PURPOSETo compare myocardial sympathetic imaging using I-123-Metaiodobenzylguanidine (MIBG) scintigraphy and striatal dopaminergic imaging using I-123-Ioflupane (FP-CIT) single photon emission computed tomography (SPECT) in patients with suspected Lewy body diseases (LBD). METHODSNinety-nine patients who performed both methods within 2 months for differential diagnosis between Parkinson's disease (PD) and other parkinsonism (n = 68) or between dementia with Lewy bodies (DLB) and other dementia (n = 31) were enrolled. Sensitivity, specificity, accuracy, positive and negative predictive values of both methods were calculated. RESULTSFor I-123-MIBG scintigraphy, the overall sensitivity, specificity, accuracy, positive and negative predictive values in LBD were 83%, 79%, 82%, 86%, and 76%, respectively. For I-123-FP-CIT SPECT, the overall sensitivity, specificity, accuracy, positive and negative predictive values in LBD were 93%, 41%, 73%, 71%, and 80%, respectively. There was a statistically significant difference between these two methods in patients without LBD, but not in patients with LBD. CONCLUSIONSLBD usually present both myocardial sympathetic and striatal dopaminergic impairments. I-123-FP-CIT SPECT presents high sensitivity in the diagnosis of LBD; I-123-MIBG scintigraphy may have a complementary role in differential diagnosis between PD and other parkinsonism. These scintigraphic methods showed similar diagnostic accuracy in differential diagnosis between DLB and other dementia.

Prognostic role of baseline 18F-FDG pet/CT in stage I and stage ii non-small cell lung cancer

Francesco Dondi, Domenico Albano, Pietro Bellini, Elisabetta Cerudelli, Giorgio Treglia, Francesco Bertagna

Summary: The objective of this study was to investigate the prognostic role of baseline 18F-FDG PET/CT in stage I-II NSCLC. A total of 296 patients were included, and clinicopathological features and PET/CT semiquantitative parameters were extracted. The results showed a correlation between PET/CT semiquantitative parameters and histology, stage, size, grading, and presence of nodal metastasis. Kaplan-Meier analyses revealed most of the semiquantitative parameters as predictive for progression-free, overall survival, and disease-specific survival. SUVmax, SUVmean, SUVlbm, SUVbsa, and S-L were identified as independent prognosticators for overall survival and disease-specific survival.

Summary: The aim of this study was to compare two different PET/CT scanners for predicting the histological classification of NSCLC using RaF and ML. 227 patients were included and RaF were extracted. kNN and LR models showed the best performances for all scanners, with kNN outperforming the others. The choice of PET/CT scanner had an impact on RaF performances, with scanner 2 showing lower AUC values. In conclusion, this study identified promising RaF and ML models for predicting the histological subtype of NSCLC, while considering the influence of the scanner type.

Effects of vitamin D supplementation in obese and overweight children and adolescents: A systematic review and meta-analysis

Antonio Corsello, Marina Macchi, Veronica D'Oria, Chiara Pigazzi, Ilaria Alberti, Giorgio Treglia, Valentina De Cosmi, Alessandra Mazzocchi, Carlo Agostoni, Gregorio Paolo Milani

Summary: Due to the lipophilic nature of vitamin D, overweight and obese patients have an increased risk of inadequate circulating 25-hydroxy-vitamin D concentrations. Although supplementation strategies of vitamin D for pediatric subjects with excessive body weight have been proposed, the efficacy remains controversial. This systematic review and meta-analysis found that vitamin D supplementation slightly increases 25-hydroxy-vitamin D levels in overweight and obese children and adolescents, but its effects on metabolic and cardiovascular outcomes remain controversial.

[18F]F-Choline PET/CT and 4D-CT in the evaluation of primary hyperparathyroidism: rivals or allies?

Francesco Fiz, Gianluca Bottoni, Michela Massollo, Pierpaolo Trimboli, Ugo Catrambone, Lorenzo Bacigalupo, Sergio Righi, Giorgio Treglia, Alessio Imperiale, Arnoldo Piccardo

Summary: Primary hyperparathyroidism is a difficult-to-diagnose condition that can remain asymptomatic for many years, causing long-term consequences such as osteoporosis and renal impairment. First-line diagnostic methods like ultrasound and parathyroid scintigraphy have unsatisfactory detection rates. Second-line imaging methods like [18F]F-Choline PET/CT and 4D-CT have shown higher detection rates and sensitivity, but are less commonly used. Both PET and 4D-CT have specific advantages, limitations, and applications. This review extensively describes the pros and cons of these two techniques and explores the potential role of combined examination in diagnosing parathyroid tissue hyperfunction.

Diagnostic tests for medullary thyroid carcinoma: an umbrella review

Pierpaolo Trimboli, Caterina Mian, Arnoldo Piccardo, Giorgio Treglia

Summary: This systematic review summarizes the more robust evidence about the performance of tools useful for diagnosis of medullary thyroid carcinoma (MTC) such as calcitonin (Ctn) and other circulating markers, ultrasound (US), fine-needle aspiration (FNA), and other imaging procedures. The findings suggest that Ctn is the most reliable diagnostic marker for MTC, while CEA doubling time is more reliable in identifying MTC cases with poorer prognosis. Ultrasound sensitivity in MTC is suboptimal, and cytology can detect MTC in just over half of cases. PET/CT is useful for detecting recurrent MTC. Future guidelines should consider these evidence-based data for thyroid nodule management and MTC diagnosis.
