Prognostic Value of Fractional Flow Reserve Linking Physiologic Severity to Clinical Outcomes

BACKGROUND Fractional flow reserve (FFR) has become an established tool for guiding treatment, but its graded relationship to clinical outcomes as modulated by medical therapy versus revascularization remains unclear. OBJECTIVES The study hypothesized that FFR displays a continuous relationship between its numeric value and prognosis, such that lower FFR values confer a higher risk and therefore receive larger absolute benefits from revascularization. METHODS Meta-analysis of study- and patient-level data investigated prognosis after FFR measurement. An interaction term between FFR and revascularization status allowed for an outcomes-based threshold. RESULTS A total of 9,173 (study-level) and 6,961 (patient-level) lesions were included with a median follow-up of 16 and 14 months, respectively. Clinical events increased as FFR decreased, and revascularization showed larger net benefit for lower baseline FFR values. Outcomes-derived FFR thresholds generally occurred around the range 0.75 to 0.80, although limited due to confounding by indication. FFR measured immediately after stenting also showed an inverse relationship with prognosis (hazard ratio: 0.86, 95% confidence interval: 0.80 to 0.93; p < 0.001). An FFR-assisted strategy led to revascularization roughly half as often as an anatomy-based strategy, but with 20% fewer adverse events and 10% better angina relief. CONCLUSIONS FFR demonstrates a continuous and independent relationship with subsequent outcomes, modulated by medical therapy versus revascularization. Lesions with lower FFR values receive larger absolute benefits from revascularization. Measurement of FFR immediately after stenting also shows an inverse gradient of risk, likely from residual diffuse disease. An FFR-guided revascularization strategy significantly reduces events and increases freedom from angina with fewer procedures than an anatomy-based strategy. Factors associated with kalemia in renal disease

Association between eating behaviour and 13-year cardiovascular damages in the initially healthy STANISLAS cohort

Summary: This study aimed to assess the associations between eating behavior and cardiovascular damage or metabolic syndrome after 13 years. The results showed that emotional eating was associated with an increased risk of diastolic dysfunction after 13 years. External eating was slightly associated with lower pulse-wave velocity. These findings suggest that cardiovascular prevention should take into account eating behavior and include teaching emotion regulation skills.

Summary: This study aimed to evaluate the predictive value of TRI- SCORE for in-hospital mortality in redo ITVS patients and compare its accuracy with conventional surgical risk scores. The study found that TRI- SCORE was the only surgical risk score associated with in-hospital mortality, with an AUC value of 0.83, much higher than logistic EuroSCORE and EuroSCORE II. Therefore, TRI- SCORE can guide the clinical decision-making process.

High-sensitivity calcium biosensor on the mitochondrial surface reveals that IP3R channels participate in the reticular Ca2+ leak towards mitochondria

Summary: Genetically encoded biosensors based on fluorescent proteins (FPs) are widely used to monitor calcium ion (Ca2+) dynamics and sub-cellular spatial distribution in cells. The mitochondrial-associated membranes (MAMs), which connect the endoplasmic reticulum (ER) and mitochondria, have recently gained attention due to their role in calcium homeostasis and mitochondrial function. However, the sensitivity of the existing Ca2+ biosensors on the outer-mitochondrial membrane (OMM) is not sufficient to measure sub-micromolar changes in Ca2+ concentration. In this study, a new highly sensitive Ca2+ biosensor was developed and demonstrated to detect smaller differences in Ca2+ levels in or near the MAMs. Notably, the study also revealed the endogenous activity of IP3-receptors in the Ca2+ leak channel on the OMM during hypoxia or when SERCA activity is blocked.

Summary: The crucial role of angiogenesis in clot resolution and fibrothrombotic obstruction of the pulmonary arterial bed in CTEPH was validated using a rabbit model. Inhibiting intrathrombus angiogenesis resulted in elevated pulmonary arterial pressure, increased pulmonary vascular resistance, and right ventricular hypertrophy, accompanied by pulmonary vessel remodeling and inflammation.

Summary: The prevalence of carotid artery stenosis (CAS) in patients undergoing transcatheter aortic valve implantation (TAVI) is not well-documented. This study aimed to investigate the impact of CAS, especially severe or bilateral CAS, on the prognosis after TAVI. The results showed that while CAS as a whole did not predict neurovascular complications, bilateral CAS was associated with an increased risk of stroke after TAVI.
