Non-obese NAFLD had no better cardio-metabolic risk profile than obese NAFLD in type 2 diabetic patients

Summary: In patients with type 2 diabetes mellitus (T2DM), non-obese non-alcoholic fatty liver disease (NAFLD) does not have a better cardio-metabolic risk profile compared to obese NAFLD. Non-obese NAFLD patients have more and worse metabolic disorders than obese patients without NAFLD, and the associations of metabolic disorders with NAFLD are stronger in non-obese subjects among women with T2DM and in obese subjects among men with T2DM.

Evaluating pharmacokinetic drug-drug interactions of direct oral anticoagulants in patients with renal dysfunction

Summary: Drug transporters, metabolic enzymes, and renal clearance play significant roles in the pharmacokinetics of DOACs. Current recommendations for the management of pharmacokinetic DOAC DDIs lack considerations for concomitant renal dysfunction. Empiric dose adjustments based on pharmacokinetic data alone should be avoided, and future research on identification of DOAC therapeutic ranges and target patient populations is needed.

Physiologically-based pharmacokinetic modeling to predict drug-drug interactions of dabigatran etexilate and rivaroxaban in the Chinese older adults

Summary: The study found that drug-drug interaction (DDI) has a significant impact on the clinical use of NOACs in older adults. The guidelines on DDI management based on studies conducted in healthy adults may not be applicable to the Chinese older adults. Aging and DDI both increase the exposure of NOACs in Chinese older adults, but aging does not have a drastic impact on the extent of DDIs. The recommended dosing regimen for DDI management in Chinese older adults can be similar to the EHRA guideline, but routine monitoring of bleeding risk is necessary when using dabigatran etexilate with verapamil and clarithromycin.

Identifying and managing potential PK drug interactions with NOACs remains a priority to optimize the clinical benefit of these anticoagulants.

Summary: This study aimed to evaluate the effects of adherence to oral anticoagulants in patients with atrial fibrillation, specifically warfarin and DOACs. Adherence to DOACs was associated with lower risks of ischemic stroke, MI, and all-cause death compared to non-adherence, while adherence was not associated with the risk of intracranial hemorrhage. Commitment programs to improve adherence in AF patients could enhance drug effectiveness and safety.

A retrospective analysis compared AF patients aged 80 and above prescribed LDDOAC or warfarin (W), revealing differences in baseline characteristics and outcomes. While there were no significant differences among different LDDOAC medications, major bleeds and mortality rates varied between the groups, highlighting the need for further randomized trials in this population.

Endovascular Treatment for Acute Basilar Artery Occlusion: A Fragility Index Meta-Analysis

Summary: Recently completed randomized controlled clinical trials have provided high-quality evidence regarding the use of endovascular treatment (EVT) in patients with acute basilar artery occlusion (BAO). A systematic review and meta-analysis of available trials showed that EVT combined with best medical treatment (BMT) is associated with higher likelihood of good functional outcomes and lower risk of death at 3 months compared to BMT alone, despite the higher risk of bleeding.

Multiple cerebral microinfarcts: an uncommon presentation of Cerebral Amyloid Angiopathy-related inflammation

Summary: A 71-year-old man presented with symptoms including headache, vomiting, disorientation, and impaired consciousness. After an extensive diagnostic work-up, brain biopsy confirmed the diagnosis of probable CAA-ri. Treatment with methylprednisolone resulted in clinical improvement and resolution of gadolinium enhancement on MRI. Follow-up imaging showed an increased number of cerebral microbleeds, consistent with CAA-ri neuroimaging criteria.

Summary: This study aimed to identify predictors of good functional outcome after symptomatic intracerebral hemorrhage (SICH) following intravenous thrombolysis for acute ischemic stroke. The study analyzed patient data from the Safe Implementation of Treatment in Stroke-International Stroke Thrombolysis Register (SITS-ISTR) and found that higher baseline National Institutes of Health Stroke Scale (NIHSS) score and 24-hour NIHSS score were associated with lower likelihood of achieving good and excellent functional outcomes at 3 months. Age, baseline NIHSS score, 24-hour NIHSS score, admission glucose values, and hematoma location were identified as predictors of mortality and disability at 3 months.

Factor XIa inhibition may offer a novel, safe and effective antithrombotic option for stroke prevention. However, the results of this meta-analysis showed that treatment with factor XIa inhibitors did not reduce the risk of ischemic stroke compared to controls.

Summary: This study aimed to assess the prevalence of baseline characteristics, clinical and radiological findings, and outcomes among patients with cerebral amyloid angiopathy (CAA) in Greece. The results showed that focal neurological deficits, cognitive decline, and transient focal neurological episodes were the most common clinical manifestations, while hemorrhagic lesions were the most prevalent neuroimaging findings.

Summary: The study aimed to evaluate the prevalence and severity of PLMS in acute stroke patients, and to clarify the association between PLMS and coexisting SDB. The results showed that PLMS were common and mostly severe in acute stroke patients, and had a significant impact on stroke outcomes. Therefore, early detection and treatment of PLMS are important for stroke patients.

Acute reperfusion treatment and secondary prevention of cancer-related stroke: comprehensive overview and proposal of clinical algorithm

Summary: Cancer-related stroke (CRS) is a clinical challenge with high recurrence and mortality rates. There are limited recommendations for CRS management. This comprehensive overview collects and summarizes studies on acute reperfusion and secondary prevention treatments for cancer patients with ischemic stroke, focusing on antithrombotic agents. The findings suggest that acute reperfusion is generally safe, but functional outcomes are often poor. Anticoagulation is necessary for many patients, with low-molecular weight heparins being the treatment of choice. Other targeted treatments and conventional cerebrovascular risk factors should also be considered. Urgent randomized-controlled clinical trials are needed to determine the most effective management options.

International beliefs and head positioning practices in patients with spontaneous hyperacute intracerebral hemorrhage

Summary: This study aimed to investigate the beliefs and practices of clinicians regarding head positioning in patients with hyperacute intracerebral hemorrhage (ICH). The results of the survey showed that there is no consensus among clinicians on the optimal head position for these patients, and they remain unconvinced by previous research findings. Further research is needed to examine the effects of head positioning on clinical outcomes in hyperacute ICH.
