Vitamin D Insufficiency among Children and Adolescents Living in Tehran, Iran

Vitamin D is important for calcium absorption and skeletal growth. Vitamin D insufficiency (VDI) is a prevalent health problem in children. A study was performed to determine the prevalence of VDI in healthy children living in Tehran, Iran. In a cross-sectional study, 963 students (424 boys and 539 girls) aged 7-18 years were selected by random sampling. Serum 25-hydroxyvitamin D (25-OHD), calcium, alkaline phosphatase and phosphorus were measured. VDI was defined as serum 25-OHD <20 ng/ml. Prevalence of VDI was 53.6% in girls and 11.3% in boys. VDI in female students was about five times more common than males (p < 0.000001). VDI in children and adolescent girls is a health problem not only for these age-groups but also for the next generation to come. Encouraging girls to have more sun exposure, fortification of foods and prescription of supplemental vitamin D are recommended.

Novel disease-causing variants in a cohort of Iranian patients with metachromatic leukodystrophy and in silico analysis of their pathogenicity

Summary: The study investigated the genotype-phenotype spectrum of MLD patients in Iran, identifying several novel disease-causing variants and highlighting the presence of alternative causes for leukodystrophy in some individuals. The most frequent alleles among Iranian MLD patients were c.931 G > A (p.Gly311Ser) and c.465 + 1 G > A variants, with five mutations appearing to be specific to this population. Population screening for these variants could help reduce the disease burden in this region.

Invasive Rasamsonia argillacea infection in chronic granulomatous disease: Report of a new case and literature review

Summary: Invasive Rasamsonia spp. infections are rare and often associated with chronic granulomatous disease (CGD). We reported a case of pulmonary and possible cerebral infection caused by Rasamsonia argillacea in a CGD girl who did not receive primary antifungal prophylaxis, resulting in a fatal outcome despite antifungal therapy and surgical interventions. A literature review on reported invasive Rasamsonia spp. infections in the context of CGD was also conducted.

Effects of Coronavirus Disease 2019 (COVID-19) on Peripheral Blood Lymphocytes and Their Subsets in Children: Imbalanced CD4+/CD8+ T Cell Ratio and Disease Severity

Summary: This study described lymphocyte subsets in pediatric patients with mild/moderate or severe COVID-19. Results showed that patients with severe disease had lower lymphocyte counts, higher CD8(+) T cell count, lower CD4(+) T cell percentage, and a lower CD4(+)/CD8(+) T cell ratio compared to those with mild/moderate disease. The study suggested that a decrease in CD4(+)/CD8(+) ratio may serve as a useful metric in analyzing immune responses in severe COVID-19 patients, but further research with larger sample sizes is recommended.

Two cases of sudden cardiac death due to TRDN mutations provide further insight into this rare inherited arrhythmia syndrome. The average age of onset for patients is 30 months, with adrenergic-mediated cardiac events being the main manifestation.
