Summary: In two intensive care unit settings in China, the implementation of a comprehensive unit-based safety program (CUSP) effectively reduced the incidence of ventilator-associated pneumonia (VAP) and other infections in patients receiving mechanical ventilation. It also prolonged the time until VAP occurrence, reduced the days of antibiotic use for VAP, shortened the length of stay in the ICU and hospital, and enhanced safety culture awareness.

The results showed that tracheotomy, multidrug-resistant bacteria, and ICU length of stay >5 days were independent risk factors of VAP, while continuous control of cuff pressure and subglottic secretion suction used together were protective factors against VAP.

Summary: The use of MEDI3902 antibody for prophylactic treatment has been shown to protect rabbits from Pseudomonas aeruginosa infection and reduce lung lesions and bacteremia. Compared to MEDI3902 treatment, rabbits treated with irrelevant control IgG exhibited severe pneumonia and systemic pathophysiological changes. These results suggest that MEDI3902 prophylaxis can decrease the severity of P. aeruginosa ventilator-associated pneumonia.

Summary: This study aimed to describe the incidence and risk factors of methicillin sensitive Staphylococcus aureus ventilator associated pneumonia (MSSA-VAP) relapse in trauma and non-traumatic brain injury patients. The presence of Streptococcus species and oropharyngeal flora as initial MSSA co-pathogen, suggested aspiration at the time of admission, and initial Glasgow coma scale were identified as factors predicting MSSA-VAP treatment failure. On the other hand, the need for emergent surgery and the presence of acute respiratory distress syndrome at the time of the first MSSA-VAP were factors independently predicting the onset of other pathogen-VAP.

Summary: This study examined the knowledge, practices, compliance, and barriers related to ventilator-associated pneumonia among critical care nurses in the eastern Mediterranean region. The results showed that most nurses had insufficient levels of knowledge, compliance, and practices related to ventilator-associated pneumonia. The main barriers reported were a lack of education, shortage of nursing staff, lack of policies and protocols, and lack of time. Comprehensive interventions are needed to improve nurses' knowledge, compliance, and practice in this area.

Summary: This study aimed to identify risk factors associated with 30-day unplanned readmissions after emergency general surgery (EGS). Using a retrospective cohort study, the analysis revealed associations between patient characteristics such as age, anesthesia score, sepsis, body mass index, procedure risk, length of stay, and discharge disposition. Thirty-day readmissions were found to have higher incidence of surgical site infections, wound disruption, postoperative sepsis, and thromboembolic events.

Summary: This study aimed to describe the characteristics and outcomes of pregnant patients who experienced intimate partner violence (IPV) and compare them with non-pregnant IPV patients. The results showed that pregnant IPV patients had higher admission rates and mortality compared to non-pregnant IPV patients. Differences in injury patterns were also observed, providing opportunities for targeted IPV screening.

Summary: Patients with severe traumatic brain injuries should be primarily admitted to a hub trauma center with neurosurgical capabilities for immediate and specialized care. However, in cases where patients are admitted to a spoke hospital without neurosurgical capabilities, there is limited data on optimal management. This consensus provides practical recommendations for the management of severe isolated TBI patients in centers without neurosurgical capabilities and during transfer to a hub center.

Summary: California issued stay-at-home orders to mitigate COVID-19 spread. This study aimed to evaluate the effects of these orders on geriatric trauma patients. The results showed a decreased rate of motor vehicle collisions and pedestrian struck for these patients, but no differences in other mechanisms of injury and length of stay were observed.

Summary: This study aimed to investigate the outcomes associated with the use of tranexamic acid (TXA) in high- and low-profile introducer sheaths for resuscitative endovascular balloon occlusion of the aorta (REBOA). The results showed that patients who received TXA had a higher risk of distal embolism. Therefore, strict protocols for immediate diagnosis and treatment of thrombotic complications should be followed when performing REBOA in patients receiving TXA.
