Summary: The article discusses the preference for antithrombotic therapy in patients with acute coronary syndromes and the difference between preference and actual practice. It also mentions a new study, but does not explicitly state whether this new study can explain the discrepancy.

Summary: Patients with CAD and prior CVE present a high risk of both ischemic and bleeding events, making antithrombotic management challenging. Balancing the risks of ischemia and bleeding is crucial, especially when considering long-term antithrombotic therapy to prevent major bleeding complications.

Summary: The study found that P2Y(12) inhibitor monotherapy following short-term DAPT significantly reduced the risk of BARC type 3 or 5 bleeding compared to standard DAPT, without significant difference in reducing the composite CVD outcomes.

Summary: This study aimed to examine the effect of available P2Y12 inhibitors on ischaemic and bleeding endpoints in older adults with ACS. The results showed that prasugrel was associated with a reduction in ischaemic events, while clopidogrel was associated with a reduction in bleeding events. Ticagrelor had the best effect on reducing stent thrombosis, but may not be the optimal choice for older adults.

Summary: This study found that clinical and genetic factors have differential effects on P2Y12 inhibitor reactivity with clopidogrel and prasugrel in patients with coronary artery disease. The severity of these factors also has a different impact on high platelet reactivity (HPR).

Summary: In acute coronary syndrome following percutaneous coronary intervention, newer P2Y12 inhibitors (ticagrelor and prasugrel) reduce cardiovascular outcomes compared to clopidogrel, but have a higher bleeding risk. This study examines the safety of each inhibitor in a real-world population.

Summary: This study comprehensively analyzed the alterations in m6A modification in clopidogrel-resistant (CR) patients. The results showed that the expression of m6A regulators, the RNA transcriptome, and the m6A landscape were altered in CR patients. Functional enrichment analysis revealed the potential involvement of inflammatory pathways and insulin signaling pathways in the pathological processes underlying CR.

Summary: This study compared the effects of concomitant administration of cangrelor and prasugrel with prasugrel alone in patients undergoing percutaneous coronary intervention (PCI). The results showed that concomitant administration of cangrelor and prasugrel led to a marked increase in platelet reactivity after stopping cangrelor infusion, indicating the presence of a drug-drug interaction.

Summary: This meta-analysis confirms that carriers of CYP2C19 LoF alleles have a significantly increased risk of stroke and composite events recurrence worldwide, especially in Asian populations. Therefore, alternative antiplatelet therapy should be thoroughly investigated for patients with intermediate or poor metabolism.

Summary: Antiplatelet therapy is crucial for preventing secondary ischemic heart disease. After PCI, temporary DAPT, which consists of aspirin and an oral P2Y12 receptor blocker, is recommended. However, long-term DAPT increases the risk of bleeding compared to aspirin monotherapy. To reduce bleeding, shorter DAPT duration or switching to P2Y12 inhibitor monotherapy have been suggested. This survey reviews the evidence and provides clinical guidance on the use of P2Y12 inhibitor monotherapy after DAPT cessation following PCI.

Summary: The treatment of acute coronary syndrome (ACS) in elderly patients is challenging due to their characteristics and lack of specific recommendations. This review summarizes the current evidence on oral antithrombotic therapy in elderly patients, discussing peculiarities of dual antiplatelet therapy (DAPT) in ACS management and analyzing antithrombotic strategies in patients with atrial fibrillation (AF) and coronary artery disease (CAD).

Summary: This study evaluates the safety and efficacy of guided vs. routine selection of potent P2Y(12) inhibitors in patients with acute coronary syndrome (ACS). The findings suggest that a guided selection of P2Y(12) inhibiting therapy provides a more favorable balance between safety and efficacy compared to routine selection.

Summary: LVAD therapy is often complicated by issues such as thrombosis and bleeding. This study suggests that noninvasive flow imaging techniques, such as 2D echo color Doppler velocimetry, may be used alongside standard clinical methods to optimize LVAD settings and minimize blood stasis on an individual basis. LVAD support was found to reverse the increase in blood stasis associated with dilated cardiomyopathy, but did not reduce intraventricular shear exposure.

Summary: There are geographic variations in the management and outcomes of CF-LVAD patients between the US and Europe, but overall survival rates are similar. Disparities in donor characteristics, duration of support, and adverse events between the US and Europe may explain differences in the utilization of mechanical bridging strategies.

Summary: The study found that LV function in stable OHT recipients reached a steady state 1 month after transplantation, while RV function gradually improved during the first year after OHT, indicating delayed RV adaptation compared to LV. Summary: This study aimed to evaluate if an INR remote monitoring system along with a standardized warfarin management protocol could improve warfarin time in therapeutic range (TTR) for patients with LVADs. The results showed that the average TTR was significantly higher in the remote monitoring group compared to the standard monitoring group, suggesting that remote monitoring may have the potential to improve anticoagulation-related outcomes in patients with LVADs.
