Peripheral and Intestinal CD4+ T Cells With a Regulatory Phenotype in Pediatric Patients With Inflammatory Bowel Disease

Objectives: Regulatory T cells (T(R) cells) play a crucial role in the regulation of intestinal inflammation. To examine the pathogenetic relevance of T(R) cells in inflammatory bowel disease (IBD), we evaluated their frequency in peripheral blood and inflamed and noninflamed mucosae of pediatric patients with IBD and age-matched controls without IBD; we also characterized the immune profile of the inflammatory infiltrate in the different phases of the disease. Patients and Methods: Circulating T(R) cells were investigated on peripheral blood mononuclear cells by fluorescence-activated cell sorting analysis; mucosal T(R) cells and inflammatory cell populations were investigated by immunohistochemistry on bioptic specimens. FOXP3 messenger RNA expression levels were confirmed using real-time polymerase chain reaction. Results: FOXP3+ T(R) cells were significantly increased in the intestinal lesions of patients with active IBD, and returned to normal levels in posttherapy remission phase. At variance, circulating T(R) cell frequency was elevated in patients with IBD independently of disease activity, as it persisted in the remission phase. A selective imbalance in the frequency of CD4+ T and natural killer cell subsets characterized the abundant inflammatory infiltrate of active intestinal lesions, and also persisted, at a lower level, in noninflamed mucosae of patients in the remission phase. Conclusions: T(R) cell frequency is differently regulated in mucosal tissues and at the systemic level during the distinct phases of pediatric IBD. The inactive stage of pediatric IBD is characterized by an incomplete normalization of the immune profile, independently of the clinical efficacy of the therapy. The pediatric, early-onset condition may represent a privileged observatory to dissect the immune-mediated pathogenetic mechanisms at the basis of the disease.

Summary: Adult AT/RT predominantly arises in female patients and in supratentorial location. Midline structures, including the sellar region, are the most affected sites, especially among females aged > 40 years. Male gender is more prevalent between the age of 18 and 40 years and more frequently associated with non-midline tumors. Factors significantly associated with better prognosis are patient's age (< 40 years), combined radio-chemotherapy adjuvant approach and Ki-67 score < 40%.

Summary: The in vivo establishment and long-term persistence of a heterogeneous memory or an adaptive NK cell pool play a functional adaptation role to human cytomegalovirus infection in humans. The FcεRIγ(-) NKG2C(+)CD57(+) memory NK cell subset, whose abundance is associated with anti-HCMV antibody levels, expands robustly and selectively in response to mAb-opsonized targets or ITP-derived platelets, demonstrating superior CD16-dependent IFNγ production. The lack of FcεRIγ chain contributes to the higher responsivity to CD16 engagement in NKG2C(+)CD57(+) memory cells.

Split-dose versus day-before regimen of sodium picosulfate plus magnesium citrate for bowel cleansing before colonoscopy in children: Randomized controlled trial

Summary: This study aimed to assess the superiority of split-dose regimen of SPMC compared with SPMC day-before regimen for bowel cleansing before colonoscopy in children. The results showed that the rate of successful cleansing level was significantly higher in the split-dose group. The BBPS scores were significantly higher in the split-dose group for the whole colon, right colon, and transverse colon. Patient acceptability was better in the split-dose group and the percentage of children needing nasogastric tube placement was lower.

Gastrointestinal Involvement in Children with Systemic Lupus Erythematosus

Summary: Systemic lupus erythematosus (SLE) is a systemic autoimmune disorder. When it presents before the age of 18 years (childhood-onset systemic lupus erythematosus, cSLE), the disease course tends to be more severe with a higher rate of organ involvement and requires an early diagnosis. Gastrointestinal involvement in cSLE is rare and scarcely reported in the literature. This manuscript provides a narrative review of gastrointestinal manifestations in cSLE, focusing on hepatic, pancreatic, and intestinal involvement, based on a comprehensive literature search using the PubMed database.

Summary: The relationship between migraines and allergies is still not well understood, although there is an epidemiological link between them. Genetic and biological factors contribute to both migraines and allergic disorders. The histaminergic system, particularly the H3 and H4 receptors, may play a role in the pathophysiology of both conditions. Antihistamine drugs could potentially be helpful in treating migraines and allergic disorders. Further research is needed to fully understand the mechanistic link between these two common and debilitating conditions.

A Treat-to-Target Strategy Guided by Pan-Enteric Evaluation in Children With Crohn's Disease Improves Outcomes at 2 Years

Summary: It remains uncertain whether a treat-to-target approach could effectively improve outcomes in children with Crohn's disease. Results from a 104-week follow-up study showed that mucosal healing (MH) was maintained in 93% of patients who had MH at 52 weeks. The study also found that complete MH at 52 weeks was associated with reduced need for steroids, treatment escalation, hospitalization, and clinical relapses.

Adult IDH Wild-Type Glioblastoma Ultrastructural Investigation Suggests a Possible Correlation between Morphological Biomarkers and Ki-67 Index

Summary: In this study, the ultrastructure of glioblastoma tissue from 9 patients with the same molecular profile was investigated to find possible ultrastructural features as biomarkers correlated with Ki-67 labeling index (LI). The main findings include anatomical basis of astrocyte-endothelial cells crosstalk, ultrastructural imaging of hyperactivated microglia cells (MsEVs) and microglia cells storing lipid vesicles (MsLVs), and ultrastructural imaging of neoplastic cells mitophagy (NCsM). Statistical analysis showed that MsEVs and MsLVs correlate with Ki-67 LI, indicating their potential as morphological biomarkers. However, further research with larger sample sizes is needed to validate these findings and fully utilize the potential of ultrastructural analysis in managing glioblastoma.
