Summary: Inherited metabolic disorders with gastrointestinal symptoms involve dysfunction in the esophagus, stomach, small and large intestines, and pancreas. This study summarizes the associations of signs and symptoms in 339 inherited metabolic diseases presenting with GI symptoms. Feeding difficulties are the most common abnormality reported, followed by intestinal problems, vomiting, stomach and pancreas involvement, and esophagus involvement.

Summary: The morphological complexity of cells and tissues, whether normal or pathological, is characterized by irregularity and self-similarity across different scales. Fractal geometry holds promise in providing a quantitative measure of the irregularly shaped morphologies and their underlying self-similar temporal behaviors. This article explores the fractal nature of the gastrointestinal system and the potential applications of fractal geometry in clinical gastroenterology and hepatology practice.

Summary: This study aimed to assess the effect of Roux-en-Y gastric bypass (RNY) on Barrett's esophagus (BE) in obese patients. The systematic review and meta-analysis of studies showed that a majority of patients with BE who underwent RNY had remission or improvement, while a significant minority showed no change on follow-up endoscopy.

Summary: This study analyzed the impact of sleeve gastrectomy (SG) on esogastric mucosa in a rat model at 24 weeks postoperatively and found that SG induced gastric foveolar hyperplasia but no esophageal lesions. The results suggest that long-term endoscopic esophageal follow-up recommended in humans after SG to detect Barrett's esophagus may also be useful for detecting gastric lesions.

Summary: Heterotopic pancreas (HP) is a rare developmental anomaly that can cause bowel intussusception and gastrointestinal bleeding. This study analyzed four cases of intussusception caused by incidental HP, finding that patients presented with severe abdominal pain and melena, with confirmation of HP through histopathology after emergency laparotomy. Medical professionals should consider HP as a possible differential diagnosis for gastrointestinal bleeding and bowel intussusception.

Summary: Ultrasound is the preferred modality for diagnosing fetal GI anomalies, with MRI used as a complementary method. While MRI is increasingly utilized for CNS fetal malformations, its role in evaluating fetal GI abnormalities is not yet established. More attention should be given to the clinical implications of MRI investigations following initial screening by ultrasound.

Summary: This study compared the outcomes between ileal and jejunal anastomoses in pancreas transplant surgery. There were no significant differences in recipient or donor factors, immunosuppression regimen, overall patient or graft survival, or gastrointestinal complications between the two groups. However, ileal anastomosis was associated with longer hospital stay and cold ischemic time.

Summary: This study investigated the effects of sleeve gastrectomy and Roux-en-Y gastric bypass on gastrointestinal motility, pH, and bile acid concentration. The findings showed that both surgeries increased pouch emptying and gastric emptying, and also led to higher pH levels and bile acid concentrations in the intestines. These results highlight the significant impact of these surgeries on gastrointestinal physiology.

Summary: The study showed that bariatric surgery can improve outcomes in patients with myocardial infarction, with Roux-en-Y gastric bypass and sleeve gastrectomy reducing infarct size and no-reflow area. This effect is associated with decreased levels of leptin and ghrelin, and increased levels of GLP-1, enhancing myocardial tolerance to ischemia-reperfusion injury in non-obese, non-diabetic rats.

Summary: Introduction: Prescription opioids, including those prescribed after surgery, have greatly contributed to the US opioid epidemic. Educating opioid prescribers is a crucial component of ensuring the safe use of opioids among surgical patients. This study implemented an annual education curriculum for new surgical prescribers, resulting in significant improvements in knowledge and comfort levels. However, there remains a persistent knowledge and comfort gap among these prescribers.

Summary: This study demonstrates the potential of using humanized anti-carcinoembryonic antigen (CEA) antibodies conjugated with near-infrared dyes to specifically label gastric cancers in mouse models. Orthotopic models showed bright and specific labeling with more than ten times higher tumor-to-background ratios compared to the control. This tumor-specific fluorescent antibody has promising potential as a clinical tool for improving visualization of gastric cancer margins during surgical resection.

Summary: This scoping review analyzed the literature on the use of broadband personality tests in the bariatric surgical population to optimize weight loss outcomes. The study found significant associations between personality scales and weight loss, but inconsistent reporting of outcome measures made it challenging to draw concrete conclusions. The American Society for Metabolic and Bariatric Surgery recommends standardization of outcome reporting to improve the reliability of predicting weight loss outcomes.
