Summary: This study examined the long-term survival and incidence of secondary fractures after fragility hip fractures. The 5-year survival rate was 62%, with higher mortality risk in patients with GNRI < 92. The 5-year incidence of secondary fractures was 22%, which was significantly higher in patients with a BMI < 20.

Summary: This study examines the direct healthcare costs of subjects with vertebral fractures available for identification on routine CT scans. The results show that patients with vertebral fractures incur significantly higher healthcare costs compared to those without fractures. The study also found that the healthcare costs per day at risk were higher in the short term for patients with vertebral fractures, although this difference was not statistically significant, and the long-term healthcare costs were similar to the general population.

Summary: This study investigated the relationship between preoperative depression and postoperative complications following hip fractures in geriatric patients. The results showed that patients with preoperative depression had significantly increased odds of complications after undergoing open reduction internal fixation (ORIF) and intramedullary nailing (IMN) surgeries.

Summary: This study retrospectively analyzed the treatment of geriatric patients with unstable trochanteric hip fractures, finding that only younger age was a significant predictor of nail failure, while other technical factors did not influence the outcome.

Summary: This study estimated the direct costs of hip, vertebral, and forearm fractures in the first year after fracture incidence in Iran, finding that the direct cost of osteoporotic fractures in Iran is high. The findings may be useful for economic evaluation and estimating the economic burden of osteoporotic fractures in Iran.

Summary: Interdisciplinary orthogeriatric care for older adult hip fracture patients is crucial, and increased involvement of geriatricians has a significant impact on outcomes, including early mobilization, mobility, and initiation of osteoporosis treatment. Standardized treatment pathways and structured data collection in specific registries improve outcome monitoring and interpretation.

Summary: This narrative review summarizes the current concepts for the treatment of ankle fractures in elderly patients. Open reduction and internal fixation is considered as the gold standard for operative care, though individualized treatment decisions based on factors such as soft-tissue status, fracture pattern, mobility, and comorbidities are essential for optimal patient outcomes. Techniques like fibular nails or minimally invasive approaches should be considered to address the high complication rates post-surgery.

Summary: This study aimed to investigate the association between recent fragility fractures and 8-year risks of revision THA, periprosthetic fracture, and secondary fragility fracture. The study found that patients with recent fragility fractures within 3 years prior to THA had significantly increased risks of revision THA, periprosthetic fracture, and secondary fragility fracture.

Summary: This study aimed to evaluate the effect of orthogeriatric co-management (OGC) in geriatric hip fracture patients in China. The results showed that OGC significantly shortened the time to surgery, but had no significant effect on mortality rate within 1 year and functional status at 1 year of follow-up compared to traditional orthopaedic care (TOC).

Summary: Osteoporosis screening and secondary fracture prevention after pelvic fractures were found to be inadequate in the study population. The majority of patients did not undergo dual x-ray absorptiometry (DXA) scans or receive anti-osteoporosis medications. Within 2 years, 41.0% of patients developed additional osteoporotic fractures.

Summary: Despite limited data on healthcare costs associated with hip fracture in Sub-Saharan Africa, this study estimated high direct medical costs for managing hip fracture within the South African public healthcare system. The findings can inform policy decisions regarding budgeting and planning of hip fracture services.

Summary: A standardized, multidisciplinary approach for surgical clearance and optimization in geriatric hip fracture patients has shown significant financial and clinical benefits. Implementation of a protocolized, multidisciplinary team has led to reductions in unnecessary preoperative testing, time to medical readiness for surgery, case cancellation rate, length of stay, and total hospitalization charges, without an increase in complications or mortality.

Summary: Haemophilia A (HA) and haemophilia B (HB) are X-linked bleeding disorders caused by the absence or deficiency of coagulation factors VIII (FVIII) and IX (FIX). Recent advancements in haemophilia treatment have led to increased life expectancy, resulting in a higher incidence of fragility fractures. The pathogenesis of bone loss in people with haemophilia involves multiple factors including joint bleeding, reduced physical activity, nutritional deficiencies, and FVIII/FIX deficiency. The management of fractures in people with haemophilia includes pharmacological treatment, surgery, and rehabilitation.

Summary: Postoperative delirium (POD) is a common complication in geriatric patients with hip fractures, associated with poorer functional recovery, longer hospital stays, and higher mortality rates. Various factors such as advanced age, preoperative cognitive impairment, comorbidities, perioperative polypharmacy, and delayed surgery increase the risk of developing POD. Effective interventions such as perioperative risk prediction, avoidance of certain medications, and orthogeriatric comprehensive care can help prevent POD. Anesthesia technique choice does not seem to significantly impact the incidence of postoperative delirium in geriatric patients with hip fractures. There are limited pharmaceutical measures available for POD treatment, with dexmedetomidine and multimodal analgesia showing potential effectiveness but also considering adverse complications when using antipsychotics. In conclusion, perioperative risk intervention based on orthogeriatric comprehensive care is the most effective strategy for preventing postoperative delirium in geriatric patients with hip fractures.

Summary: Falls are the leading cause of injury in older patients, leading to a large number of emergency room visits and fractures. While orthopaedic surgeons care for these patients, they are seldom involved in fall risk assessment and prevention strategies. Utilizing simple in-office fall assessment tools can improve care quality and reduce fall risk.
