Cluster Analysis of Autoantibodies in 852 Patients with Systemic Lupus Erythematosus from a Single Center

Objective. Associations between autoantibodies and clinical features have been described in systemic lupus erythematosus (SLE). Herein, we aimed to define autoantibody clusters and their clinical correlations in a large cohort of patients with SLE. Methods. We analyzed 852 patients with,SLE who attended our clinic. Seven autoantibodies were selected for cluster analysis: anti-DNA, anti-Sm, anti-RNP, anticardiolipin (AT) immunoglobulin (Ig)G or IgNt lupus anticoagulant (LAC), anti-Ro, and anti-La. Two-step clustering and Kaplan-Meier survival analyses were used. Results. Five clusters were identified. A cluster consisted of patients with only anti-dsDNA antibodies, a cluster of anti-Sm and anti-RNP, a cluster of aCL IgG/M and LAC, and a cluster of anti-Ro and anti-La antibodies. Analysis revealed 1 more cluster that consisted of patients who did not belong to any of the clusters formed by antibodies chosen for cluster analysis. Sm/RNP cluster had significantly higher incidence of pulmonary hypertension and Raynaud phenomenon. DsDNA cluster had the highest incidence of renal involvement. In the aCL/LAC cluster, there were significantly more patients with neuropsychiatric involvement, antiphospholipid syndrome, autoimmune hemolytic anemia, and thrombocytopenia. According to the Systemic Lupus International Collaborating Clinics damage index, the highest frequency of damage was in the aCL/LAC cluster. Comparison of 10 and 20 years survival showed reduced survival in the AcL/LAC cluster. Conclusion. This study supports the existence of autoantibody clusters with distinct clinical features in SLE and shows that forming clinical subsets according to autoantibody clusters may he useful in predicting the outcome of the disease. Autoantibody clusters in SLE may exhibit differences according to the clinical setting or population.

Description of damage in different clusters of patients with antiphospholipid syndrome

Summary: This study aimed to identify different clinical phenotypes of antiphospholipid syndrome (APS) using cluster analysis and describe the cumulative damage of these disease clusters. The results showed that SLE+APS patients with extra-criteria manifestations and older APS patients with arterial thrombosis and increased cardiovascular risk had higher cumulative damage. Effective treatment of SLE disease activity and control of cardiovascular risk may help reduce cumulative damage in these patients.

The relationship between serum A proliferation-inducing ligand and B-cell activating factor levels with disease activity and organ involvement in systemic lupus erythematosus

Summary: This study aimed to investigate the association between serum B-cell activating factor (BAFF) and A proliferation-inducing ligand (APRIL) levels with disease activity and clinical findings in SLE patients. The results showed that both BAFF and APRIL levels were higher in patients with SLE compared to the control group, but there was no correlation between BAFF/APRIL levels and disease activity scores. Serum BAFF levels were higher in patients with renal disease activity, while APRIL levels were significantly correlated with proteinuria. The study suggests that BAFF can be a valuable biomarker in SLE, especially in patients with renal activity.

Summary: This case presents a patient with Takayasu arteritis-induced malignant hypertension, resulting in bilateral renal artery stenosis, acute kidney injury, and advanced stage hypertensive retinopathy. Thrombotic microangiopathy is also detected, which is uncommon in Takayasu arteritis cases with malignant hypertension.

Performances of different classification criteria for systemic lupus erythematosus in a single-center cohort from Turkey

Summary: This study compared the performances of three SLE classification criteria sets in a large cohort of patients and relevant diseased controls. The results showed that EULAR/ACR criteria were more sensitive than the 1997 criteria and had a comparable performance with SLICC criteria. The presence of false positive results in ANA positive patients decreased the specificity of both SLICC and EULAR/ACR criteria.

Impact of COVID-19 pandemic on the course of refractory chronic spontaneous urticaria under omalizumab treatment

Summary: The impact of the COVID-19 pandemic on the course of refractory chronic spontaneous urticaria (CSU) was investigated in this study. It was found that disease activity and levels of anxiety and depression were higher during the quarantine period. Some patients discontinued or prolonged the interval of omalizumab treatment. Therefore, it is recommended for patients to continue their omalizumab treatment and consider prolonging the interval based on disease control.

Summary: The long-term safety and efficacy of a novel red meat oral immunotherapy (OIT) for α-gal allergy in adults were established. All patients who underwent OIT became tolerant to red meat. In addition, alpha-gal specific Immunoglobulin-E (sIgE) may serve as a potential biomarker for monitoring OIT.

Characteristics and course of patients with AA amyloidosis: single centre experience with 174 patients from Turkey

Summary: This study evaluated the clinical, laboratory, and genetic characteristics of patients with AA amyloidosis in Turkey. It found that FMF is the most common cause of AA amyloidosis and patients with homozygous or compound heterozygous MEFV exon 10 variants tend to develop the disease earlier. Patients with a low estimated glomerular filtration rate at admission had a higher risk of progressing to end-stage renal disease. The study also found that the mortality rate increased with the amyloid burden.

Topical cyclosporine versus allergen specific immunotherapy in perennial allergic conjunctivitis

Summary: This study aimed to assess the clinical efficacy of topical cyclosporine A and subcutaneous allergen immunotherapy (SCIT) in treating perennial allergic conjunctivitis. The results showed that both treatments significantly improved symptoms in patients. The study suggests that allergen immunotherapy and cyclosporin A treatment may be considered as effective options for perennial allergic conjunctivitis, especially in cases refractory to other treatments and with positive skin prick tests to specific allergens.
