Summary: Patients with severe aplastic anemia (SAA) are treated with bone marrow transplant (BMT) or immunosuppression (IST) depending on individual factors. The addition of eltrombopag (EPAG) to standard IST for SAA improves hematologic responses. However, the rates and characteristics of long-term complications, relapse, and clonal evolution with this new regimen are not yet known. This study reports a cumulative relapse rate of 39% in responding patients and a clonal evolution rate of 15% in all treated patients at 4 years. Relapse occurred at distinct timepoints and most relapsed patients responded to retreatment. Clonal evolution to high-risk abnormalities was noted in 5.7% of patients and was associated with poorer overall survival.

Summary: This study retrospectively analyzed the clinical and laboratory characteristics of 124 patients with transfusion-dependent non-severe aplastic anemia (TD-NSAA) and found common complications including iron overload, liver and kidney dysfunction, diabetes mellitus/impaired glucose tolerance, and severe infection. 58% of patients progressed to severe aplastic anemia, and low absolute neutrophil count, severe infection, or iron overload were identified as risk factors for progression. Combination therapy with antithymocyte globulin (ATG) and Cyclosporin A (CsA) showed a higher overall response rate compared to CsA alone, and ATG was a favorable factor for efficacy. Some patients developed secondary clonal hematologic diseases and had a low survival rate.

Summary: Severe aplastic anemia (SAA) is a marrow failure disorder with high morbidity and mortality. This study found that reduced-intensity conditioning HLA-haploidentical bone marrow transplantation (BMT) with posttransplantation cyclophosphamide (PTCy)-based graft-versus-host (GVHD) prophylaxis resulted in 100% overall survival and minimal GVHD in patients with SAA. This approach not only avoids the adverse effects of immunosuppressive therapy, but also expands access to BMT across all populations.

Summary: This study compared the efficacy and safety of eltrombopag combined with immunosuppressive therapy and immunosuppressive therapy alone in treatment-naive children with severe and very severe aplastic anemia. The results showed that adding eltrombopag to standard immunosuppressive therapy increased the complete response rate and overall response rate compared to immunosuppressive therapy alone. The greatest benefit from eltrombopag combined with immunosuppressive therapy was observed in patients with severe aplastic anemia, but not in those with very severe aplastic anemia.

Summary: The platelet/lymphocyte ratio and neutrophil/lymphocyte ratio are important factors in predicting the outcomes of severe aplastic anemia patients treated with antithymocyte globulin. A low platelet/lymphocyte ratio is positively associated with better survival rates, while a low neutrophil/lymphocyte ratio may indicate shorter survival time. Additionally, the ratio of CD4/CD8 is significantly higher in the low platelet/lymphocyte ratio group.

The results showed that patients with mild NSAA had the lowest response rate to CsA, while transfusion-dependent patients had a positive response. Patients with lower platelet count and higher interleukin 10 level were more likely to have a positive response to CsA, while a higher level of fetal hemoglobin was a negative factor.

Summary: This trial compared the efficacy of eltrombopag (EPAG)+tacrolimus versus EPAG monotherapy in patients with refractory/relapsed acquired aplastic anaemia (AA). The overall response rate was higher in the EPAG+tacrolimus group compared to the EPAG alone group, but there were no significant differences in safety, clonal evolution, and survival between the two groups. A subgroup analysis suggested that EPAG+tacrolimus may have better efficacy in patients under 60 years old.

Summary: The advances in the management of aplastic anaemia (AA) have been successful, with better understanding of the mechanisms underlying the reduction of hematopoietic stem cells (HSCs) and improvements in supportive care. Efforts to increase immunosuppression potency and stimulate HSCs have not achieved better outcomes, but the use of thrombopoietin receptor agonists (Tpo-RA) has been effective in stimulating early HSCs despite high endogenous Tpo levels.

The study found that lower HRQL during NICU stay was associated with earlier gestational age, postnatal corticosteroid usage, outborn status, and gastrostomy tube placement. Lower HRQL at 3 and 12 months post-discharge was associated with readmissions, home oxygen use, parent-reported difficulty breathing, lower developmental scores, and not playing with other children. Most parents reported similar or improved HRQL after discharge, but parents of infants with respiratory symptoms experienced less improvement. Efforts to improve parent HRQL should focus on respiratory symptoms and social isolation.

Summary: This study aims to determine if children with elbow flexion contracture (EFC) caused by brachial plexus birth injury (BPBI) are more likely to develop shoulder contracture and undergo surgical treatment. A retrospective review was conducted on children under 2 years old with BPBI who presented to a single children's hospital. The results showed that patients with EFC had reduced shoulder range of motion and higher odds of shoulder contracture and surgical treatment. Prompt referral to a BPBI specialty clinic is recommended for evaluation and potential surgery.
