Background. Twenty percent to 40% of patients with node-negative colorectal cancer die of metastatic disease. Detection of cancer cell dissemination has been proposed as a tool to select patients at highest risk for recurrence. In this review, we summarize the evidence for detection with quantitative reverse transcriptase-polymerase chain reaction (RT-PCR) assays of circulating tumor cells (CTCs) in peripheral blood of colorectal cancer patients. Materials and methods. Pubmed and Embase were systematically searched for all English publications relevant to circulating cancer cells, peripheral blood, quantitative RT-PCR (q-RT-PCR), and colorectal cancer. Cross-references and the related articles function were used to broaden the search. Manuscripts reporting on the results of nonquantitative RT-PCR assays were excluded. The study methodology, CTCs detection rates in peripheral blood, and prognostic value were reviewed. Results. Twelve manuscripts on qRT-PCR were retrieved. Stage dependence was found for detection of CTCs in four of 10 studies. From univariate analysis performed for disease-firee survival and overall survival in 4 of 12 studies, there was evidence (P < 0.05) for an effect of the detection of CTCs with qRT-PCR. None of the included trials identified detection of CTCs in peripheral blood as an independent predictor of survival. Conclusion. Quantification of CTCs in peripheral blood holds promise in predicting stage and outcome in colorectal cancer patients. At present, evidence from the literature is too scarce to integrate quantitative RT-PCR assays to detect CTCs into the management of colorectal cancer. (c) 2008 Elsevier Inc. All rights reserved.

Summary: Mutational analysis plays a critical role in guiding therapeutic decision making for patients with advanced-stage gastrointestinal stromal tumors (GISTs). The study evaluated the effectiveness of three targeted next-generation sequencing (NGS) assays in detecting driver alterations in GISTs, leading to the identification of additional mutations beyond the commonly found KIT or PDGFRA mutations. The comprehensive NGS approach proved to be feasible in routine mutational analysis of GISTs and helped reduce the number of Wild Type GISTs.

Summary: This study retrospectively analyzed a single-center cohort of patients with gastroesophageal adenocarcinoma in Belgium, and found a low prevalence of MSI-H and EBV+. Among them, MSI-H tumors were more common at the gastroesophageal junction, and one patient with MSI-H metastatic GEC achieved a complete response with nivolumab.

Prospective cohort study on short-term outcomes of 3D-laparoscopic pancreaticoduodenectomy with stented pancreaticogastrostomy

Summary: This study evaluated the clinical and oncologic outcomes of 3D laparoscopic pancreaticoduodenectomy (3dLPD) with stented pancreaticogastrostomy (sPG) and Roux-en-Y gastroenterostomy (ryGES) in patients with pancreatic or periampullary tumors. The study found that 3dLPD had a high risk of complications, including clinically relevant pancreatic fistula and mortality. However, intraoperatively placed drains were effective in managing complications in some patients.

Summary: Two preoperative prognostic models were created to predict 90-day mortality and survival after major liver resection for perihilar cholangiocarcinoma. The results showed that 17.6% of patients were suitable for surgical treatment, 26.8% of patients were not suitable for surgical treatment, and 66.2% of patients were in the intermediate risk group. These prognostic models can help identify patients with resectable perihilar cholangiocarcinoma who are unlikely to benefit from surgical resection.

Association Between Nil Per Os Status and Intubated Patients Undergoing Surgery

Educating opioid prescribers is a crucial component of ensuring the safe use of opioids among surgical patients. This study implemented an annual education curriculum for new surgical prescribers, resulting in significant improvements in knowledge and comfort levels. However, there remains a persistent knowledge and comfort gap among these prescribers.

Humanized Anti-Carcinoembryonic Antigen Antibodies Brightly Target and Label Gastric Cancer in Orthotopic Mouse Models

Summary: This study demonstrates the potential of using humanized anti-carcinoembryonic antigen (CEA) antibodies conjugated with near-infrared dyes to specifically label gastric cancers in mouse models. Orthotopic models showed bright and specific labeling with more than ten times higher tumor-to-background ratios compared to the control. This tumor-specific fluorescent antibody has promising potential as a clinical tool for improving visualization of gastric cancer margins during surgical resection.

Use of Broadband Personality Tests in Bariatric Surgery: A Scoping Review

Summary: This scoping review analyzed the literature on the use of broadband personality tests in the bariatric surgical population to optimize weight loss outcomes. The study found significant associations between personality scales and weight loss, but inconsistent reporting of outcome measures made it challenging to draw concrete conclusions. The American Society for Metabolic and Bariatric Surgery recommends standardization of outcome reporting to improve the reliability of predicting weight loss outcomes.
