Toward an International Initiative for Traumatic Brain Injury Research

The European Commission (EC) and the National Institutes of Health (NIH) jointly sponsored a workshop on October 18-20, 2011 in Brussels to discuss the feasibility and benefits of an international collaboration in the field of traumatic brain injury (TBI) research. The workshop brought together scientists, clinicians, patients, and industry representatives from around the globe as well as funding agencies from the EU, Spain, the United States, and Canada. Sessions tackled both the possible goals and governance of a future initiative and the scientific questions that would most benefit from an integrated international effort: how to optimize data collection and sharing; injury classification; outcome measures; clinical study design; and statistical analysis. There was a clear consensus that increased dialogue and coordination of research at an international level would be beneficial for advancing TBI research, treatment, and care. To this end, the EC, the NIH, and the Canadian Institutes of Health Research expressed interest in developing a framework for an international initiative for TBI Research (InTBIR). The workshop participants recommended that InTBIR initially focus on collecting, standardizing, and sharing clinical TBI data for comparative effectiveness research, which will ultimately result in better management and treatments for TBI.

Diagnosis and treatment of Chiari malformation and syringomyelia in adults: international consensus document

Summary: This study aimed to reach a consensus among international experts on controversial issues in the diagnosis and treatment of Chiari 1 malformation and syringomyelia in adults. Through a multidisciplinary panel, a Delphi process, and face-to-face discussions, a consensus was reached on 57 out of 63 statements related to the conditions. The consensus document, consisting of 63 statements, was the result of expert discussion and serves as a guide for clinicians and researchers following adults with Chiari and syringomyelia.

Summary: This study aimed to investigate if European neurotrauma centers can be clustered based on their treatment preference in different domains of traumatic brain injury care in the intensive care unit (ICU). Provider profiles from 66 centers in 20 different countries in Europe and Israel were analyzed, revealing correlations within and between treatment domains, but hospitals could not be grouped based on their treatment preference.

In this study, patients with TBI were assessed for coagulation profiles and those with elevated INR values had higher mortality rates and worse outcomes. Additionally, levels of coagulation factors and thrombin generation varied between patients with different INR values.

A daily temperature rhythm in the human brain predicts survival after brain injury

Summary: Temperature-based treatment after brain injury is controversial due to the undefined 'normal' human brain temperature. A study shows that human brain temperature exceeds body temperature by 2-3 degrees C and varies by age, sex, brain region, and time. Loss of daily brain temperature rhythm predicts death after traumatic brain injury. This study aimed to determine the clinical relevance of brain temperature in patients and analyzed the variations in healthy adults' brain temperature.

A Critical Update of the Classification of Chiari and Chiari-like Malformations
