Development of Criteria to Distinguish Inflammatory from Noninflammatory Arthritis, Enthesitis, Dactylitis, and Spondylitis: A Report from the GRAPPA 2013 Annual Meeting

Objective. To describe a research project to develop simple clinical criteria to aid in the identification of inflammatory arthritis, enthesitis, dactylitis, and spondylitis and distinguish these from noninflammatory conditions. The criteria are particularly intended to aid non-rheumatologists, e.g., dermatologists, who need assistance identifying psoriatic arthritis in patients with psoriasis, but may be useful to all clinicians in properly diagnosing rheumatologic conditions. Methods. The proposed research methodology includes the use of a nominal group exercise among expert clinicians and patient focus groups, Delphi exercises among clinicians and patients, application of criteria test sets to a small group of representative patients with inflammatory and noninflammatory musculoskeletal conditions, and validation by application of optimal criteria sets to large groups of patients with inflammatory and noninflammatory conditions. Results. Examples of elements to describe inflammatory conditions derived from a nominal group exercise conducted at the 2013 GRAPPA annual meeting are described, along with planned project activities. Conclusion. This project will lead to the development of practical criteria to aid in the diagnosis and appropriate clinical care of patients with chronic inflammatory musculoskeletal conditions.

This study evaluated the care and treatment trends of HS patients in the USA using real-world data. Results showed increased prevalence of HS, improved care, but gaps in access to dermatologic care and low utilization of biologic therapies.

Summary: Patients with hidradenitis suppurativa are greatly affected in their lives. Factors such as younger age, Black race, high BMI, active smoking, flares, depression, anxiety, high comorbidity burden, disability, and difficult access to a dermatologist have a negative impact on their quality of life. Attention should be given to these factors, especially those that can be modified, to reduce the overall impact of the disease.

Summary: This study evaluated the sociodemographic characteristics, comorbidities, treatment patterns, healthcare resource utilization, and associated costs of patients with Hidradenitis suppurativa (HS) following the approval of a biologic (adalimumab). The study found that the burden of comorbidities in HS patients continues to increase after diagnosis, and healthcare resource utilization and costs are high. These findings highlight the need for a multidisciplinary comprehensive care strategy for HS patients.

Quality of life and treatment adherence in patients with vulvar lichen sclerosus

Summary: This study describes the demographics, clinical characteristics and skin-related quality of life in patients with vulvar lichen sclerosus (VLS), and assesses the relationship between quality of life and treatment adherence. Treatment adherence was positively correlated with quality of life, and the most common factor preventing adherence was application/treatment time. These findings can help healthcare providers facilitate better treatment adherence and improve the quality of life for patients with VLS.
