Clinical and laboratory study of ocular rosacea in northern Greece

Background The prevalence of ophthalmic involvement in rosacea is probably higher than previously presumed and varies considerably among several studies. Objective This study aimed to determine the incidence of ocular disease among a population of rosacea patients in Northern Greece, to objectively determine the presence of eye dryness in rosacea patients with and without clinical ophthalmic involvement and correlate the severity of ocular disease with the severity of cutaneous rosacea. Methods One hundred patients with rosacea were assessed for the stage of their disease and examined for ocular symptoms and signs. In 24 of them the tear break up time (TBUT) and Schirmer test were performed in each eye, along with 24 controls. Results A total of 33 patients (33%) were positive for ophthalmic findings. The most frequent symptoms and signs were burning sensation and tearing, and conjunctivitis and blepharitis, respectively. Eleven patients with ophthalmic manifestations had mild to moderate erythematotelangiectatic rosacea, 17 had moderate papulopustular rosacea and four exhibited findings of phymatous rosacea. The total mean value of patients' Schirmer tests was significantly lower compared with the healthy controls (P < 0.0001). Mean TBUT was shorter in the rosacea group than that in the age- matched controls (P < 0.0001). Conclusion Ocular involvement in rosacea is a common phenomenon with eye dryness being an early sign. Tear function tests, like Schirmer test and TBUT, although not specific, could contribute to the screening and early diagnosis of the disease, to prevent the potential development of sight- threatening conditions.

Summary: Sarcoidosis and sarcoid-like reactions can develop in cancer patients treated with immune checkpoint inhibitors (ICIs), posing diagnostic and therapeutic challenges. Our review identified 80 cases of ICIs-associated sarcoidosis or SLRs, with melanoma being the most common malignancy. While systemic prednisolone was not always needed for treatment, a significant number of patients had to discontinue ICIs therapy due to these reactions.

Summary: This study retrospectively analyzed the characteristics of pruritus induced by immune checkpoint inhibitors (ICIs) and evaluated the effectiveness of different therapeutic modalities. The results showed that pruritus per se occurred in 22.0% of ICI-induced pruritus patients, with the majority also presenting with additional cutaneous toxicity. Antihistamines and topical regimens were the first-line treatment, while narrow-band UVB, oral steroids, and GABA analogs were used as second-line interventions in resistant cases. The study confirmed the efficacy of current treatment strategies and suggested NBUVB as a potential steroid-sparing alternative.

Clinical and Dermoscopic Characteristics of Cutaneous Sarcomas: A Literature Review

Summary: This study reviewed current data on the clinical and dermatoscopic features of cutaneous sarcomas (CS) in order to facilitate early diagnosis and prompt management. Based on available reports, CS are characterized by mostly unspecific dermatoscopic patterns. However, dermatofibrosarcoma protuberans, Kaposi's sarcoma, and to a lesser extent, cutaneous angiosarcoma, may display distinct dermatoscopic features for early clinical recognition. In conclusion, dermatoscopy, in conjunction with the overall clinical context, may aid suspicion of CS.

Eccrine Poroma: Pathogenesis, New Diagnostic Tools and Association with Porocarcinoma-A Review

Summary: Eccrine poroma is a rare benign adnexal tumor that mainly affects elderly patients. Its pathogenesis is still being investigated and recent gene studies have identified gene fusions as key factors in the development of this tumor. It often presents as a solitary, firm papule on the soles or palms and is known for its clinical and dermoscopic variability. This literature review summarizes current knowledge on the pathogenesis, dermoscopic features, and diagnostic methods for early diagnosis and proper management of this rare tumor, as well as the possible associations between eccrine poroma and its malignant counterpart, eccrine porocarcinoma.
