Summary: This report presents a case of a 14-year-old patient with Marfan syndrome who underwent an emergency Bentall procedure for acute type A aortic dissection. The patient required postoperative extracorporeal membrane oxygenation and was listed for heart transplantation due to persistent left ventricular failure caused by an intimal tear and thrombosis of the left main coronary artery. The heart transplantation was performed 5 days after the initial procedure, and the patient was discharged 60 days after admission.

Summary: This study aimed to evaluate the mid-term outcomes of thoracic endovascular aorta repair (TEVAR) in patients with Marfan syndrome (MFS) and type B aortic dissection (TBAD). A total of 26 MFS patients underwent TEVAR and had a perioperative mortality rate of 0%. The cumulative survival rate at 5 and 10 years was 88.1% and 82.9% respectively. Patients with thrombosed false lumen (FL) had significantly higher survival and freedom from reintervention rates compared to those with patent FL. However, patients with patent FL were at high risk of late death.

Summary: This study investigates the aortic-related risks associated with pregnancy in women with bicuspid aortic valve (BAV) and evaluates changes in aortic diameter during pregnancy. The results show that pregnant women with BAV have a low rate of cardiac complications and aortic complications are rare. However, there is a small but significant increase in aortic diameter during pregnancy.

Summary: The novel technique presented in this study successfully crossed a mechanical aortic valve prosthesis in a patient with Marfan syndrome. The 3-stage hybrid repair allowed for the implantation of a custom-made endovascular stent-graft directly below the coronary arteries without causing significant damage to the valve. Caution should still be exercised during the positioning of the endovascular graft to avoid potential damage to the valve prosthesis.

LDEMTX reduced inflammation and fibrosis, and lowered the expression of inflammatory factors. These findings suggest that LDEMTX may be a potential candidate for clinical studies.

Summary: This study found sex differences in aortic events among Marfan syndrome patients. Male patients had a significantly higher probability of experiencing an aortic event compared to female patients, but there was no increased probability for recurrent events in male patients. Follow-up revealed no sex differences in the occurrence of Stanford type A or type B aortic dissection between male and female patients. Most women were not aware of their Marfan syndrome diagnosis before conceiving. 10% of women with children suffered from peripartum aortic events, while twice as many female patients with children suffered from aortic dissection unrelated to childbirth. There were no sex differences affecting mortality in Marfan patients.
