CHADS2 and CHA2DS2-VASc Scores in the Prediction of Clinical Outcomes in Patients With Atrial Fibrillation After Catheter Ablation

Objectives This study aimed to evaluate whether CHADS(2) and CHA(2)DS(2)-VASc scores are useful for risk stratification in patients after catheter ablation of atrial fibrillation (AF). Background AF is associated with increased risk of cardiovascular events. However, limited data are available on the predictors of adverse events in patients with AF after catheter ablation. Methods A total of 565 patients with AF who underwent catheter ablation were enrolled in the study. The clinical end-point was occurrence of thromboembolic events (ischemic stroke, transient ischemic attack, peripheral embolism, or pulmonary embolisms) or death during follow-up after catheter ablation. Results During a follow-up of 39.2 +/- 22.6 months, 27 patients (4.8%) experienced adverse events. Both the CHADS(2) and CHA(2)DS(2)-VASc scores were useful predictors of events in separate multivariate models. The areas under the receiver-operator characteristic curves based on the CHADS(2) and CHA(2)DS(2)-VASc scores in predicting events were 0.785 and 0.830, respectively. Although the difference did not reach statistical significance (p = 0.116), the CHA(2)DS(2)-VASc score could be used to further stratify the patients with CHADS2 scores of 0 or 1 into 2 groups with different event rates (7.1% vs. 1.1%, p = 0.003) at a cutoff value of 2. Conclusions The CHADS2 and CHA(2)DS(2)-VASc scores are useful predictors of adverse events after catheter ablation of AF. Summary: This study investigated the 10-year outcomes of radiofrequency catheter ablation (RFCA) in patients with non-paroxysmal atrial fibrillation (AF). It was found that single RFCA procedure had unsatisfactory results, and multiple procedures were required for adequate rhythm control. Enlarged left atrium, presence of non-pulmonary vein triggers, and undergoing complex fractionated atrial electrogram (CFAE) ablation were independent predictors for recurrent atrial tachyarrhythmia after a single procedure.

Summary: The recent study showed that catheter ablation did not benefit patients with AF, HF, and severely reduced LVEF. However, it was found that RFCA could improve outcomes in HF patients with AF and impaired LVEF. The RFCA group had lower mortality, HF hospitalization, and AF recurrence rates, as well as improved LVEF and quality of life.

Clinical outcomes and structural remodelling after ablation of atrial fibrillation in heart failure with mildly reduced or mid-range ejection fraction

Summary: This study retrospectively compared the efficacy of catheter ablation (CA) with medical therapy (MT) in atrial fibrillation (AF) patients with mildly reduced or mid-range ejection fraction (HFmrEF). The results showed that CA was associated with reduced HF hospitalization and all-cause mortality compared to MT. Additionally, CA led to significant improvements in left ventricular ejection fraction (LVEF) and decreased left atrium (LA) diameter. It is suggested that early rhythm control with CA should be considered in AF patients with HFmrEF.

Summary: This study aimed to evaluate the abnormal conduction velocity using isochronal late activation mapping and predict the recurrence of atrial fibrillation (AF) after pulmonary vein isolation. The presence of deceleration zone was associated with higher risk of AF recurrence. Therefore, isochronal late activation mapping is of great importance in predicting the outcome of AF ablation in patients with paroxysmal AF.

Characterization and identification of atrial fibrillation drivers in patients with nonparoxysmal atrial fibrillation using simultaneous amplitude frequency electrogram transform

Summary: Real-time simultaneous amplitude frequency electrogram transform (SAFE-T) can be used to identify and characterize the drivers of nonparoxysmal atrial fibrillation (NP AF) during sinus rhythm (SR). The study found that termination sites during AF exhibited high similarity index (SI) and high SAFE-T values during SR, indicating the presence of high frequency components. This suggests that SAFE-T mapping is a feasible method for identifying and characterizing AF drivers.

Ablation of atrial fibrillation and dementia risk reduction during long-term follow-up: a nationwide population-based study

Summary: This study examined the epidemiological characteristics of new-onset dementia in atrial fibrillation (AF) patients and the association between catheter ablation and different subtypes of dementia. A population-based cohort study was conducted using data from the Taiwan National Health Insurance Research Database. The results showed that AF patients had a higher incidence of all types of dementia, with Alzheimer's disease being less likely to occur in AF patients undergoing catheter ablation.

Summary: This study aimed to investigate the risk factors for left ventricular (LV) cardiomyopathy and the outcomes of patients with ventricular arrhythmia (VA) originating from the LV summit (LVS). The results showed that QRS duration (QRSd) and absolute earliest activation time discrepancy (AEAD) were independent factors associated with deteriorating LV systolic function in patients with VA, and catheter ablation could reverse LV remodeling. Narrower QRSd and better LV ejection fraction (LVEF) were associated with better recovery of LV function after ablation.

Summary: This study investigated the electrophysiological characteristics of recurrence after catheter ablation (CA) therapy in patients with hypertrophic cardiomyopathy (HCM) and compared long-term clinical outcomes with patients who did not undergo CA. The most common cause of recurrence was pulmonary vein reconnection, followed by non-pulmonary vein triggers, cavotricuspid isthmus flutter, and atypical flutter. Thyroid disease, diabetes, and non-paroxysmal AF were identified as independent predictors of recurrence. Repeat CA showed better arrhythmia-free outcomes compared to drug escalation therapy. Patients who underwent CA had significantly better clinical outcomes in terms of all-cause mortality, heart failure hospitalization, and left atrial reverse remodeling.

Fractal complexity alternations in paroxysmal atrial fibrillation patients with and without recurrence after pulmonary vein isolation

Summary: This study aimed to investigate the differences in nonlinear heart rate variability (HRV) between patients with and without recurrence before and after pulmonary vein isolation (PVI) for paroxysmal atrial fibrillation (PAF). The results showed that the non-recurrence group had significant decreases in the high-frequency component (HF), root mean square of successive RR interval differences (RMSSD), and Poincare Plot index SD1 1-3 months after PVI, and an increase in DFA(slope2) 6-12 months after PVI. The non-recurrence group also had significant decreases in the LF/HF ratio and DFA(slope1) 1-3 and 6-12 months after PVI, while no significant changes were observed in the recurrence group. These findings suggest that changes in vagal tone, long-term fractal complexity, and sympathetic tone as well as short-term fractal complexity play crucial roles in AF recurrence following PVI.

High-density characterization of the sinus rhythm: a new functional substrate map of scar-related atrial tachycardia

Summary: This study aims to investigate the electrophysiological characteristics and clinical implications of the critical isthmus (CI) for the maintenance of scar-related reentrant atrial tachycardias (ATs) during sinus rhythm (SR), and to map the functional substrate of the CI during SR. The results showed that the CI regions during SR were characterized by low voltage, conduction slowing, and fractionated electrogram. Targeting these regions achieved a high rate of termination or change of the reentrant circuit.

Risks of Ventricular Tachyarrhythmia and Mortality in Patients with Amyloidosis - A Long-Term Cohort Study
