Summary: Valve-sparing aortic root replacement is a feasible and safe surgical option with good long-term outcomes and low rates of late aortic valve replacement. Preoperative annular dilation is associated with early repair failure.

Summary: During the late TAVR era (2015-2017), the utilization of SAVR decreased while TAVR utilization increased steadily from 2011 to 2017. Patients undergoing SAVR had higher in-hospital mortality rates compared to TAVR patients.

Summary: Recent studies have shown that transcatheter aortic valve replacement (TAVI) provides greater benefit than surgical aortic valve replacement (SAVR) for female patients. This study assessed the early outcomes of aortic valve interventions in female and male patients and found no difference in mortality or major postoperative complications between the two groups.

Summary: This study developed a comprehensive scoring system (TAVI-CT score) to summarize computed tomography (CT) examinations for transcatheter aortic valve implantation (TAVI) planning. The TAVI-CT score was found to be significantly associated with male gender and showed differences in access and prosthesis choice based on the score. Vascular complications were more common in the group with the highest TAVI-CT score.

Summary: Transcatheter aortic valve replacement (TAVR) has revolutionized the treatment of severe aortic stenosis, but the occurrence of conduction disturbances and new-onset atrial fibrillation post-procedure remains a challenge. Ambulatory electrocardiogram (AECG) monitoring has emerged as a valuable tool for diagnosing arrhythmic events before and after TAVR, showing promising safety and clinical implications.

Summary: This study compared the 5-year risk-adjusted outcomes of S3 TAVR and SAVR in intermediate-risk patients and found similar rates of death and disabling stroke between the two procedures. However, the incidence of paravalvular regurgitation was higher after S3 TAVR.

Summary: The study shows that concomitant replacement of the ascending aorta prolongs operative times but does not increase morbidity and mortality. In patients undergoing aortic valve replacement, replacing a dilated ascending aorta appears to be the most durable and safest treatment option.

Summary: The study found that a low GNRI score was associated with an increased risk of all-cause mortality in patients undergoing TAVR, suggesting that this vulnerable group may benefit from improved preventive measures.

Summary: This study compared the outcomes of SAVR in low-risk patients with expected results from STS models and evaluated their intermediate-term survival. The observed mortality, stroke, and reoperation rates were lower than expected, indicating that STS models overestimate the risk of SAVR in low-risk patients. Additionally, the survival rate of low-risk patients was superior to the predicted value for the US population.

Summary: This study compared the prevalence and outcomes of unicuspid aortic valve (UAV) with bicuspid (BAV) and tricuspid valve (TAV) in patients undergoing the Ross procedure. The results showed that the risks of midterm autograft reoperation and the onset of aortic regurgitation were similar among the groups, indicating that UAV patients can still be considered for the Ross procedure.

Summary: TAVR is rapidly expanding its indications, but faces technical challenges and knowledge gaps when dealing with patients with bicuspid aortic valve (BAV). Physicians need to understand the BAV anatomy and TAVR procedure planning comprehensively to guide treatment decisions.

Summary: This study investigated the levels of double-stranded DNA and deoxyribonuclease activity in plasma after TAVR, finding that baseline double-stranded DNA was significantly higher compared to healthy controls, peaked at 3 and 7 days post-TAVR, and then declined. Additionally, decreased deoxyribonuclease activity at baseline was associated with higher C-reactive protein levels and mean transaortic gradient after 12 months, serving as a strong independent predictor of outcome post-TAVR.

Summary: This review compares the stroke and mortality rates of valve-in-valve TAVR procedures with native TAVR and provides substantial data for clinical decision-making. The analysis shows no significant differences in short-term and mid-term outcomes between valve-in-valve TAVR and the comparator treatments.

Summary: AF after AVR is associated with worse outcomes, especially in the SAVR group. TAVR may be a favorable treatment option for high-risk patients with AF.
