Three new case reports of Arteriovenous malformation-related Amyotrophic Lateral Sclerosis

Despite recent advances in genetics, in most cases of Amyotrophic Lateral Sclerosis (ALS) no etiological factor can be identified. Cerebral Arteriovenous Malformations (AVMs) have been associated with ALS development in a few studies, but the nature of this connection is unclear. We report here 3 additional cases of young adults, who had undergone repeated embolizations for complex AVMs, and who then developed, after many years, ALS symptoms and signs. In two of these cases Vascular Endothelial Growth Factor (VEGF) levels were found to be extremely high, in contrast to previous reports. Our 3 cases, together with the previously reported ones, suggest that a subgroup of patients with AVMs, with a particular profile of a complex nidus with repeated embolization procedures, are at increased risk of developing ALS. The reason for this association is unclear, but may relate to dysregulation of secreted vascular factors, as suggested by our VEGF results, or more broadly to the neurovascular hypothesis of ALS. Alternatively, a transneuronal type of neurodegeneration may be involved.

Treatment of carotid cavernous sinus fistulas with flow diverters. A case report and systematic review

Summary: Carotid cavernous fistulas (CCFs) are rare vascular abnormalities that are often caused by head trauma or aneurysmal rupture. This study reviewed 38 cases of CCF treated with flow diversion devices (FDDs), showing a high success rate of clinical improvement and long-term angiographic occlusion. However, further research with multi-center prospective trials is needed to confirm these findings.

Vestibular paroxysmia: Clinical features and imaging findings; a literature review

Summary: Neurovascular compression syndromes (NVCS) occur when a cranial nerve is mechanically irritated by a vascular structure in direct contact with it, resulting in corresponding symptoms. Vestibular paroxysmia is an example of NVCS caused by neurovascular contact between the eighth cranial nerve and a blood vessel. Understanding the unique anatomy of the vestibulocochlear nerve is crucial for studying this syndrome, and imaging techniques play a significant role in demonstrating clinically relevant cases. Based on the reviewed literature, the 3D CISS MRI sequence appears to be the most effective imaging technique for visualizing neurovascular compression of the eighth cranial nerve.

Cortical involvement and leptomeningeal inflammation in myelin oligodendrocyte glycoprotein antibody disease: A three-dimensional fluid-attenuated inversion recovery MRI study

Summary: In this study, we used MRI to evaluate cortical and leptomeningeal involvement in MOGAD and RRMS patients. We found that MOGAD patients had significantly fewer cortical lesions compared to RRMS patients, with a slightly higher prevalence of intracortical-subpial lesions in MOGAD. Additionally, leptomeningeal contrast enhancement was observed in some MOGAD patients, which was associated with a higher number of cortical lesions.

The Heimann-Bielschowsky phenomenon after optic neuritis

Summary: This article reports a 32-year-old woman with relapsing-remitting multiple sclerosis and incomplete remission of right optic neuritis who presented with a unique neuro-ophthalmologic abnormality characterized by spontaneous, pendular, vertical movement of the right eye consistent with the Heimann-Bielschowsky phenomenon. This rare form of dissociated nystagmus is likely to reflect the coexistence of an asymmetric conduction delay in the optic nerve and a strategic network disruption in brainstem gaze holding centres.

Cholesterol levels in plasma and cerebrospinal fluid in patients with clinically isolated syndrome and relapsing-remitting multiple sclerosis

Summary: Cholesterol levels may be associated with disease characteristics in multiple sclerosis, particularly with demyelination and immune response. This study found lower levels of plasma and cerebrospinal fluid cholesterol in patients with multiple sclerosis. Patients with positive oligoclonal bands had even lower cerebrospinal fluid cholesterol levels, which may be related to disease progression and enhanced humoral immune response.

A Greek National Cross-Sectional Study on Myotonic Dystrophies

Summary: This study retrospectively analyzed the medical records of 561 Greek patients with myotonic dystrophies. The findings show similarities in clinical and genetic characteristics between these patients and those from other regions. Muscle weakness is the first symptom in both types of myotonic dystrophies, while multisystemic involvement and cataracts are common manifestations. The study highlights the importance of specialized neuromuscular centers that can provide genetic counseling and a multidisciplinary approach.

Screening for the FMR1 premutation in Greek patients with late-onset movement disorders

Summary: This study investigates the frequency, genotypic and phenotypic profile of FXTAS in two cohorts of Greek patients with late-onset movement disorders and suggests that FXTAS is an important, albeit rare, cause of late-onset movement disorders. It highlights the importance of FMR1 genetic testing in the diagnosis of late-onset movement disorders.

Summary: By screening 60 patients with CMT2, dHMN, or HSN in the Greek population, 20 pathogenic or likely pathogenic variants were identified, highlighting the genetic and phenotypic variability of axonal CMT. The diagnostic yield of the customized gene panel in this study compares favorably with other European populations, contributing to the characterization of pathogenic variants related to axonal neuropathies.

Summary: Bi-allelic variants in PRDX3 are associated with early onset slowly progressive cerebellar ataxia, with hyperkinetic and hypokinetic features, cerebellar atrophy, and occasional involvement of olivary and brainstem. We describe a simplex case with a reported variant and two additional cases with novel variants, expanding the phenotypic spectrum of the disease. SCAR32 should be considered in the differential diagnosis of sporadic or recessive childhood and adolescent-onset pure and complex cerebellar ataxia.
