National Trauma Institute prospective evaluation of the ventilator bundle in trauma patients: Does it really work?

BACKGROUND: Since its introduction by the Institute for Healthcare Improvement, the ventilator bundle (VB) has been credited with a reduction in ventilator-associated pneumonia (VAP). The VB consists of stress ulcer prophylaxis, deep venous thrombosis prophylaxis, head-of-bed elevation, and daily sedation vacation with weaning assessment. While there is little compelling evidence that the VB is effective, it has been widely accepted. The Centers for Medical and Medicaid Services has suggested that VAP should be a never event'' and may reduce payment to providers. To provide evidence of its efficacy, the National Trauma Institute organized a prospective multi-institutional trial to evaluate the utility of the VB. METHODS: This prospective observational multi-institutional study included six Level I trauma centers. Entry criteria required at least 2 days of mechanical ventilation of trauma patients in an intensive care unit (ICU). Patients were followed up daily in the ICU until the development of VAP, ICU discharge, or death. Compliance for each VB component was recorded daily, along with patient risk factors and injury specifics. Primary outcomes were VAP and death. VB compliance was analyzed as a time-dependent covariate using Cox regression as it relates to outcomes. RESULTS: A total 630 patients were enrolled; 72% were male, predominately with blunt injury; and mean age, Injury Severity Score (ISS), and 24-hour Glasgow Coma Scale (GCS) score were 47, 24, and 8.7, respectively. VAP occurred in 36%; mortality was 15%. Logistic regression identified male sex and pulmonary contusion as independent predictors of VAP and age, ISS, and 24-hour Acute Physiology and Chronic Health Evaluation as independent predictors of death. Cox regression analysis demonstrated that the VB, as a time-dependent covariate, was not associated with VAP prevention. CONCLUSION: In trauma patients, VAP is independently associated with male sex and chest injury severity and not the VB. While quality improvement activities should continue efforts toward VAP prevention, the Institute for Healthcare Improvement VB is not the answer. A comprehensive analysis of 30-day readmissions after emergency general surgery procedures: Are risk factors modifiable?

Summary: This study aimed to identify risk factors associated with 30-day unplanned readmissions after emergency general surgery (EGS). Using a retrospective cohort study, the analysis revealed associations between patient characteristics such as age, anesthesia score, sepsis, body mass index, procedure risk, length of stay, and discharge disposition. Thirty-day readmissions were found to have higher incidence of surgical site infections, wound disruption, postoperative sepsis, and thromboembolic events.

Intimate Partner Violence and Pregnancy: Nationwide Analysis of Injury Patterns and Risk Factors

Summary: This study aimed to describe the characteristics and outcomes of pregnant patients who experienced intimate partner violence (IPV) and compare them with non-pregnant IPV patients. The results showed that pregnant IPV patients had higher admission rates and mortality compared to non-pregnant IPV patients. Differences in injury patterns were also observed, providing opportunities for targeted IPV screening.

Summary: Patients with severe traumatic brain injuries should be primarily admitted to a hub trauma center with neurosurgical capabilities for immediate and specialized care. However, in cases where patients are admitted to a spoke hospital without neurosurgical capabilities, there is limited data on optimal management. This consensus provides practical recommendations for the management of severe isolated TBI patients in centers without neurosurgical capabilities and during transfer to a hub center.

Summary: California issued stay-at-home orders to mitigate COVID-19 spread. This study aimed to evaluate the effects of these orders on geriatric trauma patients. The results showed a decreased rate of motor vehicle collisions and pedestrian struck for these patients, but no differences in other mechanisms of injury and length of stay were observed.

Summary: This study aimed to improve the accuracy and effectiveness of the TACS classification for emergency general surgery (EGS) patients. By involving international experts through a standardized Delphi approach, the new TACS classification provides a comprehensive and reproducible triage system which can reduce waiting time and improve communication among medical professionals.

Use of Tranexamic Acid With Resuscitative Endovascular Balloon Occlusion of the Aorta is Associated With Higher Distal Embolism Rates: Results From the American Association of Surgery for Trauma Aortic Occlusion and Resuscitation for Trauma and Acute Care Surgery Trial

Summary: This study aimed to investigate the outcomes associated with the use of tranexamic acid (TXA) in high- and low-profile introducer sheaths for resuscitative endovascular balloon occlusion of the aorta (REBOA). The results showed that patients who received TXA had a higher risk of distal embolism. Therefore, strict protocols for immediate diagnosis and treatment of thrombotic complications should be followed when performing REBOA in patients receiving TXA.
