Summary: Blunt thoracic aortic injury (BTAI) is a potentially fatal condition that requires prompt recognition and urgent management. Clinical manifestations of BTAI are not straightforward and may lead to misdiagnosis. The severity of aortic injury is crucial in determining mortality and morbidity rates, as well as guiding treatment decisions, especially when other organs are also involved. Delayed endovascular repair has become the mainstay of treatment for hemodynamically stable patients, providing lower perioperative mortality and morbidity rates compared to open surgical repair. However, concerns exist regarding long-term surveillance and radiation exposure, especially in younger patients. This paper aims to provide an update on diagnostic modalities and treatment strategies for BTAI patients.

Summary: This study retrospectively analyzed the clinical data of consecutive patients diagnosed with blunt abdominal aortic injury in children from 2008 to 2019 at a medical institution. The results showed that delayed aortic repair is safe for most pediatric patients with blunt abdominal aortic injuries and preserved distal perfusion to the lower extremities. Transferring patients to a tertiary center with vascular expertise is a safe and feasible management strategy. However, a more structured surveillance strategy is needed to improve the follow-up rate.
