Summary: Decedents with ADRD in nonprofit NHs, Alzheimer's units, higher licensed nurse staffing, and more competitive markets had better EOL care/outcomes. State Medicaid NH payments modifications may promote better EOL care/outcomes. Future research on NH care practices associated with Alzheimer's units is needed to identify mechanisms for promoting higher-quality EOL care.

Summary: Around 13.7% of nursing home residents with advanced dementia received antimicrobials in the 2 months prior to the study. Facility-level characteristics, such as having a full time nurse practitioner/physician assistant and fewer existing infectious disease practices, were significantly associated with antimicrobial use, while resident characteristics were not independently associated.

Summary: The study found that while overall QoL in YOD patients in nursing homes remained relatively stable over 18 months, there were multidirectional changes in QoL subscales, with some scores increasing and some decreasing. Patients with higher levels of QoL and more advanced dementia were more likely to experience a decline in QoL over time.

Summary: Beta-blockers are commonly prescribed to nursing home residents in their last year of life, but discontinuation occurs earlier in those where clinicians perceive limited life expectancy or poor prognosis, indicating potential for reducing polypharmacy towards end of life.

Summary: This study aimed to develop question prompt lists (QPLs) for family caregivers of nursing home residents with advanced dementia and explore cross-national differences. The findings revealed that family caregivers have many questions about dementia palliative care, but the local context may influence the specific questions asked.

Summary: This study identifies and evaluates four categories of benefits associated with not living in a nursing home (NH), focusing on the Quality Adjusted Life Year (QALY) outcome effect. The results show that NHs have direct and indirect effects on the quality of life and life expectancy through increased dementia symptoms. The valuation of QALY effects is based on literature regarding the value of a statistical life year. Additionally, the study considers the savings from not incurring expenses for a NH as a fifth category of foregone benefits. The total losses incurred amount to $1.93 trillion.

Summary: This study explores racial disparities in end-of-life hospitalizations for residents with ADRD within and across nursing homes. The results show higher risk of hospitalizations for black decedents compared to whites, even among those with severe cognitive impairment. Disparities exist both within and across facilities, indicating systemic differences in care quality based on residents' race.

Summary: This study examined the association between COVID-19 outbreaks, access to geriatric professionals, care aide burnout, and the quality of life (QoL) of nursing home residents. The results showed that recent COVID-19 outbreaks, high levels of emotionally exhausted care aides, and lack of access to geriatric professionals were significantly associated with poorer QoL among nursing home residents.

Summary: This study aimed to evaluate the process of daily going outside in a nursing home garden and explore the effect of garden use on quality of life and neuropsychiatric symptoms in persons with dementia. Results showed that caregivers experienced and observed benefits of going outside for themselves, in residents and relatives. Incorporating daily garden use does not imply an additional task, but rather rearranging priorities and doing the usual activities outside a part of the time.

Summary: The purpose of the study was to evaluate the impact of an ACP Video Program on documented DNH orders among nursing home residents with advanced illness. The results showed that compared to usual care, the ACP Video Program intervention increased the proportion of residents with new DNH orders, but did not significantly reduce hospitalizations.

Summary: This study investigated factors associated with feeding tube (FT) placement in advanced dementia among nursing home residents in Texas. It found that age, race, provider type, and regional differences played a role in the prevalence of FT placement. Regional, racial, and ethnic variations suggest opportunities to improve the quality of end-of-life care for minorities living in border towns.

Summary: This study explored the experiences of nursing home staff and proxies during the COVID-19 pandemic. Staff faced difficulties implementing infection control policies for advanced dementia residents, while proxies emphasized the challenges of being separated from their loved ones. Additionally, the majority of proxies expressed deep appreciation for the care provided by nursing home staff.

Summary: Functional dyspepsia (FD) is associated with abnormal gastric signals affecting the vagus nerve, which leads to changes in affective and pain behavior in key brain regions. FD rats exhibit increased vagal activity in response to gastric distention, as well as gastric mast cell hyperplasia and alterations in Crh signaling in the amygdala. Subdiaphragmatic vagotomy reverses these changes and restores behavior, suggesting a bottom-up pathway involving the gut-brain axis in the pathogenesis of FD symptoms.

Summary: Bipolar depression (BD-D) is a common and challenging condition to treat. There are relatively few approved treatment options for BD-D, with most of them being second-generation antipsychotics. Long-term use of antipsychotics carries a significant risk of adverse effects, and the decision to use them must be weighed against the risk of relapse when patients are off medications. This article reviews commonly used treatments for BD-D, discusses the controversial topic of antidepressant use, and summarizes emerging treatment options and ongoing clinical trials.
