Object. Despite efforts to elucidate both the molecular mechanism and the clinical predictors of vasospasm after aneurysmal subarachnoid hemorrhage (ASAH), its pathogenesis remains unclear. Monocyte chemoattractant protein-1 (MCP-1) is a chemokine that has been firmly implicated in the pathophysiology of vasospasm and in neural tissue injury following focal ischemia in both animal models and human studies. The authors hypothesized that MCPA would be found in increased concentrations in the blood and cerebrospinal fluid (CSF) of patients with ASAH and would correlate with both outcome and the occurrence of vasospasm. Methods. Seventy-seven patients who presented with ASAH were prospectively enrolled in this study between July 2001 and May 2002. Using an enzyme-linked immunosorbent assay, MCP-1 levels were measured in serum daily and in CSF when available. The mean serum and CSF MCP-1 concentrations were calculated for each patient throughout the entire hospital stay. Neurological outcome was evaluated at discharge or 14 days posthemorrhage using the modified Rankin Scale. Vasospasm was evaluated on angiography. Results. The serum MCP-1 concentrations correlated with negative outcome such that a 10% increase in concentration predicted a 25% increase in the probability of a poor outcome, whereas the serum MCP-1 levels did not correlate with vasospasm. Concentrations of MCP-1 in the CSF, however, proved to be significantly higher in patients with angiographically demonstrated vasospasm. Conclusions. These findings suggest a role for MCP-1 in neurological injury and imply that it may act as a biomarker of poor outcome in the serum and of vasospasm in the CSF.

Advanced Techniques for Endoscopic Intracerebral Hemorrhage Evacuation: A Technical Report With Case Examples

Robert J. Rothrock, Alexander G. Chartrain, Jacopo Scaggiante, Jonathan Pan, Rui Song, Danny Hom, Adam C. Lieber, Joshua B. Bederson, J. Mocco, Christopher P. Kellner

Summary: This study summarizes the operative nuances of endoscopic minimally invasive ICH evacuation developed at a single center over an experience of 80 procedures. The results show a significant reduction in preoperative and postoperative ICH volume, with an evacuation rate of 88.7% and a 30-day mortality rate of 8.9%. It emphasizes the importance of academic discussion of specific surgical techniques to enhance safety and efficacy as experience builds with this procedure.

D. T. Goldman, D. Bageac, A. Mills, B. Yim, K. Yaeger, S. Majidi, C. P. Kellner, R. A. De Leacy

Summary: The study evaluated the safety and feasibility of transradial approach and distal transradial access for neuroendovascular procedures, showing that these methods are safe and effective for a wide range of neuroendovascular interventions with a high technical success rate. Despite some adverse events and complications, overall risks are relatively low.

National Trends in Utilization and Outcome of Endovascular Thrombectomy for Acute Ischemic Stroke in Elderly

Amol Mehta, Johanna T. Fifi, Hazem Shoirah, I. Paul Singh, Tomoyoshi Shigematsu, Christopher P. Kellner, Reade De Leacy, J. Mocco, Shahram Majidi

Summary: From 2014 to 2016, there was a two-fold increase in the utilization of EVT in patients >= 80 years of age. The rate of good outcome in patients over 80 was significantly lower at 9% compared to younger patients at 26%. Additionally, the in-hospital mortality rate for patients >= 80 was higher at 19% compared to 13% in the younger cohort.

Summary: This study evaluated the impact of COVID-19 on acute stroke screening using a commercial clinical artificial intelligence platform. Results showed a significant decline in stroke imaging screening during the pandemic, emphasizing the wider application of artificial intelligence neuroimaging platforms for real-time monitoring of stroke care systems.

Racial and Socioeconomic Disparities in the Use and Outcomes of Endovascular Thrombectomy for Acute Ischemic Stroke

A. M. Mehta, J. T. Fifi, H. Shoirah, T. Shigematsu, T. J. Oxley, C. P. Kellner, R. De Leacy, J. Mocco, S. Majidi

Summary: The study aimed to characterize disparities in the use of endovascular thrombectomy in acute ischemic stroke treatment based on race and income. Data from 2006 to 2016 showed an overall increase in endovascular thrombectomy use, with less significant growth observed in black patients and those in the lowest income quartile. The highest income quartile predicted higher rates of endovascular thrombectomy use and better outcomes, while black race predicted poorer outcomes.

Remote Patient Monitoring Identifies the Need for Triage in Patients with Acute COVID-19 Infection

Laura Tabacof, Jamie Wood, Nicki Mohammadi, Katherine E. Link, Jenna Tosto-Mancuso, Sophie Dewil, Erica Breyman, Leila Nasr, Christopher Kellner, David Putrino

Summary: Telehealth, particularly the Precision Recovery Program (PRP), was utilized during the COVID-19 pandemic to remotely monitor and support patients at home. A retrospective cohort study identified common factors (risk factors, vital signs, and symptoms) among patients requiring screening, triage, and hospitalization, providing valuable information for decision making. Implementing similar clinician-led remote patient monitoring programs may be beneficial for COVID-19 cohorts and other conditions at risk of rapid clinical deterioration in the home setting.

A Systematic Review and Meta-Analysis of Statins in Animal Models of Intracerebral Hemorrhage

Dominic Nistal, Muhammad Ali, Daniel Wei, J. Mocco, Christopher Kellner

Summary: The study found that statins significantly improved behavioral outcomes in intracerebral hemorrhage (ICH), with variable results in histological outcomes. This suggests that statin therapy may be suitable for randomized clinical trials in humans, and that the STAIR criteria can be effectively modified to evaluate preclinical studies in ICH.
