Cessation of Immunomodulatory Medication Use in Dermatomyositis: A Single-Center Cohort Study

Summary: The frequency of discontinuing systemic immunomodulatory therapy in adults with dermatomyositis (DM) and the factors associated with medication cessation were evaluated. A cohort of adult DM patients seen in a rheumatology/dermatology clinic between 2013 and 2020 was studied, and it was found that approximately 20% of the patients discontinued all immunomodulatory medications. Patients with clinically amyopathic disease, anti-MDA5, anti-NXP2, and anti-SAE1 antibodies were more likely to cease medication.

Summary: This study aimed to establish a framework by which experts define disease subsets in systemic sclerosis associated interstitial lung disease (SSc-ILD). The results showed that using the proposed conceptual framework, international experts were able to achieve a consensus on classifying SSc-ILD patients along the dimensions of disease severity, risk of progression and progression over time.

Summary: Results from the SCOT clinical trial showed that HSCT had significant benefits over CTX in patients with systemic sclerosis. The objective of this study was to test the hypothesis that transplantation stabilizes the autoantibody repertoire in patients with favorable clinical outcomes. Analysis of autoantibody profiles revealed significant differences between HSCT and CTX-treated patients, suggesting that HSCT alters the autoantibody repertoire while CTX treatment does not.

Summary: The patient experience of SSc-calcinosis was analyzed in an international cohort, leading to a better understanding of the pervasive disability, frustration, symptom distress, and uncertainty associated with daily living with the condition. Patient observations and self-management behavior offer opportunities for education of clinicians and patients, with a need for self-management guidance. The findings informed the development of a novel PROM, the MCQ, for assessing the severity and impact of SSc-calcinosis in clinical studies and practice.

Summary: This study further validated the ACR-EULAR Myositis Response Criteria (MRC) by assessing the contribution of each core set measure, the frequency of improvement in strength vs extramuscular activity, the representation of patient-reported outcome measures (PROM), and the frequency of core set measure worsening. The results showed that improvement in strength was the main contributing factor, and the majority of patients also had improvement in PROM.

Limited evidence supports the effectiveness of Janus kinase inhibitors, while immunosuppression or anti-platelet agents have insufficient data to support their use. Further research is needed to define the optimal treatment regimen.

Summary: The study aimed to understand the symptoms and impacts of Raynaud phenomenon (RP) in patients with systemic sclerosis (SSc) and evaluate the content validity and usability of the Raynaud Diary, a new electronic patient-reported outcome (PRO) measure for RP. Participants reported frequent RP symptoms such as color change, numbness, tingling, pain, and discomfort. Common triggers for RP attacks included temperature-related factors and stress. The participants demonstrated understanding of the Raynaud Diary instructions and indicated that they could use it to record the worst severity of individual RP symptoms.

Endothelial Biomarkers of Systemic Sclerosis-Associated Pulmonary Hypertension

Summary: The study shows that certain endothelial biomarkers (ADMA, sEng, and PTX-3) can determine the risk of SSc-PH and differentiate between SSc-PH subgroups. PTX-3 is a promising biomarker for PH risk and a possible marker for precapillary PH, but ADMA and sEng do not show significant differences in this regard.
