Pre-existing renal disease promotes sepsis-induced acute kidney injury and worsens outcome

While it is known that risk of death from sepsis is higher in patients with pre-existing chronic kidney disease its mechanism is unknown. To study this we established a two-stage mouse model where renal disease was first induced by folic acid injection followed by sub-lethal cecal ligation and puncture to induce sepsis. Septic mice with pre-existing renal disease had significantly higher mortality, serum creatinine, vascular permeability, plasma vascular endothelial growth factor (VEGF) levels, bacteremia, serum IL-10, splenocyte apoptosis and more severe septic shock when compared to septic mice without pre-existing disease. To evaluate the contribution of vascular and immunological dysfunction, we treated the folate-septic mice with soluble Flt-1 to bind VEGF and chloroquine to reduce splenocyte apoptosis. These treatments together resulted in a significant improvement in kidney injury, hemodynamics and survival. Our study shows that the sequential mouse model mimics human sepsis frequently complicated by pre-existing renal disease and might be useful in evaluating preventive and therapeutic strategies.

Experimental models of acute kidney injury for translational research

Summary: Models used in preclinical research often have limitations that result in results inconsistent with clinical practice. In the field of acute kidney injury (AKI), clinical trial failures are mainly attributed to inaccurate modeling in rodents. Currently, emerging research methods such as human kidney organoids, zebrafish, rodent, and large animal models are being applied in preclinical therapeutic research. However, the traditional preclinical research methods often overlook the limitations of model systems, leading to misleading results in the preclinical stage of treatment effectiveness. To address this problem, a more flexible and integrated approach is needed, with investigator teams using different model systems closely collaborating with clinical researchers to develop more targeted treatment methods. This review summarizes the current state of acute kidney injury model development and emphasizes the importance of adopting a multidisciplinary approach to optimize the development of treatment methods for acute kidney injury.

Plasma xanthine oxidoreductase is associated with carotid atherosclerosis in stable kidney transplant recipients

Summary: Plasma XOR activity is independently associated with atherosclerotic changes in the carotid artery of stable post-KT patients, with age and plasma XOR activity being independent predictors of CCA/ICA plaque or max-CIMT >0.9 mm. The cutoff value of plasma XOR activity for the diagnosis of CCA/ICA plaque or CCA-IMT > 0.9 mm was found to be 16.3 pmol/h/ml through receiver operator characteristic curve analyses.

Postoperative outcomes of cancer surgery in patients with and without kidney failure with dialysis therapy: a matched-pair cohort study

Summary: Patients with kidney failure and dialysis therapy (KFDT) had higher rates of postoperative complications and 30-day mortality in cancer surgery, but the risk differences varied among different types of cancer. The higher rates of postoperative complications in patients with KFDT were mainly attributable to cardiovascular complications.

The effect of continuous intravenous norepinephrine infusion on systemic hemodynamics in a telemetrically-monitored mouse model of sepsis

Summary: This study evaluated the effect of continuous intravenous norepinephrine infusion in a septic mouse model. The results showed that norepinephrine increased arterial blood pressure and heart rate, but did not improve survival rate. Moreover, heart rate may serve as an indicator for severity of sepsis.

Clinical frailty assessment might be associated with mortality in incident dialysis patients

Summary: Frailty as assessed by the clinical frailty scale (CFS) at dialysis initiation was found to be associated with lower survival rates and higher hospitalization rates within 2 years in maintenance dialysis patients. The CFS score was independently associated with the occurrence of adverse outcomes, regardless of age. This suggests that frailty assessment using CFS may predict outcomes in dialysis-initiated patients.

Summary: This study investigated the modulations of sphingolipids and glycerophospholipids in COVID-19 patients, revealing their dynamic changes depending on the time course and severity of the disease. Several species of phosphatidylcholine, phosphatidylethanolamine, and sphingomyelin may serve as better biomarkers for predicting severe COVID-19 during the early phase compared to traditional markers such as CRP and D-dimer.

Summary: This study investigated the role of interleukin-18 (IL-18) in the progression from acute kidney injury (AKI) to chronic kidney disease (CKD). The results showed that IL-18 deficiency can attenuate renal tubular damage, necroptosis, cell transdifferentiation, and fibrosis, and this renoprotective effect may be mediated by reducing the switch from M1 to profibrotic M2 macrophages.
