Summary: There is a correlation between the levels of noncoding RNAs (miRNA-23a, miRNA-99a, and lncRNA H19) and the outcomes of acute ischemic stroke (AIS) patients before and after treatment with IV recombinant tissue plasminogen activator (rtPA). A noncoding RNA score composed of miRNA-23a, miRNA-99a, and lncRNA H19 can predict the functional outcome and the occurrence of symptomatic intracerebral hemorrhage (sICH) after rtPA treatment in AIS patients. These findings suggest that miRNA-23a, miRNA-99a, and lncRNA H19 may have independent predictive value in thrombolysis after AIS.

Summary: Recombinant human tissue plasminogen activator (rh-tPA) and Microlyse are both important thrombolytic agents for the treatment of acute ischemic stroke, but they have different mechanisms. rh-tPA activates plasminogen by binding to fibrin, while Microlyse activates plasminogen by binding to von Willebrand factor (VWF). The study results show that both thrombolytic agents can increase cortical reperfusion and reduce cerebral lesion volume in different types of thrombus models. However, only Microlyse can reduce cerebral lesion volume in the platelet-rich model.

Summary: In this study, the relationship between neutrophil-to-lymphocyte ratio (NLR) and lymphocyte-to-monocyte ratio (LMR) with in-hospital clinical outcomes in ischemic stroke patients treated with intravenous tissue plasminogen activator (IV tPA) was investigated. The results showed that NLR at 24 hours was associated with early neurological improvement (ENI), while NLR at 48 hours and LMR at 48 hours were closely related to favorable functional outcomes at discharge.

Summary: Twenty-five years ago, intravenous thrombolysis has transformed the care for patients with acute ischemic stroke, and recent clinical trials have shown the benefits of mechanical thrombectomy. However, new strategies targeting tPA-resistant thrombi, such as tenecteplase, and experimental models targeting DNA and von Willebrand factor show promising results for improving stroke outcomes. Further research and trials are needed to enhance the effectiveness of thrombolysis in ischemic stroke patients.
