Summary: This narrative review examined the literature related to the risk of deep vein thrombosis (DVT) in people with hemophilia (PWH) after orthopedic surgery. It concluded that pharmacological thromboprophylaxis is generally not necessary for PWH, except in cases of major orthopedic surgery with additional VTE risk factors. Limited evidence suggests the use of low molecular weight heparins or direct oral anticoagulants in some PWH with risk factors for VTE.

Summary: The risk of venous thromboembolism is significantly higher in obese patients undergoing total joint arthroplasty, and the prevalence of obesity in the aging population further increases the postoperative thrombosis risk. There is a lack of clear evidence for dosing recommendations of thromboprophylaxis medications in obese patients, leading to suboptimal dosing regimens and potential bleeding or clotting complications. This review discusses the increased thrombotic risk in obesity, the evidence supporting dosage recommendations, and the implications of current guidelines for obese patients undergoing lower limb arthroplasty.

Summary: This study aimed to determine the effect of early versus postoperative chemoprophylaxis on the incidence of VTE and bleeding in high-risk patients undergoing major abdominal surgery. The results showed that starting chemoprophylaxis postoperatively reduced bleeding risk without affecting the occurrence of VTE.

Summary: VTE is a critical complication in orthopedic surgery, often leading to pulmonary thromboembolism with high mortality and increased medical costs. Prevention methods, including physical prophylaxis, drug prophylaxis, or both, should be selected based on the balance between VTE and bleeding risks. Patients should be informed about the risks and benefits of prophylaxis.

Summary: The risk of venous thromboembolism is low to moderate in nonmajor orthopedic surgery, with inconclusive literature and new emerging data available. Global patient risk must be considered to determine the need for prophylaxis.

Summary: Background: Pancreatic cancer (PC) has a high risk of venous thromboembolism (VTE), but there is a lack of verified risk assessment models (RAMs) for metastatic pancreatic cancer (mPC). Methods: This retrospective study investigated a cohort of mPC patients treated at an academic cancer center and found a VTE incidence of 17.5%. Survival analysis showed that VTE predicted poorer outcomes, with a median OS of 10.5 months in the VTEmets group compared to 13.4 months in the non-VTE group. Advanced stage disease was identified as the strongest risk factor. Conclusions: These results highlight the significant burden of VTE in mPC and the importance of risk stratification and thromboprophylaxis in this population.

Summary: This systematic review examines the profile of rivaroxaban for VTE prophylaxis after orthopedic surgery. The results indicate that rivaroxaban has better efficacy in reducing the risk of VTE and all-cause mortality compared to enoxaparin. However, there is no significant difference in safety between rivaroxaban and enoxaparin.

Summary: Hospital-associated venous thromboembolism is a significant cause of morbidity and mortality in patients with cirrhosis. The use of thromboprophylaxis in these patients is challenging due to the uncertain balance between thrombosis and bleeding risk. This guidance document provides pragmatic recommendations on VTE prevention for hospitalized patients with cirrhosis, including drug selection and timing.

Summary: VTE is a common and preventable harm in noncritically ill hospitalized children, with risk factors including central venous catheters, systemic inflammation, and prolonged immobilization. Standardized institutional practices and multidisciplinary collaboration are necessary for the diagnosis, treatment, and prevention of VTE in this patient population.

Summary: Venous thromboembolism (VTE) is a major concern during pregnancy and its prevention with medical thromboprophylaxis can cause bleeding complications. This study found that low-dose heparin is effective in preventing VTE recurrence in pregnant women with a history of VTE, with no significant difference in bleeding risk compared to intermediate-dose heparin.

Summary: Cancer-associated thrombosis, a condition with increasing incidence, has significant morbidity and mortality in cancer patients. Recent advances in its treatment include the use of direct oral anticoagulants (DOACs) which are more convenient and effective than low-molecular-weight heparin (LMWH). However, there are still unmet needs and concerns about bleeding risk, drug-drug interactions, and management of patients with severe renal impairment.

Summary: Staphylococcus aureus infection is associated with increased levels of neutrophil extracellular traps (NETs) and von Willebrand factor (VWF), and with reduced activity of ADAMTS13. Peptidylarginine deiminase 4 (PAD4) contributes to NET formation and inactivates ADAMTS13 in vitro. The levels of PADs correlate with ADAMTS13 activity, with neutrophils as the likely source of PAD activity in S. aureus infection. NETs are positively correlated with PADs and negatively correlated with ADAMTS13 activity.

Summary: This study compared the safety and effectiveness of percutaneous direct puncture approach and contralateral femoral native vessel approach for catheter-directed thrombolysis of occluded infra-inguinal bypass grafts. The results showed that the direct puncture approach is a safe and efficient method with comparable clinical outcomes to the native vessel approach. The rate of adverse events was slightly lower in the direct approach group, but without significance.
