Differential Features Between Primary Ankylosing Spondylitis and Spondylitis Associated with Psoriasis and Inflammatory Bowel Disease

Objective. To describe differential characteristics of axial involvement in ankylosing spondylitis (AS) as compared with that seen in psoriatic arthritis (PsA) and inflammatory bowel disease (IBD) in a cohort of Ibero-American patients. Methods. This study included 2044 consecutive patients with spondyloarthritis (SpA; ESSG criteria). Demographic, clinical, disease activity, functional ability, quality of life, work status, radiologic, and therapeutic data were evaluated and collected by RESPONDIA members from different Ibero-American countries between June and December 2006. Patients selected for analysis met modified New York criteria (mNY) for AS. Results. A total of 1264 patients met the New York criteria for AS: 1072 had primary AS, 147 had psoriatic, and 45 had IBD-associated spondylitis. Median disease duration was comparable among the 3 patient groups. Patients with primary AS were significantly younger (p = 0.01) and presented a higher frequency of males (p = 0.01) than the other 2 groups. Axial manifestations such as inflammatory back pain and sacroiliac pain were significantly more frequent in patients with primary AS (p = 0.05) versus other groups, whereas frequency of dactylitis, enthesitis, and peripheral arthritis was more common in patients with psoriatic spondylitis (p = 0.05). Spinal mobility was significantly more limited in patients with primary AS versus the other 2 groups (p = 0.0001). Radio logic changes according to BASRI total score were equally significant in primary AS. Disease activity (BASDAI), functional ability (BASFI), and quality of life (ASQoL) scores were comparable in the 3 groups. Conclusion. Patients with primary AS had more severe axial involvement than those with spondylitis associated with psoriasis or IBD. Summary: This study assessed the real-life effectiveness of secukinumab in psoriatic arthritis patients. The results showed that the drug had good retention, remission, and low disease activity rates after 6 and 12 months of treatment. The effectiveness was significantly better for patients who had not used biologic/targeted synthetic disease-modifying antirheumatic drugs (b/tsDMARDs) before.

Predictors of progression to systemic sclerosis: analysis of very early diagnosis of systemic sclerosis in a large single-centre cohort

Summary: This study analyzed the early characteristics of SSc in RP patients and identified predictors for the progression to SSc. Combinations of RP + PF + positive ANA + SD-NFC and/or SSc-specific antibody (VEDOSS level 2), as well as RP + PF + positive ANA (VEDOSS level 1; "red flags") were most associated with progression to SSc. Combinations without non-RP clinical symptoms, such as RP + SD-NFC and RP + anticentromere + SD-NFC, were associated with non-progression to SSc.

Association between Carotid Intima-Media Thickness and the Use of Biological or Small Molecule Therapies in Patients with Rheumatoid Arthritis

Summary: The objective of this study was to assess the association between carotid intima-media thickness (CIMT) and the presence of atheromatous plaque with different rheumatoid arthritis (RA) treatments. The results showed that patients on tumor necrosis factor alpha (TNF alpha) inhibitors had smaller CIMTs, suggesting a potential protective effect against subclinical atherosclerosis. However, these findings should be confirmed in larger prospective studies.

Inflammatory myopathies overlapping with systemic sclerosis: a systematic review

Summary: This study conducted a systematic review on the clinical manifestations and complementary exams of patients with myopathies and systemic sclerosis overlap syndrome. The findings suggest that this syndrome is more common in women and commonly affects the esophagus and joints. It is also associated with symmetrical and bilateral muscle involvement, Raynaud's phenomenon, and impaired forced vital capacity.

Summary: This study assesses the immunogenicity of a fourth dose of mRNA SARS-CoV-2 vaccine in autoimmune rheumatic disease patients who had a poor or non-response to the third dose of an inactivated vaccine. The results show that most patients had a significant increase in antibody levels after the fourth dose, but a portion of patients still did not have an adequate antibody response. The use of specific drugs, such as rituximab and prednisone, may impair the antibody response.

Severity and mortality of COVID-19 in patients with systemic sclerosis: a Brazilian multicenter study

Health-related quality of life among spondyloarthritis and chronic low back pain patients: results from a nationwide population-based survey

Summary: The study found that both spondyloarthritis and chronic low back pain have similar impacts on health-related quality of life (HRQoL), including mental, physical, and social domains. Patients with spondyloarthritis and chronic low back pain reported significantly lower HRQoL compared to those without rheumatic diseases. Modifiable factors such as multimorbidity, active disease, obesity, and regular physical exercise were associated with HRQoL in these conditions.

Symptomatic fractures in systemic sclerosis: A case-control study

Critical digital ischaemia in systemic sclerosis exacerbated by multiple myeloma: A case report

Summary: This case report presents a patient with systemic sclerosis and multiple myeloma who experienced severe digital ischaemia that led to amputation of several fingers. The association between systemic sclerosis and multiple myeloma is rare, but should be considered in cases of significant worsening of Raynaud's phenomenon.

Similarities and differences between non-radiographic and radiographic axial spondyloarthritis: The patient perspective from the Spanish atlas
