Summary: This study evaluated the effectiveness of platelet-rich plasma (PRP) combined with oral tadalafil in diabetic patients with erectile dysfunction (ED) who did not respond to PDE5 inhibitors. The results showed that PRP therapy significantly improved erectile function and erection hardness in these patients. Furthermore, the level of diabetic control was associated with treatment outcomes.

Summary: Continuing our research on sildenafil analogs as inhibitors of PDE5 for erectile dysfunction, we found that modifications at the N-methylpiperazine ring with hydrophobic region followed by hydrogen bond donor or acceptor region have potential. However, previously identified leads had limitations such as poor pharmacokinetic profile, low aqueous solubility, and poor bioavailability. Therefore, we designed, synthesized, and screened a new series of sildenafil based analogs. Compound 18 showed excellent in vitro activity with selectivity towards PDE5 isozyme, as well as excellent in vivo activity and pharmacokinetic profile. The compound also exhibited excellent cyp inhibition and CaCO2 permeability.

Summary: This study found that in sickle cell disease, decreased haptoglobin levels contribute to abnormal erectile function. Treatment with haptoglobin can improve erectile function by regulating the expression of key proteins involved in the control of penile erection. This suggests that haptoglobin treatment may be an additional strategy to prevent priapism in sickle cell disease.

Summary: In 1998, sildenafil was introduced as the first FDA-approved oral drug for treating erectile dysfunction (ED). Over the past two decades, the development of other synthetic PDE5 inhibitors has been accompanied by the increasing popularity of natural remedies from various chemical classes. In this study, in silico tools were used to evaluate the efficacy of 30 natural compounds claimed to be effective against ED, focusing on pharmacokinetic properties, binding energies for PDE5, and comparison with sildenafil's interaction pattern with PDE.

Summary: This paper reviewed 97 medicinal plants with PDE5 inhibitory effect, of which 77 were selected based on their traditional use as aphrodisiacs. Isoflavones and biflavones were identified as the major active constituents responsible for action, while Icariin and Icariside II were found to be the most effective inhibitors from natural sources. Traditional medicines containing extracts of Ginkgo biloba, Kaempferia parviflora, Clerodendrum colebrookianum, Eurycoma longifolia, and Vitis vinifera were also found to inhibit the PDE5 enzyme.

Summary: A two-sample Mendelian randomization (MR) study revealed a positive causal link between hypertension and the risk of erectile dysfunction. Various MR methods consistently showed that hypertension is associated with an increased risk of erectile dysfunction. Therefore, more attention should be given to prevent or improve erectile function in the management of hypertension.

Summary: This study evaluated the prevalence of diabetes mellitus (DM) among patients with erectile dysfunction (ED) and the impact of glycometabolic compensation and antihyperglycemic treatment on ED severity. The study found that DM patients had higher severity of ED and lower response to PDE5-i treatment. A better glycometabolic profile, as well as new antihyperglycemic drugs, seemed to have a positive effect on improving ED.

Summary: This real-world study found that co-possession of nitrate and PDE5i prescriptions is not associated with an increased risk of cardiovascular outcomes. Most cardiovascular outcome rates were not significantly different between co-possession and ED + PDE5i periods, although some outcomes were higher with co-possession. Many patients discussed the risks of co-possession with their physicians and received warnings or instructions.

Summary: The present study aimed to develop and validate the Sexual Desire and Erotic Fantasies questionnaire-Part 2 (SDEF2), focusing on the use, experiences, attitudes, and sharing of sexual fantasies. The SDEF2 showed good psychometric properties, internal reliability, construct, and discriminant validity, being able to differentiate between sexually clinical and functional individuals. This study validates the usefulness of the SDEF2 in assessing different aspects related to fantasizing activity, which is associated with sexual functioning and satisfaction.
