Predictors of Sperm Recovery and Azoospermia Relapse in Men With Nonobstructive Azoospermia After Varicocele Repair

Purpose: In this study we determined the recoverability and sustainability of motile sperm in semen of men with nonobstructive azoospermia after varicocelectomy as related to different variables. Materials and Methods: Men with documented infertility for more than 1 year, with nonobstructive azoospermia and clinically palpable varicoceles were included in this prospective noncontrolled study. Participants underwent simultaneous subinguinal microsurgical varicocelectomy and testicular biopsies. Preoperative as well as initial and late followup semen analyses were performed. Outcomes of sperm recovery and relapse of azoospermia were correlated with the variables of patient age, infertility duration, varicocele grade, laterality, follicle-stimulating hormone, testicular volume and testicular histology. Results: The study included 31 men with a mean +/- SD age of 34.9 +/- 8.7 years and mean followup of 19.3 +/- 3.3 months. Hypospermatogenesis, late maturation arrest, early maturation arrest and Sertoli-cell-only were observed in 13, 6, 2 and 10 patients, respectively. Overall, sperm recovery was evident in 10 of 31 (32.3%) patients (persistent recovery 19.4%, intermittent recovery 6.5%, relapse 6.5%). Sperm were recovered in patients with hypospermatogenesis (7 of 13, 53.8%) and late maturation arrest (3 of 6, 50%). No sperm were recovered in those with early maturation arrest or Sertoli-cell-only. Among the variables only histological patterns demonstrated a significant correlation with recovery (rho = 0.504, p = 0.004). None of variables was significantly correlated with relapse. Bilateral varicocele repair demonstrated a strong yet nonsignificant negative correlation with relapse (rho = -0.612, p = 0.06). Conclusions: Varicocelectomy could recover motile sperm in men with nonobstructive azoospermia, palpable varicoceles and hypospermatogenesis or late maturation arrest. No sperm was recovered with early maturation arrest or Sertoli-cell-only. Recovery might be persistent or intermittent, or involve relapse of azoospermia. Testicular histology was the sole parameter significantly correlated with recovery and no predictors of relapse could be identified. This prognostic role of testicular biopsy is imperative in couple counseling.

Summary: This study aimed to evaluate the impact of X chromosome overdosage in 47,XXY males iPSCs obtained from patients with different genomic backgrounds and ethnicities. The results indicate that a transcriptomic signature of X chromosome overdosage in KS is potentially attributable to a subset of X-linked genes sensitive to sex chromosome dosage and escaping X inactivation, regardless of the geographical area of origin, ethnicity, and genetic makeup.

Summary: Infertility is a global health concern with a significant burden on the global economy and socio-psychological well-being. The role of male infertility is often overlooked, despite approximately 15% of couples worldwide experiencing infertility, with a male factor contribution of around 50%. This study investigates the potential toxic effects of two common pyrethroids, cypermethrin and deltamethrin, on androgen receptor (AR) signaling. The results suggest that cypermethrin and deltamethrin may disrupt AR signaling, leading to androgen dysfunction and subsequent male infertility.

Summary: Brucellosis is a potentially life-threatening zoonotic disease caused by Brucella bacteria, which can infect various farm animals and humans. Genitourinary brucellosis, including testicular abscesses, is common in endemic areas. Treatment usually involves long-term antibiotic therapy, with surgical intervention required for larger abscesses. This case report presents a rare occurrence of a huge multilocular brucellar testicular abscess, successfully treated with organ-sparing management.

Summary: The purpose of this study was to investigate global practices related to the management of elevated SDF in infertile men, summarize the relevant professional society recommendations, and provide expert recommendations for managing this condition. The results of a large global survey on the management of infertile men with elevated SDF revealed a lack of consensus among clinicians and highlighted the scarcity of professional society guidelines in this regard.

An Analysis of the Structural Relationship between Thyroid Hormone-Signaling Disruption and Polybrominated Diphenyl Ethers: Potential Implications for Male Infertility

Summary: Polybrominated diphenyl ethers (PBDEs) are widely used fire-retardant chemicals in consumer products and have the potential to bioaccumulate in the environment, including wildlife and humans, leading to adverse health effects such as neurodevelopmental deficits and reproductive dysfunction. In this study, the structural interactions of four PBDEs with the thyroid hormone receptor (TR alpha) were investigated. The results showed that all four PBDEs bound tightly to TR alpha and disrupted the thyroid signaling, which could have implications in reproductive function and infertility.

Influence of BMI on the Recurrence Rate of Nephrolithiasis in the Adult Population of Saudi Arabia: A Retrospective Study

Summary: This study retrospectively analyzed the data of 248 patients and found no significant association between obesity and the recurrence rate of renal stones. However, Saudi nationality, chronic diseases, unilateral stones, and a stone diameter of 10-19 mm were significantly associated with the recurrence rate. Larger studies are needed to confirm these findings.

Summary: This study provides evidence for the positive effect of varicocele repair on conventional semen parameters in infertile men with clinical varicocele. There is significant improvement in sperm concentration, total motility, progressive motility, total sperm motility, and normal sperm morphology after varicocele repair. However, there is high inter-study heterogeneity.
