Health-related quality of life in children and adolescents after open-heart surgery

Objective To assess health-related quality of life (HRQOL) in children with congenital heart disease (CHD) after open-heart surgery and to identify medical, individual, and family-related predictors of outcome. Study design. Cross-sectional cohort study. 155 children with CHD (40% cyanotic type) who underwent open-heart surgery between 1995 and 1998 were eligible. One hundred and ten patients (response rate 71%) were investigated at a mean age of. 10.4 years. Mean age at operation was 2.3 years (range, 0-8.7 years), mean duration of cardiopulmonary bypass was 95 minutes (range, 5-206 minutes), and circulatory arrest was performed in 9% of the children. Child- and parent-rated HRQOL was evaluated by the TNO-AZL Child Quality of Life Questionnaire. Scores were compared with healthy references. Results Most dimensions of self-reported HRQOL were impaired, including autonomy and motor, social, and emotional functioning. In addition to the children's ratings, parents rated cognitive functioning of their children as diminished. Multivariate analyses revealed that duration of cardiopulmonary bypass, length of hospitalization, need for current cardiac medication, and adverse family relationships had a negative impact on parent- but not on child-rated HRQOL. Conclusions HRQOL is impaired in children with CHD after open-heart surgery. Medical variables and the quality of family relationships are important determinants.

Key patient-reported outcomes in children and adolescents with intoxication-type inborn errors of metabolism: an international Delphi-based consensus

Summary: This study aimed to define patient-reported and observer-reported outcomes (PROs/ObsROs) relevant to children with inborn errors of metabolism (IT-IEM), their families, and healthcare professionals, and to provide valid and standardized measuring instruments. Through a two-round Delphi survey and an online consensus meeting, a final set of 17 PROs/ObsROs were determined, covering social, emotional, and disease-related aspects of patients' lives as well as the experience of parents. These PROs/ObsROs should be incorporated into daily IT-IEM care and considered as key psychological outcomes in clinical trials.

Sexualized Behavior Among Adolescents Who Sexually Offended

Summary: This study retrospectively examines the prevalence and patterns of sexualized behaviors (SB) among juvenile sexual offenders (JSO) before their sexual offenses. Three SB profiles were identified: low/no SB, preoccupied SB, and dysregulated SB. The preoccupied and dysregulated SB groups had higher rates of psychiatric disorders, but none of them reoffended sexually within one year after conviction. Overall, the study suggests that SB is not a reliable indicator of high risk for persistent sexual offending among JSO.

Mental health-related quality of life in mothers of children with surgically repaired congenital heart disease: a 13-year longitudinal study

Pediatric Trauma and Trauma Team Activation in a Swiss Pediatric Emergency Department: An Observational Cohort Study

Health-Related Quality of Life for Parents of Infants with Bronchopulmonary Dysplasia

Summary: This prospective cohort study aimed to investigate the impact of bronchopulmonary dysplasia (BPD) on the health-related quality of life (HRQL) of infants from NICU hospitalization to one year post-discharge. The study found that lower HRQL during NICU stay was associated with earlier gestational age, postnatal corticosteroid usage, outborn status, and gastrostomy tube placement. Lower HRQL at 3 and 12 months post-discharge was associated with readmissions, home oxygen use, parent-reported difficulty breathing, lower developmental scores, and not playing with other children. Most parents reported similar or improved HRQL after discharge, but parents of infants with respiratory symptoms experienced less improvement. Efforts to improve parent HRQL should focus on respiratory symptoms and social isolation.

Reference Values for Cardiorespiratory Fitness in Patients Aged 6 to 18 Years

Early Elbow Flexion Contracture Predicts Shoulder Contracture in Infants with Brachial Plexus Birth Injury

Summary: This study aims to determine if children with elbow flexion contracture (EFC) caused by brachial plexus birth injury (BPBI) are more likely to develop shoulder contracture and undergo surgical treatment. A retrospective review was conducted on children under 2 years old with BPBI who presented to a single children's hospital. The results showed that patients with EFC had reduced shoulder range of motion and higher odds of shoulder contracture and surgical treatment. Prompt referral to a BPBI specialty clinic is recommended for evaluation and potential surgery.
