Summary: Imaging plays a central role in the diagnosis of axial spondylarthritis (axSpA), with MRI considered the reference standard for early detection of inflammatory changes. Recognizing vertebral involvement patterns can help establish a diagnosis early and initiate therapy before irreversible changes occur. This review aims to present an overview of imaging findings in axSpA with vertebral involvement and detail the current recommendations for diagnosis in patients with isolated vertebral involvement.

Summary: A high prevalence of sacroiliitis on MRI was observed in patients with early pSpA, with significant decrease in SPARCC scores on TNF inhibition during clinical remission. Residual inflammation on MRI was not predictive of relapse of peripheral manifestations, and no relevant inflammatory spinal involvement was detected. Our findings suggest a higher inflammatory burden in early pSpA patients than expected.

Summary: This study found that patients with ankylosing spondylitis (AS) have a higher mortality rate compared to the general population, and healthy individuals carrying the HLA-B27 gene also have a similar risk. Additionally, female patients have a lower life expectancy. However, in the overall European population, the presence of the HLA-B27 gene does not affect survival or the risk of death from cardiovascular disease.

Summary: ERAP1, ERAP2, and LNPEP, which are aminopeptidases, play a role in autoimmune diseases. The expression of ERAP2 is increased while ERAP1 is decreased in psoriasis patients with autoimmune-linked variants of ERAP. Different layers of skin cells show different expression patterns of aminopeptidases, and in psoriasis, basal keratinocytes also express CCL2 and IL-15. In addition, ERAP2-high skin fibroblasts also express more CXCL14.

Summary: The objective of this study was to assess the association between carotid intima-media thickness (CIMT) and the presence of atheromatous plaque with different rheumatoid arthritis (RA) treatments. The results showed that patients on tumor necrosis factor alpha (TNF alpha) inhibitors had smaller CIMTs, suggesting a potential protective effect against subclinical atherosclerosis. However, these findings should be confirmed in larger prospective studies.

Summary: This study assesses the immunogenicity of a fourth dose of mRNA SARS-CoV-2 vaccine in autoimmune rheumatic disease patients who had a poor or non-response to the third dose of an inactivated vaccine. The results show that most patients had a significant increase in antibody levels after the fourth dose, but a portion of patients still did not have an adequate antibody response. The use of specific drugs, such as rituximab and prednisone, may impair the antibody response.

Summary: The study found that both spondyloarthritis and chronic low back pain have similar impacts on health-related quality of life (HRQoL), including mental, physical, and social domains. Patients with spondyloarthritis and chronic low back pain reported significantly lower HRQoL compared to those without rheumatic diseases. Modifiable factors such as multimorbidity, active disease, obesity, and regular physical exercise were associated with HRQoL in these conditions.
