-control study

Summary: This case-control study identified maternal stress as a new risk factor for congenital solitary functioning kidney (CSFK), and confirmed associations with in vitro fertilization/intracytoplasmic sperm injection (IVF/ICSI), maternal infections, smoking during pregnancy, and parental CAKUT. However, previous associations with diabetes and obesity were not replicated. Folic acid supplement use and younger maternal age seemed to reduce the risk of CSFK.
