Apremilast in Palmoplantar Psoriasis and Palmoplantar Pustulosis: A Systematic Review and Meta-analysis

Summary: This review aimed to compare the efficacy of apremilast versus placebo in palmoplantar psoriasis (PP) and palmoplantar pustulosis (PPP), as well as apremilast versus methotrexate in PP. Seventeen original studies were analyzed, including randomized controlled trials, cohort studies, and case series, with a total of 1117 participants. The results showed that apremilast had a significant advantage over placebo in improving palmoplantar-specific outcomes in PP, and had comparable efficacy with methotrexate in PP. However, there were limitations in interpreting the results due to variations in outcome measurements and risk of bias in included studies.

Nail involvement features in palmoplantar pustulosis

Summary: The study found that 66.3% of PPP patients had nail involvement, with the severity of nail involvement correlated with the severity of cutaneous lesions. Compared to PsO, onycholysis and crumbling were more common in PPP, but nail changes in the first digits were less prominent. Risk factors for nail involvement in PPP included female sex, younger age at diagnosis, and higher severity of the disease.

Economic burden of generalized pustular psoriasis and palmoplantar pustulosis in the United States

Summary: This study aimed to estimate the healthcare resource utilization (HCRU) and economic burden of generalized pustular psoriasis (GPP) and palmoplantar pustulosis (PPP) in a commercially insured population in the United States. Results showed that both GPP and PPP patients had high HCRU, with significantly higher total healthcare costs compared to their matched cohorts. Further investigation is needed to understand the drivers of economic burden in these patients and how disease severity impacts HCRU and costs.

Comorbidities in palmoplantar pustulosis: a Swedish population-based register study

Summary: This study compared the comorbidity burden of Swedish patients with palmoplantar pustulosis (PPP) with the general population and patients with psoriasis vulgaris (PV). The results showed a significantly higher comorbidity burden in PPP patients compared to the general population, with increased odds for diseases in different categories (metabolic, cerebrovascular, cardiovascular, psychiatric). Although PPP and PV patients had similar overall comorbidity burdens, the comorbidity profiles differed significantly. Comorbidities such as type 2 diabetes and chronic obstructive pulmonary disorder, which had a high prevalence (> 5%) in PPP and higher odds than PV, may be of particular importance for the specific comorbidity burden of PPP.

Principal Component Analysis to Differentiate Patients with Palmoplantar Pustulosis from Those with Palmoplantar Pustular Psoriasis

Dietary habits in Japanese patients with palmoplantar pustulosis

Summary: PPP is a chronic dermatitis characterized by pustules on the palms and soles. The inflammatory pathway involving TNF-α/IL-23/IL-17 may play a role in its pathogenesis. Dietary habits in Japanese PPP patients include higher BMI, pulses and sugar intake, and lower vitamin A intake, with high BMI and pulse intake associated with PPP. Sodium intake and BMI were correlated with PPPASI. Age and sodium intake may be lower in PPP patients with PAO, suggesting a potential association with the disease. Further studies are needed to investigate the impact of dietary intervention on PPP progression.

Clinicopathologic and Dermoscopic Features of 20 Cases of Spark's Nevus, a Dermoscopic Simulator of Melanoma

Summary: Spark's nevus is a unique type of melanocytic nevus, exhibiting characteristics of both Spitz and Clark nevus. The histologic and dermoscopic features of Spark nevi were analyzed in a retrospective study of 20 excised lesions. The nevi showed symmetrical and well-circumscribed features, with predominantly compound cell morphology and presence of bridging of the nests. Dermoscopically, various patterns and features were observed, including asymmetry, brown color, atypical pigment network, irregular globules, and areas of hyperpigmentation and hypopigmentation. BRAF mutation was common in cases with a history of evolution. Spark nevi are frequently atypical and suspicious of melanoma.

Risankizumab: Daily Practice Experience of High Need Patients

Summary: Psoriasis is a chronic inflammatory disease that affects 29.5 million people worldwide, causing negative impact on quality of life when it affects specific areas. Risankizumab, a humanized monoclonal antibody, effectively treats moderate to severe psoriasis by targeting interleukin-23. Our real-world cases demonstrate the efficacy of risankizumab in improving clinical manifestations and relieving symptoms, without safety concerns.
