Two predictive models for IVIG resistance with excellent performance were established.

Summary: This study aimed to determine the risk factors for resistance to intravenous immunoglobulin (IVIG) treatment and coronary artery aneurysm (CAA) development in Chinese pediatric patients with high-risk Kawasaki disease. The Xie Liping scoring system was found to be the most appropriate method for identifying high-risk patients. IVIG resistance could be predicted based on the bilirubin-to-albumin ratio. Serum albumin level and Z score of the left main coronary artery were independent predictors of CAA development.

Summary: This study aimed to investigate the predictive value of monocyte to high-density lipoprotein cholesterol ratio (MHR) for coronary artery lesions (CALs) and intravenous immunoglobulin (IVIG) resistance in complete Kawasaki disease (KD). The MHR values of 207 complete KD patients were analyzed along with their clinical characteristics. The study found that MHR was significantly associated with CALs, but not with IVIG resistance. The results suggest that MHR can serve as a practical inflammatory biomarker for predicting CALs and IVIG resistance in complete KD.

Summary: This study assessed the risk factors of IVIG-resistant Kawasaki disease and evaluated the performance of three Japanese risk-scoring systems in predicting IVIG resistance among Indian patients. The results showed that skin rash, high total bilirubin, high CRP, high AST, high ALT, and low albumin were important predictors of IVIG resistance. Among the three scores, Sano score was the most reliable in identifying potential non-responders to IVIG, although it lacked good specificity. Therefore, an exclusive scoring system may be needed for Indian KD patients to predict non-responsiveness to IVIG.

Summary: Dosing guidance for children with obesity is often unknown and it remains unclear whether children with and without obesity experience equivalent enoxaparin exposure. A physiologically-based pharmacokinetic model was developed to explore the differences in anti-Xa concentration between children with and without obesity. The model showed that weight-normalized clearance was reduced in children with obesity, leading to higher anti-Xa concentrations under weight-based dosing. Fat-free mass weight-based dosing improved enoxaparin exposure across different age groups and obesity status.

Due to the lack of pharmacokinetic studies, there is limited information to guide the dosing of opioids in obese children. In order to fill this knowledge gap, researchers developed physiologically-based pharmacokinetic models to explore the interaction between obesity, age, and pharmacogenomics in children. The models accurately predicted drug concentrations and dosing requirements in obese children, highlighting the importance of considering both age and obesity status when prescribing medications.

Summary: We evaluated the transportability of a mortality prediction algorithm using a multi-national network of healthcare systems. The algorithm utilized baseline laboratory values, demographic and clinical covariates to predict COVID-19 mortality across different healthcare systems and countries. The results showed that the model had consistent performance and reliable transportability across healthcare systems in the US and Europe, highlighting its generalizability.

Summary: This retrospective observational study analyzed data from 12,891 COVID-19 patients and found that age, severe COVID-19, severe acute kidney injury (AKI), and ischemic heart disease were associated with worse mortality outcomes. The severity of AKI was associated with poorer kidney function recovery, while the use of remdesivir was associated with better recovery. In patients without chronic kidney disease, age, male sex, severe AKI, and hypertension were associated with post-AKI kidney function impairment. COVID-19-associated AKI was also linked to higher mortality and worse long-term kidney function recovery.

Summary: This prospective cohort study aimed to investigate the impact of bronchopulmonary dysplasia (BPD) on the health-related quality of life (HRQL) of infants from NICU hospitalization to one year post-discharge. The study found that lower HRQL during NICU stay was associated with earlier gestational age, postnatal corticosteroid usage, outborn status, and gastrostomy tube placement. Lower HRQL at 3 and 12 months post-discharge was associated with readmissions, home oxygen use, parent-reported difficulty breathing, lower developmental scores, and not playing with other children. Most parents reported similar or improved HRQL after discharge, but parents of infants with respiratory symptoms experienced less improvement. Efforts to improve parent HRQL should focus on respiratory symptoms and social isolation.

Summary: This study aims to determine if children with elbow flexion contracture (EFC) caused by brachial plexus birth injury (BPBI) are more likely to develop shoulder contracture and undergo surgical treatment. A retrospective review was conducted on children under 2 years old with BPBI who presented to a single children's hospital. The results showed that patients with EFC had reduced shoulder range of motion and higher odds of shoulder contracture and surgical treatment. Prompt referral to a BPBI specialty clinic is recommended for evaluation and potential surgery.
