Summary: Rheumatoid arthritis (RA) is an autoimmune disease that causes pain and disability. Current drugs for treating RA aim to halt the disease progression and reduce inflammation, but they have drawbacks. To address this, targeting transcription factors (TFs) has become a focus, as they play a vital role in RA pathogenesis. Synthetic drugs and natural compounds are being explored to target key TFs and their signaling pathways in RA. This review discusses the potential of targeting four key TFs, namely NF-?B, STATs, AP-1, and IRFs, for treating RA.

Summary: Although the exact cause of rheumatoid arthritis (RA) is still unknown, there is evidence to suggest a strong genetic predisposition and the presence of preclinical antibodies before symptoms manifest. This study investigates the impact of an expansion of Eggerthella lenta on the severity of RA using a humanized mouse model. The results show that mice colonized with E. lenta develop more severe disease when induced for arthritis, particularly in female mice. Furthermore, among RA patients, women tend to have a higher abundance of E. lenta. The expansion of E. lenta is associated with changes in immune responses, gut dysbiosis, and systemic inflammation.

Summary: Inflammasome is a cellular complex involved in the innate inflammatory response, and its excessive or abnormal activation has been implicated in various diseases. In rheumatoid arthritis, inflammasome activation plays a key role in immune dysregulation and joint inflammation. This review summarizes recent research on inflammasome activation and its effector mechanisms in the pathogenesis of rheumatoid arthritis, as well as potential therapeutic strategies.

Summary: This study explores the correlation between cell-free DNA (cfDNA) levels and rheumatoid arthritis (RA) disease activity, and demonstrates the important role of cfDNA in disease development. A novel therapeutic approach utilizing anti-inflammatory (M2) macrophage-derived exosomes, modified with oligolysine and matrix metalloproteinase (MMP)-cleavable polyethylene glycol (PEG), provides substantial suppression of inflammation, chondroprotection, and osteoprotection in CIA models.

Summary: Pulmonary disease is a common manifestation of RA, associated with increased morbidity and mortality. This study found a high prevalence of undiagnosed parenchymal lung disease in RA patients, primarily manifested as preclinical emphysema. High resolution CT scans may be the most effective screening modality for detecting lung abnormalities in RA patients.

Summary: This study describes a combination strategy using phytosomes co-loaded with curcumin and leflunomide to improve the clinical outcomes of rheumatoid arthritis therapy. The CUR/LEF-loaded phytosomes demonstrated high stability, sustained release profile, and significant reduction in inflammation and paw swelling in the arthritic rats.

Summary: A new humanized anti-IL-6 mAb HZ-0408b with potent binding and neutralizing activity to human IL-6 has been developed. It showed a high species specificity and lower cross-reactivity compared to other anti-IL-6 mAbs. HZ-0408b exhibited a stronger inhibitory effect on IL-6 signaling and effectively reduced joint swelling and plasma CRP levels in a monkey CIA model.

Summary: Traditional herbal medicines (THMs) have potential pharmacological mechanisms in the treatment of rheumatoid arthritis (RA), with main components including flavonoids, phenolic acids, alkaloids, and triterpenes. THMs can alleviate symptoms of RA through regulating multiple therapeutic targets such as immunity, inflammation, microRNAs, etc. Further research is needed to uncover the complexities and detailed mechanisms of THMs in alleviating RA.
