GFR Estimation in Adolescents and Young Adults

The performance of creatinine-based equations to obtain the estimated GFR in adolescents and young adults is poorly understood. We assessed creatinine-based GFR estimating equations in a cross-section of 751 adolescents and young adults (10-54 measurements), using inulin clearance (measured GFR [mGFR]) as the reference method. We evaluated the following: Cockcroft-Gault, four-variable Modified Diet in Renal Disease, and the Chronic Kidney Disease Epidemiology Collaboration equations for adult participants, as well as the Schwartz 2009 and Schwartz-Lyon equations for pediatric age groups. Participants ranged in age from 10 to 26 years (mean 16.8 years); we divided the population into four groups according to age (1012 years, 13-17 years, 18-21 years, and 21-25 years). Evaluation of the agreement between these formulas and mGFR (e.g., correlation, Bland-Altman plots, bias, and accuracy) showed that there was a good correlation between mGFR and both pediatric formulas in all age groups, whereas the adult formulas substantially overestimated mGFR. In conclusion, we recommend the use of pediatric equations to estimate GFR from childhood to early adulthood.

Summary: The study aimed to compare 24-hour glycemic levels between obese pregnant women with normal glucose tolerance and non-obese pregnant women. The results showed that obesity in pregnancy was associated with higher glycemic levels even in the presence of normal glucose tolerance test findings. Obese pregnant women had significantly higher blood sugar levels at various time points compared to non-obese pregnant women.

Summary: The Cockcroft-Gault creatinine-based equation is still commonly used for estimating glomerular filtration rate for drug dosage adjustment, but it has been found to perform worse than more recent equations in terms of bias, imprecision and accuracy. Specifically, the CG equation showed poor ability to estimate GFR overall and in analyses stratified for mGFR, age, and BMI, as well as inferior classification of patients in mGFR staging compared to newer equations.

The study found that CKD was a significant long-term consequence, either with glomerular dysfunction or with the more insidious tubular dysfunction, and emphasized the importance of specialized long-term nephrology follow-up for these patients.

Summary: This study compared and validated the performance of the new Chronic Kidney Disease Epidemiology Collaboration equation (CKD-EPIAS) with the European Kidney Function Consortium (EKFC) and Lund-Malmo Revised (LMREV) equations in European and African cohorts. The results showed that the EKFC equation had the best performance in both European and African populations, while CKD-EPIAS had suboptimal performance.

Summary: Objective: To determine the incidence of congenital hypothyroidism (CH) over a 10-year period at the Reference Service in Neonatal Screening of the state of Rio Grande do Sul (RSNS-RS). The overall incidence of CH was 42.1 per 100,000 newborns screened, with a sensitivity of 97% and specificity of 11% for the neonatal TSH cutoff value of 9 mIU/L.
