Summary: This study aimed to describe the ECG phenotype of childhood HCM in a large, international, multi-centre cohort and investigate its role in risk prediction for arrhythmic events. The results showed that ECG abnormalities were common in childhood HCM, but none of them were associated with 5-year arrhythmic events, suggesting limited role of baseline ECG phenotype in improving risk stratification in childhood HCM.

Clinical and Genetic Screening for Hypertrophic Cardiomyopathy in Paediatric Relatives: Changing Paradigms in Clinical Practice

Summary: Hypertrophic cardiomyopathy (HCM) is a significant cause of illness and death in children. Most cases are caused by gene variants in the cardiac sarcomere components, inherited as an autosomal dominant trait. Recent studies have shown that phenotypic expression of HCM can occur in young children, highlighting the need for clinical screening and genetic testing in pediatric relatives. The multidisciplinary care of HCM-affected children and families relies on genomics.

Identification of circRNA-miRNA-mRNA regulatory network and its role in cardiac hypertrophy

Summary: By analyzing gene expression files in the GEO database, this study identified differentially expressed circRNAs, miRNAs, and mRNAs in HCM, and predicted the target molecules of circRNAs and miRNAs. A ceRNA network was constructed by calculating the intersection of these differentially expressed RNA molecules and their expected targets. Functional enrichment analysis revealed the potential etiology of HCM, which is associated with calcium channel release. The qPCR results showed differential expression levels of circRNA, miRNA, and mRNA, suggesting their potential as noninvasive indicators for early screening and prognostic prediction of HCM.

Early identification of at-risk individuals is crucial to prevent lethal episodes. Genetic analysis can identify deleterious rare variants in genes encoding desmosome proteins, but other factors like microRNAs may also modulate disease onset and outcome. Understanding the role of microRNAs may have clinical implications as potential biomarkers in arrhythmogenic cardiomyopathy.

Current therapies focus on symptom management and prevention of sudden cardiac death, but there is a need for improved treatments targeting heart failure and atrial fibrillation. With advancements in understanding the genetic and molecular basis of HCM, novel treatments are expected in the near future.

Association of QTc Interval and V4-S Wave With Appropriate ICD Therapy in Hypertrophic Cardiomyopathy

Summary: This study investigated the relationship between electrocardiogram (ECG) indicators and the risk of appropriate implantable cardioverter-defibrillator (ICD) therapy in patients with hypertrophic cardiomyopathy (HCM). The results showed that long or deep S wave in V4 lead and QTc interval were independent risk factors for appropriate ICD therapy. The new model that combined the QTc interval and S wave anomalies in V4 lead significantly improved the prediction effect.

These findings suggest that deep intronic variants may have a greater impact than previously thought, highlighting the importance of accurate classification and follow-up for family members.

Exposure to secondhand smoke is associated with increased left ventricular mass

Summary: Chronic hypertension is associated with left ventricular hypertrophy, and recent evidence suggests that secondhand smoke exposure is also related to chronic hypertension. This study found that exposure to secondhand smoke is associated with increased left ventricular mass index among non-smokers, with a dose-response relationship between level of exposure and left ventricular mass index. These results were consistent across different subgroups, reinforcing the harmful effects of passive smoking on the cardiovascular system.
