Protective Role of Simvastatin on Lung Damage Caused by Burn and Cotton Smoke Inhalation in Rats

Background. Smoke inhalation injury is a major comorbid factor in patients with thermal injury and occurs in about 30% of patients with major burns. In addition, inhalation injury reportedly accounts for 20%-84% of the mortality in burned individuals and is associated with higher mortality rates for every age and burn size category. The aim of the present study was to investigate the effects of simvastatin on lung damage with burn and cotton smoke inhalation. Methods. Wistar rats were randomly assigned to three groups: saline treated control group, via an orogastric route (group 1, n = 6), burn (30%) and cotton smoke inhalated group (group 2, n = 6), and simvastatin treated (25 mg/kg/d, via an orogastric route) burn (30%) and cotton smoke inhalated group (group 3, n = 6). Rats were sacrificed at 48 h of the treatments and the trachea and lungs were removed completely. Tissue samples were taken for histopathologic, immunohistopathologic, and biochemical analyses. Univariate analysis of variance coupled with Duncan's posthoc test was performed for statistical evaluation. Results. Lung parenchymal and tracheoepithelial damage was confirmed in group 2 by histopathologic examination. Lung malonedialdehyde (MDA) levels were significantly decreased (P < 0.001), while glutathione (GSH) concentration did not alter in group 2 compared with group 1. Also, immunopathologic data revealed that epithelial iNOS level was elevated, while no modulation was detected in the level of myeloperoxidase (MPO). Simvastatin administration resulted in decreasing the lung parenchymal and tracheoepithelial damage. Tissue MDA levels were decreased significantly (P < 0.001), whereas GSH concentrations were elevated in group 3 compared with group 1 and group 2 (P < 0.001). Simvastatin treatment caused a decrease in epithelial iNOS levels, while MPO levels were not modulated. In addition, simvastatin significantly reduced pulmonary apoptosis in lung injury. Conclusions. Our results have indicated that simvastatin administration seems to play beneficial role in lung injury of rats promoted by combined burn and smoke inhalation. Thus, simvastatin may represent a potential approach to prevent smoke inhalationassociated lung dysfunction. However, the significant decrease in basal oxidant production may cause impairment in cellular signalling processes. (C) 2011 Elsevier Inc. All rights reserved.

Use of Eculizumab in Pediatric Patients with Late Antibody-Mediated Rejection After Kidney Transplantation

Summary: We report 2 pediatric cases of severe late antibody-mediated rejection resistant to conventional immunosuppressive therapy who were successfully treated with eculizumab. Eculizumab therapy effectively reduced the markers of late antibody-mediated rejection and improved kidney function in both patients.

Summary: This study aimed to investigate the relative merits of echocardiography and cardiac magnetic resonance (CMR) with T1 mapping for the assessment of functional adaptation and remodelling of the right ventricle (RV) in heart transplant recipients. The study found that echocardiographic parameters can reflect changes in cardiac function and structure in patients, while CMR is better at reflecting changes in myocardial microstructure.

Positive Effect of Platelet-Rich Plasma on Neoureterocystostomy in Rats: An Experimental Study

Summary: This study was conducted to evaluate the positive effect of platelet-rich plasma on the healing process of an anastomotic wound in a rat model of ureterocystoanastomosis. The results showed that the use of platelet-rich plasma significantly increased tissue hydroxyproline levels, and there were no significant differences in epithelial damage, acute inflammation, or fibrosis between the platelet-rich plasma group and the control group.

Summary: This study investigated the hepatic arterial Doppler ultrasound findings related to cholangitis after liver transplantation. It was found that the hepatic arterial flow, resistive index, and diameter decreased in patients with cholangitis after recovery. These findings can help in the early diagnosis of cholangitis and prevent graft loss.

Choroidal Vascularity Index and Choroidal Thickness Changes Following Renal Transplantation

Cardiac Function in Children After Kidney Transplant

Summary: The aim of this study was to evaluate the gradual changes in left and right ventricle functions in children and young adults before and after renal transplant. The results showed that both systolic and diastolic dysfunctions of both ventricles were common in pediatric renal transplant recipients, and these dysfunctions usually decreased with time. However, improvements in right ventricle dysfunction were slower, even in optimally treated posttransplant patients.

Educating opioid prescribers is a crucial component of ensuring the safe use of opioids among surgical patients. This study implemented an annual education curriculum for new surgical prescribers, resulting in significant improvements in knowledge and comfort levels. However, there remains a persistent knowledge and comfort gap among these prescribers.

Humanized Anti-Carcinoembryonic Antigen Antibodies Brightly Target and Label Gastric Cancer in Orthotopic Mouse Models

Summary: This study demonstrates the potential of using humanized anti-carcinoembryonic antigen (CEA) antibodies conjugated with near-infrared dyes to specifically label gastric cancers in mouse models. Orthotopic models showed bright and specific labeling with more than ten times higher tumor-to-background ratios compared to the control. This tumor-specific fluorescent antibody has promising potential as a clinical tool for improving visualization of gastric cancer margins during surgical resection.

Use of Broadband Personality Tests in Bariatric Surgery: A Scoping Review

Summary: This scoping review analyzed the literature on the use of broadband personality tests in the bariatric surgical population to optimize weight loss outcomes. The study found significant associations between personality scales and weight loss, but inconsistent reporting of outcome measures made it challenging to draw concrete conclusions. The American Society for Metabolic and Bariatric Surgery recommends standardization of outcome reporting to improve the reliability of predicting weight loss outcomes.
