Autologous Bone Graft: Properties and Techniques

Bone grafting is involved in virtually every procedure in reconstructive orthopaedic surgery. Although autologous bone grafts have excellent biologic and mechanical properties, considerable donor site morbidity and the limited volume available must be taken into consideration. Currently, there are no heterologous or synthetic bone substitutes available that have superior biologic or mechanical properties. This review article summarizes the biologic and mechanical properties of autologous bone grafts, differentiates various autologous bone graft types, and compares them with other bone substitutes.

Summary: This multicenter randomized controlled trial aimed to compare the outcomes of locked lateral plating and retrograde nailing as treatment options for distal femur fractures. The functional testing showed no difference between the two groups, but patients in both groups remained significantly affected by their fractures 12 months after injury. Additionally, the plating group had more coronal plane valgus, approaching statistical significance. Overall, both lateral locked plating and retrograde intramedullary nailing are reasonable surgical options for these fractures.

Definition of occult hypoperfusion in trauma: A systematic literature review

Summary: The aim of this study is to evaluate the definitions of occult hypoperfusion (OH) in trauma and propose a comprehensive definition. A review of literature found differences in the definitions of OH in different studies. The proposed comprehensive definition is lactate > 2 mmol/l or base excess < -3 mmol/l, with systolic blood pressure > 90 mmHg and pulse rate < 120 bpm. This definition helps identify trauma patients at risk for OH.

Summary: A new bioengineering strategy using oriented biocompatible 3D PLGA/aCaP nanocomposites has been developed to enhance osseointegration and treat large bone defects. Two types of scaffolds, decellularized or seeded with adipose-derived mesenchymal stromal cells, showed high osseointegration, improved bone quality, increased alignment of collagen fibers, and optimal alignment and size of hydroxyapatite minerals, as determined by micro-CT analysis and SAXS imaging.

Evaluation of Cannulated Compression Headless Screw (CCHS) as an alternative implant in comparison to standard S1-S2 screw fixation of the posterior pelvis ring: a biomechanical study

Summary: This biomechanical study aimed to evaluate the stability of S1-S2 fixation of the sacroiliac (SI) joint using three different screw designs. The study found that using two cannulated compression headless screws or two partially threaded SI screws achieved better interfragmentary stability compared to fully threaded SI screws. Therefore, in the treatment of posterior pelvis ring injuries in young trauma patients, the former can be considered as a valid alternative to standard screw fixation.

Summary: This study aimed to identify non-infection related risk factors for impaired fracture healing of proximal femur fractures in non-geriatric trauma patients. It found that increasing fracture complexity, open reduction and internal fixation (ORIF), and diabetes mellitus type II (DM) were independent risk factors for delayed union.

Pre-hospital and Hospital Trauma Care during the Covid-19 Lockdown - Experience in a Metropolitan European Level 1 Trauma Centre

Summary: A study found that during the lockdown in Switzerland, there was a significant increase in trauma sustained in the household environment compared to the previous year, and a decrease in severely injured patients due to motor vehicle accidents and horizontal decelerations. However, there was an increase in household-related injuries. The study calls for a qualitative analysis of treatment during lockdown to gain further insights into the impact of the pandemic on trauma care.

Simulated full weight bearing following posterior column acetabular fracture fixation: a biomechanical comparability study

Summary: The purpose of this study was to evaluate construct stiffness and failure load following posterior column fractures fixation with standard plate osteosynthesis, screw fixation, or using a screwable cup for total hip arthroplasty under full weight bearing conditions. The results showed that both standard plate osteosynthesis and screwable cup fixation demonstrated encouraging results for post-surgical treatment concept with full weight bearing.

Anterior column acetabulum fracture fixation with a screw-augmented acetabular cup-a biomechanical feasibility study

Summary: This biomechanical study concludes that minimally invasive cup fixation with screw augmentation demonstrates comparable stability to plate osteosynthesis for anterior column fractures in osteoporotic bone. However, the study does not provide a conclusive answer regarding the immediate full weight bearing following cup fixation. Therefore, a more conservative approach is considered more reasonable.

Summary: This study evaluated the biomechanical competence of posterior column acetabulum fracture fixation using either a standard cannulated screw or a cannulated compression headless screw (CCHS) via a posterior approach. The results showed that CCHS fixation had higher initial stiffness and fatigue performance compared to the standard cannulated screw. Therefore, CCHS fixation could be a valuable alternative option for treating posterior column acetabulum fractures.

Is a Washer a Mandatory Component in Young Trauma Patients with S1-S2 Iliosacral Screw Fixation of Posterior Pelvis Ring Injuries? A Biomechanical Study

Summary: This study evaluated the biomechanical competence of S1-S2 sacroiliac (SI) screw fixation with and without using a washer across three different screw designs. The results showed that the number of cycles to failure and the corresponding load at failure were significantly higher for fully threaded short screws with a washer compared to its counterpart without a washer. However, there were no significant differences in these parameters when comparing fixations with and without a washer using either partially threaded short or fully threaded long transsacral screws.

The impact of fasciotomy on inpatient outcomes in lower leg fracture management

Summary: This study aimed to identify predictors of compartment release and risk factors related to complications in patients with lower extremity fractures and compartment syndrome. The results showed that age, sex, duration of surgery, and antibiotic use were associated with the need for fasciotomy. Patients who underwent fasciotomy had higher complication rates and longer hospital stays.
